When should you be screened for prostate cancer? Check out my interview on CBS LA , 6-11-24
The age at which you should start prostate cancer screening depends on your risk factors, which include:
Age
The risk of prostate cancer increases with age, and after age 50, your chances increase even more.
Race
Black men have a higher risk of developing and dying from prostate cancer. Black men should start screening in the their early 40s. We need to encourage and welcome black men into our medical practices.
Family history
If a close relative, like a father, brother, or uncle, was diagnosed with prostate cancer before age 65, your risk is higher than average.
Inherited gene mutations
Some genetic mutations, like BRCA1, BRCA2, ATM, HOXB13, MLH1, MSH2, or MSH6, increase your risk. These can be...

Rezum water vaper therapy helps men suffering from BPH. Check out Dr. Mehan on ABC15, Sonoran Living
Просмотров 1527 месяцев назад
As men age, they often experience excess growth of prostate tissue. This condition is called BPH or an enlarged prostate. The prostate is a small gland located at the base of a man’s bladder, and the urethra passes through it. When extra prostate tissue develops, it obstructs and compresses the flow of urine from the bladder through the urethra. In severe cases, BPH can completely block the flo...

Will they let you take the ultrasound probe home?
Amazing how it works and I've been through that procedure recently in February 2024.
Thanks for sharing your comments. My aquablation procedure is set for three weeks. The more I know, the less I worry....
Had it in Jan 24. Had a 205gm prostate. Amazing results.....
I found this fascinating! I had Aquablation in May of ‘24 with excellent results. Thankfully, no Euroclips in me. Watching this before my procedure made me feel totally at ease. I experienced absolutely no pain. There was a lot of discomfort from the catheter at the entry point which was gone the moment the catheter was removed. Spent the night, released the next day with first follow up two days later when the catheter was removed. Resumed normal activity, and I’m a very active 74 year old, when the urine ran clear after being quite red, then pink and lighter and lighter. It’s important to check the semen for bleeding as well. Finally, I loved listening to Dr. Mehan’s play by play commentary. He really is an artist!
Dr. Mehan completed my Aquablation procedure on July 11th. I had the exact same experience that you had no blood on day 9, no pain other than the annoyance of the catheter. Went to the Hollywood bowl last night and peed once. 68 years old and it is a whole new world.
Can anyone tell me if aquablation is available in Washington State?
That is awesome. I just set an appointment for a consultation to get this done.
Estou para ser operado com essa tecnica alguem me pode dizer em questao á vida sexual fica normal?
As of 2/7 the day of my surgery, I have been out for 2 months with pain and on UTIs and running to the restroom which sometimes I don't make it. I don't know now if it was worth it or not. Please people do your homework before getting this done. I may lose my job over this. I was not counting on being out this long. My sex life is non-existent now.
Have you made any positive progress?
@@Bonesmcgreeny 50/50 Going to the restroom got better but I sometimes stop urinating out of no wear. Blood clots get stuck sometimes if they are too large.
Anyone could tell how much does this operation cost?
This is covered by Medicare and most insurance plans.
@@doctormehan7218 Thank God for that because it isn't cheap. However, it is worth every penny.
Wow! I am a new patient of Dr. Mehan's waiting to get scheduled for aquablation. I have now enlightened myself as to what I will be going through. I am fortunate to have Dr. Mehan. I have confidence that I will be getting the best care available.
Wow Great video
Hello, I'm contemplating Aquablation Surgery now.❤❤.
Im getting it too Had the same chronic UTI s. On flowmax now, which is great, but Im ready to retire and want to be free of meds.
Hello Dr. Mehan. I'm 69 years old. My IPSS score is 32. According to my urologist in Washington State, my prostate is 98 grams and I have a large intravesical protrusion and no median lobe. Due to the size, he told me I am a poor candidate for Urolift or Rezum. I'd like to maintain antegrade ejaculation. Would Auqablation be a good procedure for me? Do you suggest patients bank their own blood due to possibly needing transfusion in case of bleeding? If I come to Mesa for this procedure, how far out are you scheduling? Thanks.
I try not to give medical advise through RUclips comments. In general, Aquablation is a great option for men with large intravesical protrusions from BPH. This is the best resective procedure for men with a larger prostate who want to minimize their risk of having retrograde ejaculation. In my series, this risk is around 7%. Bleeding is also a risk, but in my series, the risk is very low, less then 1%. I do not have patients bank blood before as this is not needed. I see patients from all over the US and beyond. I'm booked for surgery this year, but could see you in January if you are willing to travel. Please call my office at 480-219-1010. I wish you well with whatever you decide.
@@doctormehan7218 Thanks for the quick reply. My insurance will be changing in January and will cover your practice at that time. I'll call your office tomorrow.
Hello James, I am also 69 years old with a enlarged prostate (don't have a measurement as of yet, seeing a urologist soon). I was wondering if you had the procedure aquablation and how it worked out for you, as I am going to ask about it when I see my urologist. I am also in Washington State and was told Overlake has the machines for this proceedure so hopefully would not have to go out of state.
@@jamesdavis2284 do it James. You will be pleased with the results. I know I am....
I know this was 9 Month's ago (I guess), I was watching the GreenLight Laser Treatment / Procedure on this Channel. I have a question for Doctor Mehan, or someone whom may know. Does the Size of the Package make any Difference as to how difficult the procedure can be? IE, Being a bit shorter than what it was in the Past. Thank you in advance.
My urologist first made sure my urethra diameter was sufficient for the robotic instrument itself and the post-surgery catheter. He stuck a scope in there in an office visit to check that out and examine the prostate and bladder neck. I am not particulary well-endowed but the surgery was very effective. Wed afternoon - surgey Thur afternoon - release from hospital with catheter Fri morning - catheter removed in doctor's office. Drank a lot of water for the next week to flush things out. No pain. That was on Nov 1st. I feel terrific. I'm 71.
@@yaneznayu9997 Thanks for the Feedback, I have another Appointment to go over my Blood work up and see what's what. My PSA went up from 1.9 to 2.05 on this yearly Physical exam. I'am just not sure of any Issues with any Cancer at this point. And, If the GreenLight Laser treatment is performed, will it Interfere with Prostate Surgery down the road? Thanks again. I'll be 70 this Feb 2024.
Good luck. That PSA sounds super low, though as I underrstand it, it's not widely seen as a relaible marker for cancer. Mine was 3.5 and went to 7 once due to a UTI. if I ever get prostate cancer, I'm just going to have the hollowed-out little fellow removed. The doc asked me if I cared about ejaculation. I didn't give a crap as long as erections and orgasms weren't affected and told him to bring his biggest apple corer. He said he could remove more tissue this way and at my age I wouldn't have to worry about regrowth.
@@yaneznayu9997 With all these Newer Technologies that are available now day's It just amazes me that the System don't want to just do what's right? "IE" do the PSMA / PET scan, get the best Information available and let the patient do what's right for them with the Doctors guidance? I think at our ages, the Insurance companies just want us DEAD. I don't think they even care often times. I been Paying Into this system since I was 15 years old, that's 55 Years of TAXES and such, I feel I deserve the Care! And they should just give us the treatment that we paid for all this time we were working and Paying into the "System"! Hopefully if things go right, we will be around for a while longer. Ya never know. I had a Heart Attack 3 Years ago with No Symptoms at all until BAM, It hit and I was Lucky to have the good doctors that Helped Keep me around for my Grandchildren !
Glad the dude is happy with the results. I believe Im a candidate coming up by the way I pee. I was wondering what type insurance covers this. Medicare and a supplemental insurance?
I had this 3 weeks ago. Medicare covers it and my Plan F medigap covered the final 20%. Peeing easily and normally after 9 years of flowmax. I'm 71. No side effects, trace blood i urine for 2 week. Easy peasy.
yes its covered by medicare i just had it
I had enlarged prostate 105cc. My IPSS score was 25 (bad symptoms with poor quality of life). In May 2022 I had an aquablation to remove excess. Huge difference. My IPSS is now 4. Back to normal. However, I now have stage 3 chronic kidney disease. The reason is I waited too long to get the surgery. For a couple of months my bladder was not emptying fully. I did not realize this was a danger but more important none of my doctors warned me of the danger. Before surgery my blood test showed dangerously high creatinine level so doctor told me to go to ER and get catheter in to empty bladder. The catheter worked, my bladder emptied and my creatinine level came down but the damage was done. If you have an enlarged prostate that is making it hard to pee you need to tell your doctor/urologist. He can check to see if your bladder is emptying. I highly recommend aquablation for fixing enlarged prostate.
So then the aquablation is like a pressure washer ablating tissue? then you have to follow with the TURP to shave the fluffy tissue smooth. Or at least in this case of 280 grams. But what about patients who are 80 grams or less?
I had this done a year ago and side effect is Retrograde ejaculation . Why the best doctor in the region could not prevent the retrograde ejaculation ?
I understand that a very small percentage can have that.
@@americanazheck Less than one in ten get it...
I recently turned down a Urolift recommendation. Patient satisfaction is 2 out of 5 for Urolift. Still checking options.
I had urolift done 18 months ago and I am already deciding on which treatment I want to do next. The urolift is not very effective
Most irrelevant comparision!
That's just a donut.
How do you remove those pieces of urolift once you remove them?
It is washed out through the scope! The urethral end piece is actually quite small, around 8 mm.
does the operation with aquabeam preserve all the functions? to what extent? Furthermore, does it make it more difficult later to ascertain any prostate cancer that may have occurred or not? I also heard that the aquabeam procedure makes radiotherapy unfeasible.
In their FDA trial, the risk of urinary incontinence and erectile dysfunction afterwards is 0%. There is a small risk of permanent retrograde ejacualtion at 7%.
I recommend Aquablation to men with prostate cancer who desire radiation treatement, but are now suffering from bothersome lower urinary tract systems. It is important to open the prostate up before radiation is given.
@@doctormehan7218 Doc, what if the cancer patient has had radiation already? could patient benefit from the Aquabalation surgery?
Im gonna ask my doctor about getting this done👍👍👍
I'm confused also 🤔 i thought it's 1 or The other Turp & Aquablation.... Hmmm...
At the end of the Aquablation procedure, I use TURP to remove Urolift implants and for focal bladder cautery to minimize bleeding.
How much money 💵 there in the USA ?
I have treated a large number of men from outside the United States. The rate is determined by the hospital. Please contact my office and we can discuss further if you are interested
I heard there could be bleeding...😮 1 women on another video said her husband had severe bleeding was rushed to hospital.
Bleeding is common afterwards, but this will resolve with time. Severe bleeding is rare. We published a paper that showed our transfusion rate was less then 1%.
After my aquablation I was kept overnight and my bladder was flushed out with lots of saline until the fluid was mostly clear of blood and clots. I went through 81 liters of saline. Poor nurse was changing a 3 liter bag about every 45 minutes. Flushing helps prevent clogging. I went home day after surgery. Catheter was removed 3 days after surgery. Very little blood in my pee. Pee was all clear after about a week.
Didn't the surgeon know that the patient had had a urolift? He acted like he was surprised to see the urolift.
I was surprised! I large number of my patients travel to Arizona for surgery with myself. This patient failed to inform before had that he had the Urolift procedure in the past. It worked out just fine.
Which procedure has least side effects (preserved sexual function, retrograde ejaculation, better urinary function) aquablation or green light?
Does it shrink the prostate or carve it out, like a pumpkin?
🎃
strait shooter. I like this guy.
No pun intended?
@@davidjaeger492 lol
I'm still confused as far as where is the probe in the flushing being done... are you in the urethra and what is the scraping scraping off.... if it's in the urethra why does scraping of the urethra is needed when it's being pushed in by the prostate gland or are you inside the prostate gland scraping or are you outside do urethral I don't get it🙄🙄🙄🙄🙄🙄🙄🙄🙄
I’ll work on making a video to best answer your question. This procedure totally obliterates the urethra in the prostate. The body will form a new urethra in the resected channel of the prostate afterwards
@@doctormehan7218 Very interesting. Thanks ... 👍
Does that orange part include two ejaculatory tissue when mapping? The Aquablation procedures preserve two ejaculatory tissue or only one when contouring?
maybe it goes a bit left and also to the right of the VM, having this the 29th - but mine is only like 65g
@@a2b3c Can you tell the users directly, the Aquablation procedure preserves two ejaculatory ducts or only one?
@@glongshotit does
I've been doing alot of research they say this is the best procedure. Here in IDIOT Canada only 2 place do it Ontario & Montreal not covered by our shit medical 1 place wants $ 22k 😡 the other if not a mistake $ 6,500. But pretty booked up 😮. I'm starting to look at the Philippines.
Rezum I think is very similar but use hot steam can damage nerves but Aquablation uses cold water spraying the Prostate tissue away like blasting water into a sand hill.
Terrific presentation! Dr. (Cure!) Mehan+++++
Be confident when you visit with Dr. Mehan at his office or in a conference call from wherever you live that his advice is "ROCK SOLID ". Be fully confident he will guide you in the best manner your medical condition requires. His reputation precedes him and is of a skilled surgeon with expert training in urology. I found him to cheer lead: " LATER FOR THE OPERATION UNTIL WE EXHAUST ALL OTHER MEDICAL MEANS AND OTHER RELATED HOLISTIC SUGGESTIONS IF AVAILABLE BEFORE ANY WELL TRIED SURGICAL TREATMENT". Furthermore I concluded over the years I have been his patient, that his experiences are deep in any required aspect of the patients needs associated with his special skills, as well as having a foundation of genuine care and concern for his patients with whom he exudes great interest. In my experiences one could not expect more from such an accomplished medical expert in this field! I also enjoy his company in the exam room itself and implicitly trust his advice .
