    Improvised tourniquets are a great way to teach why you really want a good manufactured tourniquet. If you show them the "real" TK , then show them what it takes for an effective improvised TK, give them the building blocks and show them the downsides in comfort, security, and speed , they'll be more likely to carry a real one and better prepared for when they have to go with option 3. Talking down to people and saying " because xyz said so" is a horrible training model.

    Excellent demonstration! I’ve seen this argument a lot online but never knew the how and why behind the right answer. FYI: your subtitles incorrectly read 1-2 mL when you said “1-2 liters” at 1:54. Not nitpicking but it undermines the point you’re making. Not sure if it’s fixable since the video is published though.

    End up arguing with so many idiots online about this, especially when they claim the whole reason tampons were orginally designed was for musket wounds (no it really wasn't) Going to link them to this video in the future since it does great at explaining and showing that tampons are rubbish for bleeding control.

    I'm going to dive into this, and I hope you don't take this the wrong way. First- the absorbancy isn't the goal- packing is about providing pressure on the bleeding artery. This is accomplished in two different ways-direct pressure and hematoma effect(ITclamp etc) . Second- the reasons the tampon discussion exists in the first place and persists are numerous but form factor, lack of studies, and the origin of their initial use ar all part of it. Third there's absolutely no reason to plan to use them today as opposed to 30 years ago, but that doesn't necessarily mean there wasn't a valid , although twisted, beginning for their use. To be honest I truly wish someone would come up with a more reasonable product than Xstat in that kind of form factor for narrow wound tracks- celox rapid ribbon in an xstat 12gm applicator or slightly larger....

      Hi James, if you watched the video you will notice that the tampons, even when packed with direct pressure on the artery, provide no occlusion. This is because absorbency is what they are designed for, not for occlusion like Kerlix gauze.

      ​@@greenfeetmedical7735I did, and a have seen a dozen others like it in the last 10 years. Yet every six months or so this still pops up. You can't replicate hematoma effect in an non clotting fluid outside of a living being. Until an honest attempt at doing this in a porcine model is completed ( one was attempted by Dr. Scott Oschenbein in Nashville, but ended after a lot of threats) it will continue. Here's the thing direct pressure relies on volume of the packing material( why quikclotxl performed better than standard, and kerlx was within the margin of error in the navy study) and the ability to manipulate it to the point of bleeding. I'm not trying to argue here that kerlix should be replaced or supplemented by a tampon. What I am saying is that talking down to people with the same talking points that were used on you is ineffective. Your platform, your choice...

      You got us! But realistically packing a lightweight kit for each use case is up to the individual. A fully decked out med bag should and would have an eyeshield.

      Eye shields aren't the definitive intervention for eye medical/trauma. Gauze rolled into a donut can be just as useful in these cases. Thanks for watching.

      Hi! This item can be used as a pelvic binder as well, however if a broken pelvis is suspected you could always use it above the pelvis on the aorta.

    If the orignal woundpacking gauze doesn't take. do we 1. unpack the original and replace or 2. pack more gauze on top and add a pressure dressing?

      You rip it all out and repack it. Thus is why it's important to carry lots of gauze. Hope that helps!

      @@Vinnytwotimeik this is an old comment, but I think there’s a serious problem with that approach: you’re actively breaking all the formed blood clots and removing all pressure, so it’s just undoing and wasting what you did already while the casualty still keeps bleeding? if the original wound packing was done with poor technique and there’s still a large cavity inside, you can just keep packing and fill that cavity with additional gauze. If the wound has been packed with proper technique and blood is still seeping out, I’d place more gauze on top of the wound site, and continue to apply direct pressure.

      @@Mike-ww3bq The reason why that doesn't work, is because the infiltrated vessel has not yet been occluded. When the artery is compromised, we create a powerball, find the bleeder with our finger, and replace our finger with the gauze. Kerlix and other hemostatic gauzes are designed to occlude, not absorb. So, when you pack sub-par and it starts to bleed through, you need to take it out because the gauze it just a sponge now. It no longer is doing it's job. If it is bleeding through the gauze then there are no clots that have formed. Does that make sense? I'm currently a licensed paramedic, TP-C, tccc instructor, stb instructor, live tissue instructor, reconnaissance medic, and other things.... I see junctional wounds often and this is the only way to get it to stop bleeding. You'd be amazed how much gauze it could take.

      ⁠​⁠​⁠@@VinnytwotimeI guess I’d agree with you then regarding swapping out the gauze if the wound packing technique wasn’t good and there wasn’t direct pressure on the artery, which you’re absolutely right is what’s actually needed. But would you still say the same for the scenario where even with good packing technique, blood is still seeping out? As far as hemostatic gauzes go, shouldn’t you still maintain direct pressure with your hands, because it can take around 3 minutes to fully form the clots. If even then it was still bleeding, would you not add more gauze on top and maintain strong pressure, as per the manufacture’s instructions. Just ripping it out and starting again is kinda hasty in my opinion?

      @@Mike-ww3bq Technique is good, but it's still bleeding? I don't quite understand the question. If it's bleeding through your gauze, then the technique wasn't done correctly. Now, does blood slightly soak into your gauze when first applying, yes. But you have several yards of more gauze to continue packing. As far as direct pressure, for hemostatic it's 3-5 minutes and for kerlix it's 5-10 minutes. If there is too much blood in the wound cavity, you can also "scoop" it out. I find taking a few 4x4s and "sweeping" it out works OK. I hope all of this makes sense. Would love to explain this in person. I'm in South Texas if you ever get the chance.

    I've had to do this to stop myself from bleeding out after severe trauma to my lower arm and it's almost impossible to get a good application with a sleeve in place. Take the 2 seconds to remove any clothing and prevent having to remove the TQ and re-apply, which hurts even worse than the original application.. Even after clothing removal, I still had a bit of seepage. I was using a CAT II. Did plenty of TQ apps on other people but it's much harder to do on yourself right after trauma. I'm trained up on CAT and RAT style TQs but when my own life was on the line, I went straight for the CAT.
