Mine started doing that also, I disassembled it and put it back together (with the help of a RUclips video and much rewinding of the video) and the slide still would not go all the way back, I started scratching my head and a dull light came on inside my head and I figured out it started jamming when I installed a side ammo holder and Picatinny rail on top, so I removed the four screws that came with the kit and presto, fixed it, the screws that came wit the kit are to long and they were hitting the slide. I bought the shotgun used around 8-9 years ago and I haven't even fired it yet, was sitting in my closet, it was pretty dusty/dirty so this gave me an reason to give it a much needed cleaning.
