I am curious about option D, particularly the "Utilize circular questioning to identify dysfunctional communication patters." Is that using systems theory terms incorrectly? I don't know much about circular questioning, but I do realize I use it often. I'm curious about the misuse of the terms for option D (which one and how so?)

Let's clarify further. When a client shows signs of bipolar disorder, the first priority is to ensure they receive the right medical treatment to manage their symptoms effectively. Correct Answer: B. Refer to a psychiatrist for medication management. Why? Bipolar disorder usually requires medication to control mood swings. Psychiatrists are trained to diagnose and prescribe these medications. This makes it the critical first step in treatment. Why the other options are not first steps: A. Refer to an MD for a physical evaluation. A physical evaluation can rule out other conditions but won't address the immediate need to stabilize mood swings, which is essential for managing bipolar disorder. C. Refer to a support group for young adults with bipolar disorder. Support groups provide emotional support and coping strategies but don't replace the need for medical treatment, which should come first. D. Begin psychotherapy. Psychotherapy is important for long-term management and coping strategies but isn't sufficient alone to stabilize severe mood swings typical in bipolar disorder. Medication often needs to be in place first. In summary, the most urgent need is to manage the client's mood swings effectively through medication, making referral to a psychiatrist the necessary first step.

This is a great example of where you need to learn how the test thinks and how that may differ from real life clinicians. The main refrain in my Master's was "you always have bias, it's your job to be aware of it and not allow bias to negatively affect your social work". The idea of treating "without bias" is nonsensical based on my training, but that's why it's so good I have access to these videos!

This is the problem that i have with TDC. She has a tentative plan! And she mentioned losing a job (loss of social support) and not feeling connected to friends! Sure i would ask about family support but B kind of negates the fact that the stem says she is having trouble connecting socially. The answer basically invalidates what the client said!!

As a licensed therapist for 20 years, I can say I don’t remember the Ericksonian stages at all. Theoretically interesting, versus scientifically valid? I tended to move towards more of an evidence-based approach. It’s important to have a theory. The very common “imposter syndrome,” explains this better than some thing that “in theory” might have happened in his very early childhood and is virtually impossible to deal with at this stage of the persons life. Imposter syndrome is common, and often leads people to feel as though they are not as confident as they might actually be. However, when you have a masters degree in something, you probably know more than 95% of the people in society about that particular narrow topic. You’re only an imposter if you pretend to know more than that, and some of us have difficulty understanding that we actually do know what we know and are as knowledgeable as we actually are. Insight about theoretical stuff that might have - or did - happen forty years ago? Good luck. Write a Treatment plan for that. Another evidence-based, scientifically researched phenomena that accounts for this has to do with how people who are actually good at something measure them selves as opposed to people who believe they are good at something but might not actually be competent. It might be from Daniel Goleman‘s research in his book “thinking fast and thinking slow,” where it was mentioned that a study of people who are judged by others as being very competent, versus a group of people who are judged by others as being virtually incompetent. Often times an incompetent person judges their relative proficiency on what they believe, and denigrate the knowledge of others, and asserts themselves to be more confident than they actually are. However, people who are actually highly competent are often worse judges of themselves and measure themselves by the perfection that they seek to attain, and compare them self not to others, but to their own potential. Truly competent people are seeking to get better. They judge themselves against the gap between perfection and their current level. Lazy people think they’ve arrived. Thus, there’s limitless ways to theoretically characterize this scenario. I am not convinced that in practice these stages are as helpful as more behavior based change. Those are just a couple of examples of scientific research that explains much better than these theories that seek to root all these things in our childhood stages, which are real, but we don’t understand them. We don’t understand their interactions. Individual psychology is very unique. If you were putting together a treatment plan for this person, and you wanted to line your pockets with a year of therapy, how about it. Some people just want to run their mouth and you can talk about their childhood until they die and it’s not gonna change very much of anything for most people. In a case like this you’d have to say what are the deficits that you’re seeking to remediate? What is your goal for being in therapy? What would it look like to you if the problems that you’ve described magically disappeared overnight and you had to give me proof that you no longer suffered from these problems? A solution focused approach the counseling doesn’t often spend a lot of time in non useful hypothesizing

I suffer from BDSM or being tied up. As kids, my sister from Macedonia and some friends I no longer have contact with played and tied with adhesive tape. In the beginning, as a child, it was strange and greasy, and later in my puberty, it appeared as sexuality. To be honest, I only get an erection when a woman does it to a person. I'm not gay, I love women, but I think I went too far with my fantasies that I can't get a normal erection when I sleep with girls, that's why I'm still innocent to this day. I'm 31 years old and I'm starting to suffer from it, it's bothering me terribly, and I'm ashamed to tell my family or girlfriends what the problem is, I've been feeling desperate for a long time. The traumas that I experienced as a child through those bonding games turned into my sexuality, and when I have to have real sex with women, I don't experience a full erection. I think I'm hypersexual... Is there a cure for it and how can I treat it because it's getting serious, I'm still a virgin and it's hindering my social skills with people and girls the most. The last relationship I had lasted a month and she left me because of my fear of admitting to her what it was about because she thinks she is not attractive, and I really loved her, but then I lost her and she found someone else. I want to be normal, what should I do? :( I became desperate and without a purpose in life because if this continues I will not have a girlfriend and a family... Thank you.

Searle discussed countertransference as an analytic datum to be used by the therapist in the therapy. The analyst must be aware without judgement of these feelings, thus allowing them not to interfere, but rather to aid in the process. Transference and Countertransference are not necessarily separate. Thus, I disagree that countertransference is inherently damaging.

A pedophile can feel comfortable in their lifestyle choice, is that really the criteria we want to use to determine if that desire is pathological or not? If it were your children he was attracted to and flirting with would that still be an "acceptable lifestyle choice?" Tricky ethics on this subject.
