I got it today with a dude. He helped me and i got it. All u gotta do is explore the place then find the sanity thing. After getting it shiuld be at 70 or 60 then wait in a box if its kills you just restart and the sanity will be saved. Go in a box and wait. The reading constructions angred me but i eventually got it. Plus theres a skeleton saying "THE END IS COMING" all over ur screen. Then my screen got inverted and got the badge. I rejoined and i got it!

You cant do it in private apparently
I got checker to 0 and my screen is blue and I resetted and i didnt see the toxin-psv2 in my loadout.

yeah, also dont shoot the fake friends or it will insta kill you. wait to you see the skeleton monster and jumpscare. when you get reset your colors will be inverted, and you will get the gun. also idk if this is true, but i think the damage either stacks or multiplies depending on how many you pierce.
