    Yup I lost 50 pounds in 3 months last year because when I eat I get pain. There’s been no diagnosis yet for me and all the doctors just want to give me different prescriptions but they don’t know what they are treating. It’s extremely frustrating on my end. Not sure what to do. Any advice would be greatly appreciated thanks.

    I have a 70% MAL compression. I also have 4 aneurysms. 1-Splenic artery, 1- right hepatic artery, 1- left hepatic artery and 1 gastroduodenal artery also. I had a craniotomy at Emory in 5/2022 to clip 6 brain aneurysms. I was 58. My weight has dropped from 155lbs to 90 lbs. Do I need surgery for the MAL? My vascular surgeon seems to think so. And why do I have 10 aneurysms? No family history, I am autoimmune with Hashimoto’s and I have severe osteoporosis. My body does not produce enough collagen. I’ve had high blood pressure since I was 30. Had Rectal Prolapse surgery in 2/2023. Any response would be appreciated. Thank you 😊

      I’m sorry for all that you have gone through. About 1/3 of my patients with MALS have a connective tissue disorder, most often Ehlers Danlos Syndrome, which is a collagen vascular disorder. It affects the integrity of your soft tissue, including the collagen that makes up your blood vessels. The walls are weaker and more likely to form aneurysms. If you have lost that much weight, it is very likely to be caused by MALS and also likely to get better with surgery. You need to complete the full work up and make sure you go to somebody experienced.

    The question is, which specialist to see. I have seen two vascular surgeons that have confirmed i have mals, but they disagree at to wether or not that is causing my symptoms. I still have to rule out other conditions before i proceed with surgery. I have had a very hard time getting to see a GI doc, my appointment is four months from when i got my first symptoms. I went the the ER at 3 different hospitals but they all focus in on Mals and disregard looking at other possibilities. They read my “my chart” and say “ok he has mals send him to surgery with vascular”. But im old, 60 im a male, and my symptoms came on overnight. All of which are very unusual for mals diagnosis. WHat i hoped was going to be a quick, diagnosis and treatment is turning into a long journey. But thats ok, i figured out that i can live off of 500 calorie boost shakes 4x a day and maintain my weight. I have lost over 25 lbs and cant afford to lose more. Sorry for the vent here, i hope this helps somebody else going through this and having a similar journey. Larry

    I was diagnosed with mals 2 months ago and i wish i had come accross this video at the beginning. After 4 different hospitals and 3 long ER visits, doctors still disagree if i have mals or something else. I think the important thing is to eliminate all other possible diagnoses before proceeding to mals surgery. I have had two ctas, two ultrasounds, one EGD and lots of blood work. I have an upcoming celiac block. I have lost over 25 pounds and now surviving on boost high calorie shakes, 4x a day. This is a very difficult disease to diagnose. I have found it very hard to find a surgeon with experience doing the mals procedure, even then they all seem reluctant to do the surgery unless every thing else has been ruled out. Getting an appointment with a GI doc has been the hardest part as they are all booked months in the future. What i thought would take a month or two to resolve is going to take 6 months to a year to figure out and hopefully get back to normal, that is the big change for me is my state of mind about what to expect.     I have SMA sydrome found on ct scan with contrast after losing going from 130 pounds to 109 pounds in a year of constant nausea last week I have gone down from 116 pounds to 109 pounds and constant nausea and agonizing pain in lower abdomen. My condition is so bad I got another diagnosis on top of everything which is chronic fatigue syndrome and it’s not my fault because I have been averaging 2700-3000 calories a day for the last 4 years and haven’t had a problem until a year ago. I have been forced not to eat lunch since nausea peaks at this time so I’m stuck eating a 1,500 calorie breakfast and a 1,500 calorie dinner and nearly dying of dehydration even though I’m consuming 5 liters of liquid a day.

    How do we get doctors to order the ctA with breathing protocol? My daughter had surgery. It’s back. Ultrasound shows stenosis. Every doctor says you don’t need the breathing protocol for a ctscan. It doesn’t make sense to me to do it for an ultrasound and not the other

    I think I might have this. I have had several procedures and hospital visits because of my symptoms since the beginning of the year but still no diagnosis, just guess treatments (some did help to relieve the pain). I finally have an appointment w my primary doctor next week(which I booked months ago because he was booked through out)

      Search for other diagnoses (ie MALS, Gastroparesis, GERD)..

      Have GERD and took Omeprazole for about a year. Stopped and don’t have much issue with it anymore. Negative gastroparesis from emptying tests. Not sure on the MALS but I would imagine it would have shown up on one of my prior tests?

      ⁠sometimes MALS does not show up on any imaging tests. However, if you have a good response to a celiac ganglion nerve block, MALS should still be in the differential diagnosis.

      Ok,thanks;so initially should treat the cause ,especially those younger patients have psychiatric elements like anorexia nervosa,PLUS nutritional support (TPN)with considerable attention to the severity and the duration (surgical intervention should be nearly considered)

    Have had symptoms extending back to childhood of early satiety, bouts of nausea, and upper GI pain/spasms. You mention rapid weight loss but that hasn’t happened with me though a 14 degree angle and 6mm between arteries was recently caught on CT and CTA. Dealing with a bad bout now but I don’t vomit. Also, dealing with chronic malabsorption issues. I am a nurse and have always forced nutrition and treated symptoms but my bouts seem to be worsening. Had to pull out of grad school for a year. Do you see chronic intermittent cases ever?

      I’m researching surgical intervention because, honestly, I’m very aware of my nutritional needs and I’ve always done everything to counter these bouts and not lose too much weight. Wish there was a really good study out there comparing all the surgical interventions because I’m done with the “conventional” method I’ve essentially been doing for years. I’m out East so I’m trying to get to an SMA specialist out this way. But would really like to thoroughly compare pros and cons of various surgeries. Is your double approach procedure essentially more fool proof? Do you do the DDJ in addition to the Strong’s in case it were to try to move back? My greatest fear is adhesions and scar tissue causing other problems. And that was me just commenting through my husband’s account above- same person, lol.

    Noone will do surgery in Australia and horror outcomes are scary. Mine is getting so bd its constant gripping gnarling pressure pain and nerve irriation in right side chest and now as vetting worse is spreading down abdomen and into back. Im so underweight as food is hard and no appetite as in flight or fight all the time. Its causing severe autonomic dysfunction issues and the lonher the nerve irriation is left with worse issues are getting

      Many possibilities, unfortunately. Nausea does not commonly improve as often as pain does with MALS surgery. Examples for reasons for nausea incidence: Gastroparesis, GERD, SMA syndrome, SIBO, etc..

      @@dannyshouhedmd401 Thanky You for answers! Basen on Your experience where would You start looking for the cause? Gastro- and colonoscopy without changes. Before surgery there were no abdominal pains, only big nausea, tachykardia and cold tremors. And only nausea left :(

    No surgeon in Australia will touch this surgery and needs to be done by an expert and open surgery so can deal with the scar tissue and fibrosis to reduce further issues still. However i am finding it impossible to get help and meanwhile now can hardly eat and the nerve bundle irritation means stuck in flight or fight and compressed tightness now is 24/7 from chest to radiate down abdomen and into back. Feels like im stuck internally right aide body and nothing helps ease the pain. Spreading and more deep and visceral issues longer its left. My autonomic dysfunction is worse and blpod pooling in legs wtc etc. Vicious cycle

    I am the rare male that was born with this and required open double bypass emergency surgery. MALS is profoundly painful and finding a surgeon who knows how to do repairs is really difficult considering the rarity of MALS, not to mention it is fatal without the surgery, which still has a 1 in 3 death rate.

      Hi I am so sorry for what you have been through but sounds like you were well taken care of, which I am glad to hear. I do want to clarify though that MALS is not an emergency nor is it fatal without surgery. I don’t want patients to get the wrong idea. There also is not a 1 in 3 death rate, whether you are referring to patients who do or do not undergo surgery. MALS surgery is performed to improve one’s quality of life because of debilitating pain, not because of the risk of death without surgery. I have done over 100 MALS surgeries with no deaths so if you are in the right hands, surgery is safe. Thank you for commenting. 😊

    Dr I am suffering from 1 year I already text u in watsaap I have epigastric pain after eating meal , that long last for 3/4 hours every meals gives me a pain , food regurgation when I take enough meal , difficult to breathe feels tightness in epigastric region my all gi test gastroscopy , barium swallow, gastric emptifying test, ultrasound ,come out normal ph manometery show food regurgation does it might be mals or smas . I take anxiety medication because my gi dr told u don’t have any gi issue and told me anxiety medication . I am taking since 4 months but my symptoms are worst after meal no medicine working on me .

      Hi your symptoms are very suspicious for MALS. I would recommend getting a CT of the abdomen pelvis with IV contrast with breathing protocol as well as a dynamic duplex ultrasound with breathing protocol (inspiration and expiration). Even if those are negative, you may have MALS. A celiac ganglion nerve block is a very important test. I get this for all patients. If you have significant relief from the nerve block for 6 hours or more, there is a good chance you would benefit from MALS surgery, even with negative imaging studies.       Yes it can. A CT scan with IV contrast of the abdomen is a key test. If the angle between the aorta and SMA are less than 18 degrees, that is very suspicious for SMAS. 18-22 degrees is gray zone. Symptoms must correlate, of course.     VtOS aTos nutcracker syndrome and malS sufferer here. Looking at surgery for VTOS but mals surgery has horror stories. I am underweight and struggle to gain and ny vascualr compressions have caused worsening autonomic dysfunction hence wasting away further as cant exercise, gain weight or strengthen body and posture. Would a feeding tube to gain weight help cure MaLS and nCs.

      I’m sorry for what you are going through. Weight gain does not cure MALS..only surgery. If surgery is done by someone with experience, you will be ok.

      ​@@dannyshouhedmd401 their is no help in Australia for surgery and given financial issues travelling internationaly is not an option. Im worried given the constant spiral and stress and wasting away that ill soon die or take own life as cannot live like this anymore. Their has to be some sort of expertise help in Australia

    Hi Dr Shouhed, our daughter was just diagnosed with SMA. She is 11 and has major motility issues and is Autistic and has limited verbal skills. She is always telling us “I hurt”. Is there anything for pain you would recommend she is currently on gabapentin. Would you recommend surgery on a child. We are in Michigan at Helen Devos children’s hospital. Do you know anyone on the East Coast that does surgeries regularly on children. Thank you.

    I had a successful open MALS surgery 3 years ago. Unfortunately it took too long for me to get help. My pancreas is now insufficient, my stomach hurts after eating, it feels like food is at a standstill and noisy in the gut (assuming due to low blood flow). I am waiting for ctscan results. Edit: In addition, my artery is now a zigzag pattern (new normal) and right below this I have an aneurysm.

    Why foes nobody ever mention EDS is. Big cause of this. Rapid weight-loss is not the only reson this happens. Thousands of patients who have been diagnosed usually have EDS or some type of connective tissue disorder. It NEEDS to be mentioned.

      Yes EDS can be a primary etiology as well.

      And building on from that then we get patients with lung pathology too, ie those associated with connective tissue disorders, collagen or genetic, who also have SMA. Imagine that, connective tissue disease, serious lung disease, and then problems with digestive system inter alia SMA. Whoever coined the phrase “multiple pathologies are rare” really doesn’t know what they are talking about. Human body really sucks.

      No it does not. The 3 tests that diagnose MALS are: 1. CT scan with IV contrast, preferable with breathing protocol. 2. Dynamic duplex ultrasound with expiration and inspiration. 3. Celiac ganglion nerve block
