Very good and important discussion. I am new to this subject but it seems like the result of Platonic thought is the problem of “the perfect is the enemy of the good”. The same problem I see in christianity…. The person in this video talking about the actual physical thing being in effect -belittled- by the platonic version of the thing, that is a big problem. And actually…I do believe this may be a big part of the problems of various neurosis & other sicknesses in our western society.

Agreed. In retrospect, I overstepped my reporting of the study and I've tried to be more careful since then. The real result here is that there's no difference between groups. No benefit, but no harm either.

This guy is a fucking dork. His "analysis" is testosterone benefits are mostly placebo but people don't like to admit that to themselves, yet the downsides are all very serious and you'll die early from some kind of heart failure. I've been around guys like this one. They worship the idea of the beta nerd male. They hate jocks and anything that promotes masculinity and hide that blinding hate behind Science(tm). This guy was likely lightly bullied (aka a couple fun jock guys in high school made jokes to him and he couldn't laugh at himself because of crippling jealousy and low self esteem) in high school by some football players or wrestlers who had hot cheerleader girlfriends and he can't STAND IT!!! Go shove it "doc".

Ther's abuse of steroids and abuse of TRT then ther's ah healthy use of modern resources to live the rest of our lifes in a better condition till the last day shall come .TRT is not the Sacred Graal..it doesn't walk you to the gym and doesn't find You a loving wife...but sometimes can give you the energy necessary to do it...who never experienced the plethora of symptoms of low T or Hypothyroidism just cannot understand... I have more than 10 years struggle and many times I've been succesfull avoiding any TRT but is exausting many times and I'm a kind of Buldozer mentally.. recently in my 57/58 YO I'm thinking about try TRT cause the life is very hard those days and I cannot count on extreme sacrifices when may be a less stressfull way to try.. I take my responsability for myself like I did not walk 60 miles to go to work in 2024 having a car . my 50 cts

I appreciate your thoughtful take on TRT. That's very interesting about the A/B group study. I started TRT one week ago and the only thing I've noticed so far is that I cry easier. I'm told that if I don't notice drastic improvements within 6 weeks, then TRT isn't for me. I don't want to depend on TRT for the rest of my life, but if it can help me want to stay alive, continue providing for my family, and increase my quality of life, then I'd count that as a win, even if it was to shave off a few years on the tail end.
Really informative and logical. People should feel very insecure when their square thinking gets shattered with alternative logic.

Hey! Im happy i stumbled on your video cause i was just going to buy some B12. Im 23 y.o. male and i got 31 mcmol/L of homocysteine (must be genetic cause my grandpa and mom have arterial diseases). I found out during testing to find a cause of my intermittent pulsatile tinnitus. Anyways my question is: Would B12 + folate acid + VO2 training give me the best results? I really dont wanna die early hahah. Is it prossible that i have atheroscleroses with that amount of Homocysteine? Im pretty active Thanks in advance.
No need for b12 or folate. Vo2, strength training, DEFINITELY avoid smoking (more than most), keep up your social connections and friendships. And then... Remember we're all gonna die. Figure out how to make the most of the time you have. Find something meaningful to do with your life and commit to building something you value, in a way that's truthful and ethical.

This is very interesting to me... A good conversation. How ever it appear to me that the classification of PLOTINUS under Gnosticism is problematic. Your knowledge base and use of it always astounds me! I always come away with new insights and information that is not only useful, but changes the way I think or approach health and wellness!
This guy has no idea what he's talking about, giving false info off one supposed study about men having their balls cut. A man's increased muscle mass, builds a stronger heart, better sleep and so on will all cause you to live longer. Steroids are bad, but normal test levels are absolutely essential to overall health. This guy in this video is talking nonsense. Low T will cause fat gain, diabetes, bad sleep, stress and so on which will all effect longevity. These type of "experts" that read some "studies" and run with it as undeniable fact have no idea what they are talking about. People with low test, who are stressed, depressed, fat and so on die everyday from multiple different causes and different diseases, but those ailments won't be linked to a man having low testosterone so this guy won't see it in a "study". When he said cut off the balls of a child before puberty makes them live longer you should know not to listen to this guy. A boywho will never develop strong muscles, a strong heart, strong bones and immunity will not live longer, its pure bs. High test will increase muscle mass, bone density, thus increaes heart strength. He compares normal test levels to body builders who take dangerous steroids, of course too much of anything is bad and steroids are bad. Too much misinformation in this video. He shows a study of taking test from 300 to 500 which will give no discernable difference and putting people on an off it at that. Going from 300 to 900 will cause a big difference, and it's not placebo. This guy simply speaks from lack of experience and knowledge about this topic, just regurgitating bs studies.

I would contend that your assumption that it's not placebo is an underestimate of the power of placebo, which is why placebo-controlled studies are the only way of determining the truth. If you have access to placebo-controlled studies showing the opposite of what I've claimed, I'd love to see them.

Great video, but one nitpick: Huberman and Sinclair have PhDs, but Peter Attia is an MD. I could be wrong, but I don't think Attia is particularly a pusher of full body MRI scans. He didn't mention them at all in his book, wherein he recommended a CAC scan for fairly young people and a yearly DEXA scan to evaluate visceral fat (among other things not based on guidelines).
Hey, thx for the kind words. This is worth some nuanced clarification: It's true that Attia has his MD, but medical school only gives you the language to become a doctor. Attia dropped out of his surgical residency, without becoming board certified. Now, board certification is nonsense, so I'm not too worried about that, but the issue is that his entire training was in surgery, which did not give him the skills to correctly understand how to evaluate the quality and importance of diagnostic testing and screening tests. CAC is a good example - the ONLY benefit that's ever been shown is that it can scare people into adhering to their medications more (which does have a benefit). Preventative stenting of visualized heart disease doesn't work to reduce mortality or future heart attacks (ISCHEMIA trial), nor does it work to reduce symptoms (ORBITA trial). So... What exactly are you going to do with the CAC information? You're going to recommend the same things you would have recommended without it (he is right about vo2 max though - that's where most of the effort should go). He doesn't have the training to understand these things. (DEXA is a much more complex discussion). More directly to your point, he has recommended the full body MRI in many other places than his book. He had the founder of prenuvo on his podcast and had a very positive attitude there, and he's mentioned prenuvo multiple times on Twitter. You can also see him discussing full body MRI very positively on the medcram lecture (around 1hr 4min), ruclips.net/video/v8VFTQ74bqo/видео.htmlsi=ls7-c8mHaWvHEcZG He discusses sensitivity and specificity, but completely misses the real issue, which is pretest probability. He does not understand how to practically apply Bayes Theorem to diagnostic testing because that's not his training background. That's why I lump Attia in with Huberman - he lacks the humility to go learn a different field (the science of screening) thoroughly before trying to improve it. Thus he continues to make errors. Vinay Prasad has a much better understanding of screening than either Attia or myself.
@@riverrockmedical OK, I was wrong; he clearly is recommending full body MRIs for his patients. It still seems to me like he's a much lower-tier quack than Sinclair and Huberman, but lumping them together in this video makes sense. Interestingly, in the video you link to (around 1:07:13), he mentions that 20% of his patients get a positive result and that almost all of them end up being nothing. But he frames it as an issue of emotional fortitude rather than clinical utility, which seems like a bad framing.
Oh god, so basically there is nothing you can do if you have high homocysteine, I’ve been having weird symptoms/anxiety and can’t find a cause, did a vitamin test and showed I was low on folic so I started supplementing b12 methyl and folate then the doctor retested me and my levels were high. It I still have symptoms. I’m waiting on a mthfr gene test and was going to book a homocysteine test after that to see what I should be taking. It all causes anxiety to worsen of course. I don’t even want to know my homocysteine levels anymore in case it’s high so Il just panic the rest of my life.
TMG worked for me. B vits did nothing
This is exactly why I recommend against checking things like this. It causes anxiety without any effective treatment options. However, you can still do the things that would be effective anyway - increase your vo2 max, strength, and social connectedness. No need for nonsense bloodwork that won't affect anything.
@@riverrockmedical yeah , I’m very fit and healthy otherwise, eat well excercise a lot. My blood pressure has always been on the high normal side. This year been especially hard with wierd symptoms , pins and needles, anxiety and burning mouth syndrome which is annoying. I’m on SSRI now for it all.
@@Ryan____Z Homozygous 1298C and heterzygous C667T. I have major difficulty with methylation and high homocysteine levels. I take methylated multivitamins, TMG and and glutathione. If I missed 2 days of glutathione and TMG, I have brain fog and memory issues. I cannot explain how much better I feel and think with increased energy after taking the above. I do not take other medications. I am at risk for clotting issues, and in fact, I had 2 pulmonary emboli this past summer.

This didn't actually disprove his point... if anything, it proved it. Looking at your hemoglobin in conjunction with discussing activity (eg, athlete training in high altitude) can give you a lot of information. He also never claimed to just look at hemoglobin, nor did he say bloodwork is the only factor to be examined, just like you said it's a tool
As I mentioned, I'm just using hemoglobin as an example, the same logic applies to most bloodwork. I'm not sure why you think looking at hemoglobin would give you "a lot" of information, since it would be uninterpretable. it's "low" or "high", you'd have no actionable insight from it (for longevity purposes). The only thing that makes it interpretable for an athlete is the ability to run self-comparative studies on performance (N-of-1 style), not comparisons to general population. But since you can't do that for longevity, the information becomes uninterpretable and thus useless. And I also do disagree, he *does* imply that bloodwork alone, without context, is a major factor to examine for optimization, and promotes using bloodwork as the metric to determine the efficacy of an intervention (supplement or behavior).
