    The final comments by Dr. Alger are critical. Very few urologists are skilled in this procedure and it is VERY user dependent. If you want a HoLEP procedure, you should make certain the urologist has done, at a minimum, several hundred HoLEP procedures. Ideally, the urologist should have performed several THOUSAND HoLEP procedures and he/she should be almost entirely devoted to doing this procedure full time, not simply incorporating it into a routine urological practice in which they do a little bit of everything. In the best of hands, this can be performed as an outpatient procedure and you can walk out of the hospital 3-4 hours later without any difficulty.

      Retired orthopedic physician here. It is professional for Dr. Jordan to not respond to questions as your not his patient. He is doing a service providing information regarding the procedure. I am looking into this procedure myself as it seems the least invasive. Ask your Urologist your questions and he will help decide the most appropriate treatment. It is good you are first doing your own research to make an informed decision. Best of luck.

    I had radiation treatment for prostate cancer. It appears the cancer is gone and my bladder is healed from the radiation treatment. During the healing from 12/15/23 to now (about 5 months) and hospitalized for severe bladder spasms, UTI and bacterial infection. I have had several UTI's, severe bladder spasms. I have an enlarged prostate blocking the urethera. I have had some form of Foley catheter, spanner stent, suprapubic catheter in and out since 12/23. Doctor may I ask a question. Is HOLEP a procedure for me knowing the side effects could be greater due to having the radiation treatments? Thank you if you can answer my question.

    Had this procedure almost a year ago. Bled daily for 3 mos afterwards. Still have occasional bleeding after physical excursion almost 1 year later. Some improvement in frequency/urgency of urination. But, considering the long recovery period not feeling there has been all that much positive gain. There's been some improvement, but not as much as I was told would be the case.

      @baskan3272 Aquablation does not have the long-term side effects of other methods, such as TURP. A month after I wrote that post, things are still slowly improving. A lot regarding recovery time will be determined by how enlarged the prostate is and one's general overall health. But, to say "no" side effects is a bit of an over simplification imho. Best wishes.

    Hello hero! How many catheters do you have to wear during Aqua Baem? I heard that two catheters must be used, one through the urethra and one through the abdomen into the bladder, is that true? If it is possible to use a catheter through the urethra?

    Been on TRT for the past 10 years (age 21 to 31) at anywhere from 400mg (earlier on) to now closer to about 150mg per week. I usually take a break off TRT for a month every few years which includes stopping TRT and taking HCG for 4 weeks. At age 25 I purposely cycled off for 3 months and took HCG while being off so my wife and I could get pregnant with our first kid. 5 years later we decided we wanted another child so I cylced off with the same protocol and it took another 3 months for us to get pregnant. Both kids are healthy and normal. Cycling off now so we can start trying for our third child!

    I had cyberknife in 2020 treating 3 tumors with gleason score of 6. PSA to start was 13.6 2.5 yrs later it was down to 0.8 which was cose to nothing. Side effects was external burn sensitivity of the anus and a bit of fatigue , I was 57 and super active and in good health. One thing that I think helped was having sex each day after the procedure, even though it was very painful similar to been hit in the nuts, I feel it kept the prostate contracting and helped from hardening up as its been cooked by radiation. 2.5 yrs later my PSA is back to 12.4 now, it suddenly started creeping up i. Feb 2023. I just had a PSMA Pet Scan and have a new tumor hence the increase in PSA. I still dont want surgery and looking at potential new radiotherapy program or focal treatment. Any suggestion would help.

      There doesn't appear to be data beyond five years yet, but at the five-year mark Aquablation appears to come out ahead of TURP in terms of ongoing symptom relief. See here: pubmed.ncbi.nlm.nih.gov/35150215/#:~:text=At%205%20years%2C%20IPSS%20scores,the%20TURP%20group%20(p%20%3D%20.

    I'm on coumadin for a mechanical heart valve. My past surgeries allowed me to go off of the coumadin for 3 days. Is it still possible for me to get the aquablation treatment? If not what's the difference between a the aquablation, holep or TURP procedures when it comes to bleeding? From what I have seen, it is common practice to cauterize any bleeders after the main procedure has been completed. Why is the bleeding (off of thinners) issue so critical for aquablation and not TURP and holep? Thanks

    Around 4:00 in the video the point is made that many men are uncomfortable talking about ED. The narrator answers this issue by saying that he and his staff talk about ED all day long and are very comfortable talking about it. Wrong answer. It's the PATIENT who's uncomfortable. Whether or nor the STAFF is comfortable or not is irrelevant.
