Applicant Information of the NYSED form 2F My problem 1 . is the start of my name which has MARIA in my passort and birth cert. but in my diploma, TOR & PRC ID the MARIA is shortened to MA only. Should I put the full MARIA based on my passport&BC or the shortened MA based on my diploma/TOR/PRC ID? 2. For NYSED form 2F question #8: asking for the date of attendance (from mo. /day/ yr to mo./day/yr ) will the informations be provided in my school? 3. For form 3F for question #4 Print name I will put the full MARIA shown in my passport and birth cert. While for question #8 If you took a licensing examination using a different name. Should i put the shortened Ma because bec it is whats in my prc ID and diploma? 4. Is there a need to make an affidavit of the one and the same person because of my MARIA and MA name difference in my Passport/BC vs Diploma/TOR/PRC ID? Hello Mam on the process po ako ng application ko. Pagdating sa online form 1 idedeclare ko po kasi PHRN license but recent lang po kasi ung date released ang sabi sa system please enter past date.. Ano po kaya pwede gawin mam.. Ongoing nadin po kasi process ng school crendentials sa school at state board verification ko sa prc.. eto nlang po online ang pending ko. Any suggestion po sana what to do.. Thank you

Hello po. Question lang po regarding 2F Form NYSED. Planning po kasi to DIY my NY NCLEX Application. Ask lang po pano if anong ilalagay po sa Dates of Attendance if nagtransfer ng school? First school 2018 then graduated sa different school year 2023. Okay lang po ba na nilagay kong start year ay 2018 (first school) kahit 2020 ako nagtransfer sa school where I graduated? TIA
