Просмотров 1964 года назад
This presentation was given at the L3Harris 2020 Safety Seminar on the 25th Sep 2020. It provides an overview of the Risk Culture Framework based on Sir Charles Haddon-Cave's '4 States of Man' concept and also an innovative approach to organisational culture assessment which proposes the 'Continuous Monitoring of Risk Culture' by introducing a 'Risk Reporting Programme' using a unique data coll...

Dr. Bill Johnson's Video Message on FFP @ the IFA Conference @ Emirates Engineering 27/28 Nov 2018
The dumb camera man is more focused on showing his useless mask than where his camera is pointed.
@6:39 when the standby rudder shut off valve fully opens, valve position relay K8 energises to open the circuit to the low pressure light and it extinguishes - not because the standby hydraulic pump is powering the rudder. The low pressure switch is located within the respective flight controls hydraulic module downstream of the flight controls shut off valve and is only exposed to system pressure (A or B). The standby hydraulic fluid lines don’t connect to flight controls hydraulic modules
@3:25 you talk about the safety precaution with regards to turning system B on incase the steering by pass pin is not installed. I understand the point of your video but if you are going to talk about safety you should really point out at the start of the video or at some point before you pressurise a system that there is no one around the aircraft and all areas are clear because you are pressuring hydraulic systems at will without asking for a clearance to do so. Number one rule before turning on a hydraulic pump is to get clearance from the ground crew to ensure everyone on the outside of the aircraft is clear. Without getting clearance someone could be seriously injured or killed
WOW, just worked out what you mean by 360 view in the cockpit - well done
@8:44 PTU is located on the keel beam in the main landing gear wheel well
@7:53 never knew the reason the guard was orientated the opposite of the others on the panel - good info I wonder why they didn’t do the same for the CSD disconnect switches - that can only be reset on the ground with the engine shut down 🤔
Nice.
Solid camera work.
Hi where can i find the full video?
The final statement is that defences are not enough …so what are we to do?
We put more defences which can help in closing holes
I like a “turtle diagram” exercise from the ISO 9001 quality standard, to help identify where a risk for escape exists in our defense barriers.
By asking in the surveys specifically to think of a risk taken or rejected for the stories in the surveys, do you think you create bias that may show risk taking overweighted? By this I mean that normal ops and normal times should far exceed the thought of risky moments though these don’t get captured. Hence a risk accepted survey may artificially slew downward and perhaps right while a risk rejected survey may slew artificially upward and perhaps left. As such, I’ll assume you want to merge the two before asking ‘less like these more like those.’
I love these old time airplanes. Probably 1960s, now replaced by the airbus a220.
Well, I’m flying the 737 max that is quite recent and the system is the same…
@@AandrewB. wut? That's madness. The max needs sharp pilots. Hope your sched is treating you well
Great video
Excellent video!!! Love the 3D
THANKS CHRIS! VERY INFORMATIVE AS USUAL...
thank you capt,
If only the captain knew
He knew...if only he had sat on the pilots seat instead of Bonin. Because Robert wanted the controls but Bonin still controlled the plane after giving him the controls.
It’s not as clear cut as that far from it however I do believe had either correct CRM been adhered or as you state the captain removing Bonin from his seat & taking control it would of been very likely recovering the flight & to its flight path. However there is huge factors of what was going on that caused these pilots to get it wrong.

Holy shit didn’t this guy lead the heavens gate cult.
Very informative 👏 👌 👍
appeared like pretty sweet timing to me !
I couldn't find a better example of the Reason´s model as this short but smart video . It shows in 4 minutes the very intrincated human nature prone to error. I was looking for a short educative material and *voila* ! I found it. You will never know how many accidents or serious incidents has this video helped to amend on some latent and active failures existing in the organization to which we will present our Air Safety workshop the next week. Thanks for uploading this material.
Thanks for sharing this interesting video!
This isn't a runway incursion. The large jumbo took too much time to take off as the incoming plane closed the distance. Because there was not enough separation, the incoming place was waved off by the tower since landing while there is still turbulence from the departing jumbo is unsafe.
thx so much
Thank you
