Great case and presentation. At 2:41 is that not a cellular crescent? If not what are the cells at the Bowman capsule? Thanks
This is a good question and distinguishing collapsing glomerulopathy and crescentic glomerulonephritis can be challenging in practice and when looking at a glomerulus in isolation. The cells that you see here within the Bowman's space are visceral epithelial cell that have undergone hyperplasia and hypertrophy. In general, the features that help us favor a collapsing glomerulopathy include reabsorption droplets as seen in the previous image. In a crescentic glomerulonephritis we would expect to see glomerular basement membrane breaks, karyorrhexis, and fibrinoid necrosis, as well as red blood cell casts in the biopsy.

If only a single core of tissue is obtained some pathologist recommend to put it Michel’s medium not formalin. The author here says otherwise. Can anyone comment on this?
I had chronically high calcium (hypercalcemia) and went to three different kidney doctors, a blood doctor, an encrinologist, among several general physicains and not a one of them was able to tell me why I had this condition. PTH was always low, calcium was always high (just outside the reference range but high nonetheless). Began to urinate with blood (red-tinted urine) and soon thereafter passed a calcium stone that thankfully did not become lodged. After going to the Mayo clinic and a barrage of tests, a panel of genetic tests was ordered. Thankfully, my doctors at Mayo Clinic were wise enough to select the correct things to test for with this genetic test. It came in the mail, and was a tube which was supposed to be filled with saliva. After sending it back, about 4 weeks later the results were in. The geneticist told me on our next appointment that she had a diagnosis for me (which after so many tests and doctors all shrugging and basically saying "dunno" I did NOT expect a concrete diagnosis). A mutation on my cyp24a1 which was the cause of hypercalcemia and the systemic effects on my system. IF YOU HAVE HYPERCALCEMIA AND LOW PTH --- TALK TO YOUR DOCTOR ABOUT GETTING THESE GENETIC TESTS FOR CYP24A1. You can actually order these tests on your own via invitae (which is who I used, I believe it cost $250 and insurance does not cover it sadly). This has me hella depressed, life isn't fair :(
