An explanation of Kaplan-Meier graphs in cancer research
What happened ma’am?
Hello Doc sorry for the question but I don't fully understand how this works my CA19-9 come back 3U/mL is that normal or should be a concern? Thank you
This video was super helpful. Thank you!!!
Just diagnosed with PC last week for 1st time. They put in a stent and markers looked better. But not CA 19-9. Stunned to see that before the stent/biopsy it was 4200. Then a follow up test a few days later came in at 6200. Other than that they tell me it was caught early with a tumor of 2 cm. Should I be panicking and think that surgery is out of the picture?

My mom has elevated Leukocyte sitting at 22, 3 weeks after starting folfirnox, and her Neutrophils is also elevated at 16. She has no symptoms of infection. Does this mean a poor response to chemo?
Brilliant informatics!
Great stuff, Dave. Thank you for all of your videos!
Thank you - very nice explanation!
It's terminal. No cure. The whipple surgery technique is 85 years old. Never been improved.
2024 and this has been more useful than you think. Thank you for this gift sir. R.i.p.
2024 and this is still useful to us. Thank you.
This has been so helpful
2023 and this was so helpful. Thank you for this.
Dear sir, I found your videos during research and I am engineer (retired) also. I am 57 years oldu and female. During check up (I made almost each year) my ca19-9 value was 277 on 20 september. There was no other anormal values for other blood controls. Then bt for boton, endoscpy and colonoscopy done. There is no tumor. Also enzyms of liver and gall bladder are normal. There was no other symptoms. On 20 october my ca19-9 value is 1150 u/ml. Which doctor would you recommend to go to and what should they investigate. How long does it take for tumors to become visible? Thank you for your help. I am living in Turkey and I am Turkish woman.
Thank you, we just got the results today .. My father has pancreatic cancer stage 3 (we don't know what stage it has become now after 6 sessions). After 6 sessions of chemotherapy , CA 19-9 dropped from > 800 , to 35 (note that it increased to > 1500 somewhere between session 2 - 3 ) ... Tomorrow he'll get a scan imaging.
My mother has been diagnosed with stage 3 pancreatic cancer. Doctors are going with Neo Adjuvant Chemotherapy, they giving her Gemcitabine and Abraxane. Can you please tell what chemo drugs was your father given
@@rishabhbhardwaj3878 I am sorry to hear that, wish her fast recovery, don't lose hope. My father was on Folfirinox regimen Protocol ... that's what works for him. His response to chemotherapy is good so far, and now it's been a month since he started doing radiotherapy in order to try and remove the rest of the tumor (that's the good news, but the bad news is that it also metastasized to the liver, that too is addressed through radiotherapy), but overall he feels so weak after 12 chemo sessions, although his far better than he was a year ago. I feel you brother, we are going through the same ... this year was full of tears, depression, expenses... I feel you ! wish your mother a fast recovery, just be with her❤🙏
Interesting. I copped a horrendous dose of c-diff (bowel infection) with associated mouth ulcerrations after about 7 or 8 days following my 2nd dose of FOLFIRINOX when my neutrophils dipped waaaaay below the neutropenic line, despite having pegfilgrastim after my chemo was disconnected. Then I copped a second bout of c-diff after round 4 of FOLFIRINOX at about the same time (7-8 days). My chemo was cut short because I lost so much weight due to c-diff. My experience matches the time frames you have detailed. I am now awaiting surgery and just trying to put on as much weight as possible for my distal pancreatectomy and splenectomy. I am not looking forward to the adjuvant chemotherapy following that. I have no idea how to avoid getting c-diff again due to neutropenia
Hi sorry to hear that. I have just had the same thing, go well regards Steve from NewZealand

Do pancreatic enzymes rise only if there is a head tumor or can it happen even if it's in the body or tail?
Pancreatic enzymes are made throughout the entire pancreas.
@@pancanology2786 thank you. I read a symptoms list that indicated high enzymes might be a result of cancer in the head, so I was wondering if that was accurate.
@@vika_2323 The larger ducts are in the head of the pancreas so it's probably true that head tumors have more trouble with enzymes. Anyone who puts out absolute statements about cancer (like this very sentence) is over-simplifying. If it was easy, it would have been cured by now.
CA 19-9 over 100,000 No jaundice. Is this a typical level for pancreatic cancer? Or what else could it be? It seems much higher than the usual definition of extremely elevated. Appreciate your informative video. Thank you!
I'm no doctor, but studies seem to show the higher the CA19-9 the more likely it is cancer. In any case, numbers like that should make any doctor sit up and take notice.
What if the the inactive proteolytic enzymes Activate while in the pancreas? How would that happen too?
The enzymes need the right environment to get activated. If they leak from the pancreatic ducts as is common in pancreatitis or pancreatic *ductal* adenocarcinoma, they could become activated. Normally, they are only activated when they enter the small intestine.
Merhaba.. Brca gen testi pozitif olan bir hastanın ca 19-9 değeri 82 çıktıysa bunun anlamı nedir? Taş yada iltihap mıdır yoksa kanser midir?
As far as I know, the BRCA mutation status and ability to make CA19-9 are totally independent.

What if wbc is higher than usual and rbc getting low and there is some sort of back pain while having doses of chemotherapy????
Sir May I ask you; my father has CA 19-9 256. He has drenching night sweats and severe itching skin All over his body. And maybe at certain areas feels really bad itching like lower back area and abdomen both sides. Guess after laboratory tests and mrcp and endoscopy the pancreas is clear the bile duct is fine as well. What could cause the elevated ca19-9 and the symptoms??? Any suggestions!..... (None of steroids or anti allergy topical creams made the itchiness feel better. Useless.)
Will small enough tumors not show on a normal ct scan?
No, they won’t. The CT scan takes thin slices (actually a corkscrew-like spiral) of scans and interpolates the information between these slices. If a tumor is only seen by a single slice or none at all, it can go undetected.
@@ddessert314 Thank you for the response. I'm dealing with nasty issues that all align with the beginnings of pancreatic cancer but no one will listen because nothing can be seen on normal imaging. Frustrating!
I've had Idiopathic Chronic CP for 9years now now. Diabetic Gastroparesis and I've been experiencing a change of symptoms like those of Pancreatic cancer this past month I cant eat, loose pail floating stool grease film, dark urine, pain increased 10 fold ex.I begged my primary for a Ultrasound cause my last GI just quit. They just saw "calcifications" on my pancreas via CT scan while looking for Gallstones. I cant get into a new GI till next year and all I've had previously just wont listen to me. My lipase and other levels stopped elevating during flares 4yrs Into my condition. So when I got to the Emergency department they dont believe me.Food is going right through undigested and I'm wasting. I've asked about creon before, but I've always been denied! Is there anything I can do to get help.Any test to beg for, Or anything else I can try? I have 3 kids and I'm just essentially starving and in severe pain. Ty
Correct, they need to be at least 1cm to be spotted usually and larger ones can be missed by radiologists who are not specialised in tumours/cancer. It's an art not a science.
Thank you from France !
ulcer paitence have hight Ca 19 9 value possible ?
Have not heard of this, but if the ulcer was in the duodenum near the Ampulla of Vater, it might be possible.
@@ddessert314 i have ulcer and focal active colits in terminal illum . i have tested ca 19 9 . i have 49.6 . is it normal ?
@@malayalininja9057 That sounds like a question for your doctor, which I am not.
yes, an ulcer can cause an elevation in CA19-9 for sure.
So glad I found your videos.
Insightful video. 👍
As a medical student I appreciate this
Thank you for this very helpful video! I learned so much.
Nice work! Meta analysis using this technique, a great idea! Could be overly ambitious, though on the other hand, it may not be considering the immense work behind getting trials like run and completed in the first place. Manual aspects of this would need to be optimised, I gather the assembly of the spread was fairly manual, yes? Fine for a few studies, but if 10, 20, 30 studies could be a bottleneck.
You're right, this is a proof of concept, not practical on a large scale. One option is using a PDF parser (such as R's pdftools package) to extract the desired figure and grab the coordinates from the PDF data itself. In older studies, I've seen that the PDF really just contains an image file of the K-M plot. A fallback but less accurate option is to find and measure all the vertical and horizontal line segments from the image file. Not too hard to do, but making sense of them all is probably a big task and not easily reproducible. But probably the first option is to see of the authors are sharing their dataset in the first place.
@@pancanology2786 Oh yes, fully understood, and quite necessary ... I was in no way taking away from your study, which I think is excellent. I just wanted to evaluate which stage it was at. I will continue this post in private message.

My moms CA 19-9 was 70 two years ago. We just got her tested and now the CA 19-9 is normal at 19. I discovered that the Statin drug was destroying her entire system to where she almost died an untimely death. I stopped the statin use 2 years ago and started to revers all her disease with supplementation like a probiotic and a digestive enzyme. The disease that she had was jaundice, IBS, gallstones, Bile Duct inflammation that was thought to be cancer. Doctors put a stent in her bile duct but that was just a temporary fix. Lucky I took action even thought her doctor begged me not to stop the drug. How miss informed she and most doctors are about the use of Statin Drugs.
What medicine was she taking? Glad you took matters in your hand
What are statin medications
Sir my CA19.9 is 930 how i can control it , i had periampullary carcinoma surgery now liver is effected by it, i hav scare in live
CA19-9 usually drops when the tumor shrinks and the damage it causes heals up. The best way to make it go down is with an effective treatment. Genetic/Molecular testing is the best way we have right now to find an effective treatment.

My father had an elevated CA 19.9 level of 273 and he had pancreatic cancer. After surgery and several sets of chemotherapy, it is now 18. Does that mean cancer has gone?
CA19-9 is not a sensitive enough indicator for this. 18 is a good sign that it is not present in any large way and that he is healing well, but small numbers of cancer cells could still remain behind.
How he is now i am asking as my mom also have same issue becouse of psoriasis and her CA 19.9 IS HIGH
Where did he have surgery
@anushkagupta1866 he is no more. But he had 4th stage pancreatic cancer while detecting itself. Hope your mom doesn't have any other problems and she do well.
My condolences to you. Did his CA 19-9 levels rise again before he passed? I'm sorry if this is a very insensitive question.@@dreameronbike
Thank you so much for such a great explanation of Kaplan-Meier curves. Hope you are doing well and would absolutely love to see a video about extracting raw data from KM curves (mentioned @13:27).
With a CA 19.9 at 773 but otherwise my other organ functions result are fine, decided to just live life as normal. Just went to see a specialist who want to probe me further with colonscopy and ct scan. But after further consultation if it is really necessary, decide another blood test would suffice for now.
now all okay ?
2nd result goes up to 1223 but since physically normal, I decided to just live a normal life. Still no symptom of liver or pancreas cancer. Just had gone through Covid-19 with the omnicron variant and recovered. I feel while an elevated CA19.9 trigger further medical investigation, if you knows you are fit and have none of the symptom, just stop worrying and go live a healthy snd happy lifes.

Great Video David!! Thanks for sharing!!
Thank you - clear and concise.,
Thank you Dave. I'm one of your readers at FB. Fortunately, my chemo was for cleanup after the modified Whipple, instead of curative.
Hi Pancanology - Here is my mom’s case, 65yo female. We are confused by her rising Ca19-9 levels. Can you provide any thoughts? November 2019: Ca19-9: 9000. PanCancer diagnosis, 2 cm tumor in pancreas, no Mets March 2020: ca19-9: 500. Post Fulfirinox chemo treatments April 2020: ca19-9: 200. Post distal pancreatectomy surgery Post April 2020: ca19-9 levels have continued to rise back to 8000, checked every month or so. All scans are clear as of December 2020. (CT, laparoscopy, blind biopsy, etc) Any thoughts on what could be causing her ca19-9 levels to rise so high without visible cancer? Have you heard of any other cases like this? Her drs seem stumped as well that they are not finding anything in her scans. Thanks in advance!
With this late reply, you probably already have your answer. A rising CA19-9 is usually not good news, it could, but not always mean cancer. Ulcers or pancreatitis after surgery are fairly common, too.
brother my dad antigen 19-9 report saying,, its 10,000, he get jaundice twice in month,, and he also have galbladder stone,, what's ur opinions on this
Try a digestive enzyme and probiotics. This happened to my mother and it was the Statin drug that was destroying her digestive system and 600 plus more functions. Thanks to her GP.
