    I couldn't find a better example of the Reason´s model as this short but smart video . It shows in 4 minutes the very intrincated human nature prone to error. I was looking for a short educative material and *voila* ! I found it.

    You will never know how many accidents or serious incidents has this video helped to amend on some latent and active failures existing in the organization to which we will present our Air Safety workshop the next week.

      We put more defences which can help in closing holes

      I like a “turtle diagram” exercise from the ISO 9001 quality standard, to help identify where a risk for escape exists in our defense barriers.
