    Doctor, how much does electrical muscle/nerve stimulation helps in treatment of facial palsy in the initial months? I have had a Grade 6 facial palsy 2 months ago and been having electrical stimulation along with excercises and stretching and I am now thinking to stop stimulation and only do exercises as I have had 60-70% recovery. Would you advise to continue or its okay to stop?

      Hello. Grade 6 means you have complete flaccid paralysis, so no movement, and no return of nerve conduction to your facial muscles. You should not try to make facial movement at this point. Neither through the us of electrical stimulation nor "exercises". IF there is no conduction, it will not make your face move....and that can lead to reinforcement of mass patterns, which could make synkinesis worse if you were to develop that later. In this stage, gentle massage and stretches are recommended.

      Thank you doctor for such a prompt response, really appreciate it. When does synkinesis usually surface in the timeline, if it develops, since i had already started with electrical stimulation as per doctor advice in the first week so just wanted to look out for any symptoms of it.

    Will you please post a video on how to see "overactive buccinators? Also, underactive- I am having difficulty seeing this in a patient. I am seeing asymmetry in the smile and trying pin point the area.

      @@ButheauPhysiotherapy yes i realize this after several years i have been dealing w spinal issues and have weakness of the nerves muscles everything on the left side I also have scoliosis but have gotten several things fixed . But after all these years i have realized myself it was weaken nerves so my muscles atrophied in my areas im working so hard to heal. Its effecting my vision in the left eye also but i can feel pressure behind that ear . Im trying hard to get into U of Ms Facial Nerve Clinic but we are plying phone tag. Its so defeating some times but i keep pushing . Ty for ur videos ♥️

      Sorry to read this. Actually facial palsy is from a peripheral nerve injury to the facial nerve, it is not a central nervous system injury , like a stroke is. so all the concept I discussed in my video are not applicable to a stroke. I hope you are getting appropriate rehab from an appropriate therapist.

      @@marmeebean if you have been dealing with this for over two years, you possibly and likely have some synkinesis or other movement pattern problem, and so your unique presentation needs to be assessed to determine what should be efficiently worked on for best results… my two cents.

      @@ButheauPhysiotherapy yes, I do have synkinesis - most movements of my mouth cause my eye to close. I'm scheduled for a tarsorraphy and a gold eyelid weight in October. But I will also work on regaining my smile with your post. I've seen so many doctors and health practitioners - the oprhthomologist is the first one suggesting specific procedures for my eye.

    When would you suggest doing facial exercises for bells palsy? Today is day 7 since i was diagnosed. And at these early stages what would you recommend doing?

      I would never do "exercise" for the face...Ther his no such things since the muscles of the face behave differently than muscles in the rest of her body. It is very early for you now and do not attempt to move your afce if there is no movement. You can't use an appliance if it is not plugged in or if there is no electricity.... Massage and gentle stretching should be your routine at this point.

    @ButheauPhysiotherapy Hello Doctor, saw in one of your replies that if synkinesis happens its in the 4th, 5th, 6th month or later. Thank you for that reply. Is there a time duration, post onset of facial palsy, after which we are out of the woods and need not look out for any synkinesis symptoms and can say with relief that did not develop synkinesis?

      There is not really. But the 2/3 of people that recover from BP without complications generally recover in 2-4 months.

      @@ButheauPhysiotherapy sorry doctor, didn't quite understand. Talking about synkinesis here. You mentioned in the video that you didn't develop synkinesis post recovery. When did you conclude that there is no possibility now of any occurence of synkinesis? Is it after 6 months of post recovery or post onset of palsy or 12-18 months post recovery of palsy? What is that time duration to conclude that synkinesis cannot happen now?

    Thank you, doctor, for this valuable information. I have a tilt in my lower lip to the right. I have not found the appropriate diagnosis yet. I am currently doing Botox on the effective side to weaken it, but the result is still not satisfactory for me. I am looking for a permanent solution. What do you advise me?     Hi Dr, suffering from facial palsy due to trauma leading to 7th nerve inflammation. My affected eye's lower eyelid had retracted down showing more of the white. My eye closes now in week 7 that is currently ongoing. With time,would the lower eyelid go back to its normal position? Are there any exercises to help with that? Or is surgery to pull up the lower eyelid the only way to go?

      Lower eyelid contraction generally returns after upper eyelid. It is slower to heal. There are some targeted stretches of eyelid you can do to help with that. But to do this correctly, the whole picture has to be looked at. You are also in week 7… your nerve is not done healing. Give it time.

      The tension on the facial muscles, which includes the auricularis, muscles of the ear, can affect hearing and the feeling of clogging one may have following Bell’s Palsy. Targeted stretches could help but It could also be other thing that need to be evaluated by an otolaryngologist

    Hello Pierre, Haalo again, today is day 4 of my Bell's Palsy and seems like mine is Lower Motor Neuron affected, most likely at the level of Stylo-Mastoid foramen and beyond. So, today, as I am waiting in my truck at school, I felt current flowing thru my RT cheek muscles and it came from behind the RT ear and came down to the mid-level of cheek and it went up and did that running around at least 8-10 times and it stopped. I have been doing exercises since Tuesday morning and today is Wednesday afternoon. My Acyclovir and Prednisone is round the clock. Heat, massage, exercises of various cheek-groups...all of that. But to feel that random current flow in my cheek muscle, is phenomenal to me. What would you consider this?

      I cannot answer this. I have not evaluated you fully. You are on day 4… take your meds, massage… DO NOT “exercise” your face.

    Hey Doc, are there any specific exercises or stretches that would help with squinting of eyes or scrunching of eyes when we tightly close the eyes? Currently the eyes close but squinting and scrunching are hardly there

      you should not attempt tightly closing your eyes. You just have not regain the tone and muscle contraction adequate to do that...I would give it time. We never tightly close our eyes...unless we have soap in our eyes.... I do not know more about your condition to guide you specifically in this.

From your reply, does that mean that, with time and not attempting any shrinking and tightly closing my eyes, there is hope that I would be able to tightly shut my eyes with normal wrinkled eye lid? And also able to shrink my eyes when looking into bright light?

    I was diagnosed with bells Palsy 2 months ago and now it is recovered almost 90% ...but i am having very irritating noice in my ears ( sometimes in one and sometimes in another ) . Doctor also not prescribed MRI test ....pls sir let me know is this noice because of bells palsy or any other disease..i will always be grateful to you.

    I just got diagnosed yesterday. Started with a bad taste in my mouth last week, and a few days later, i couldn't move the right side of my face. They gave me prednisone and some antibiotic to take. Idk how i set it off. Some said it could've been from the bad tooth in my mouth or from cleaning with ammonia. They can't really say. I don't have really have a medical history because there hasn't been anything seriously wrong with me.

    I got bells palsy in 2019 without doing physiotherapy i am cured 100 percent but later after that 2024 i got facial palsy but this time it is taking more time its been 75 percent is cured after 1 month but my smile is taking too much time my upper lip is not raising to show full teeth on left side ,my left nostril is coming down compare to right side ,i cant able to lift my nose muscle it is very hard

    I got Bells Palsy over 5+ years ago. I never got proper treatment for it. 5+ years later i still have Synkinesis. I'd say my face is about 85-90% normal. Do you think it's too late to improve to get it closer to 100?

      no it is not. I have seen people improve several decades after the onset of their facial palsy.

      I cannot give a detailed answer if I have not assess your current situation. Many things to look at. Is your movement affected because of lack of return of nerve function, or because of tightness and synkinesis blocking your movement? I work with some people that have been dealign with this for several decades, and we tend to always be able to make some progress. But again, I don't have a full picture and cannot be specific to your case.

    I had Bell's palsy almost 20 years ago. I recovered after 3/4 months but I think I developed synkinesis around that time but I was too young so I didn't understand and pay attention to it. Now I'm very self aware about this and feel really really insecured. Is there any was to fix it now 😢

      the first step is being evaluated by a facial therapist. You might be appropriate for it or need to be referred, that really much depend on your current situation, which I do not have a full pictures of.
