I believe this is a fascinating example of the trigemino-cervical complex in action. I've encountered patients whose neck movements triggered vestibular migraine attacks. Could the trigeminocervical complex be the key? It seems plausible
Dizziness is so complex - great job teasing all that out.

What state are you out of? I would love if I can be treated or looked at as another case study for you. Undiagnosed CD, no history of VM and ENT cardio and Neuro cleared me with imaging and the works. I’ve been dizzy for 4 months now. On and off but every day. Some days few minutes the whole day some days are the whole day. Started when I was at work, sitting looking up at a colleague and head turned 45 degrees, then I will look back down at my paper and back up, did this a few times, last time I did everything went black, never fainted but extremely dizzy, the rocking off feeling and just never went away. Never fainted. Occasional numbness in left arm. Sometimes I’m better when I walk, active, driving but sitting I start to feel off and rocking. Couldn’t work, drive for a month. Better now but all symptoms remain. 33M, no history of migraines. +C5/6 radiculopathy. Active, but do a lot of desk work. I’m desperate to feel normal again to play with my 3 girls and not feel like I’m off unmotivating me to just sit at home.

Hi there! Oh my that is quite the combination. Definitely common to have multi factorial caused of dizziness, but I can see why that’s difficult because you have to do a lot of hand motions for the neuritis, but it will definitely bother your neck so you have to proceed with caution. Just try what you can and set functional goals for yourself each week.
Gonna try it! Thanks for the content, it really helps.

Yes

When it comes to really advanced, you would benefit from a customized program based on what your stuff still having symptoms with. You can take all these exercises. Thank you. Is there a way to decide via exercises at home which ear is affected? And if unsure which ear, is it ok to do the exercises for both sides?
@@Clearlight201 it’s always best to get an in person diagnosis if you can. You should not do both ears. Especially not in the same session or day or a few days for that matter. Hello darling. Today I was diagnosed with left vestibular dysfunction. I went through all the tests and that was the impression the doctor got. What about migraine from driving?
How long can you drive for before you get a migraine? This would probably make a good video response to this question. In the meantime , do you try to limit the noise while driving, use sunglasses (sometimes rose colored or amber work better) to reduce glare? Optokinetic training to get use to that driving stimuli over time ?
@@treatdizzinessathome2023 I’ve been going in the evening right before sunset. I can drive 1 mile before my head starts hurting. However lately as I try to progress in physical therapy my head is already hurting before I drive. For that reason I’ve taken a break from driving this past week since my head ended up hurting a lot worse from driving in addition to everything else.
In 2005, there was a very strong earthquake in Pakistan. After that I was very much myself. Neither I fell nor hit the wall. But since 2005, I feel like everything is shaking. Near objects are not felt but even a little far away are felt and my MRI and CT scan is clear
That is very interesting! Have you had a VNG or VEMP test? Any hearing changes? Checked for convergency insufficiency from eye doctor?

Great video. The only problem I have is I have tried various methods to determine which ear is the problem and I can't figure it out. They say whichever side makes you the most dizzy but the dizziness is the same no matter which way i turn my head, or the clock/counterclock method I can't tell for sure either. I have a question, what about when you’re at those same angles but you look down? That’s when I feel it.
Have you been assessed for cervicogenic dizziness? That would be one reason. Or if you’ve seen Therapist in person, they could determine if you have a crystal in the short arm of a canal which sometimes can give you symptoms when you look down.
Thank you! This tip is very helpful.

What about when we sleep?
I do have a few patients that develop the bPPV from sleeping positions and they do sleep a little elevated. The research doesn’t suggest that it’s needed because typically people aren’t below horizontal when they sleep like your head is craned back like in the dentist or salon. But if you do find that keep developing, and you wake up in the morning with it, I do have a couple patients that sleep on a wedge pillow, because they feel better that way😊

Awesome! For a post stroke in the cerebellum due to a vertebral artery dissection would the head movements be well to help with the dizziness and vertigo? I’m fine sitting down and doing them but once standing it gets tricky lol
Well.. cerebellar stroke requires some additional treatment, but head movements can be beneficial because the vestibular nerve goes to the cerebellum and then the cerebellum gives us our eye output and balance output. So it is an effective treatment strategy, but more is needed for a full program. Are you in vestibular therapy?
@@treatdizzinessathome2023 No mam, I’m going on Oct7 for follow up with neurologist to see how the artery is doing but not therapy for VRT. I have emotional therapy now and just did an evaluation with OCVT in Austin to get and evaluation and actually I’m getting my program follow up 2mrow to see what I need to work on. I has a little bit of depth perception issues but major culprit is eye convergence:/ but I had vertigo and disequilibrium :( it’s gotten better 3 months out but when I sit down on a hard llchair or stand up i have damn wobble sensation and especially turning left or right. Sorry for novel
I realize I've limited my exposure so much, now I'm miserable.
I completely understand! It’s a condition that causes you to be very isolated and withdraw, the good news is with very slow and concerted effort, you can increase your function overtime. I have quite a few resources on my PPP playlist one on habituation, please let me know if I can help in anyway.
It depends on the diagnose!!!
👍
Wow, this is an incredible testimony! I'm so encouraged by Mark's story with chronic dizziness and how he was able to get back to living and pursing his career. How did the ear specialist miss the nystagmus that Amy was able to detect?
I was using infrared goggles, and the other therapist was just using his naked eye. a good vestibular therapist should always at least do the initial exam with the infrared goggles to make sure you have a clear picture of what you’re dealing with.
Thank you Amy! I have heard horror stories about the Caloric Test. Where people cannot finish it because the dizziness is so bad, or the dizziness it creates stays with them for days/weeks afterwards. How common is this? I am finally starting to feel better after months of dizziness after a BPPV episode, and I really dread having this test and feeling like that again. It causes me a lot of anxiety.
It certainly isn’t interesting phenomenon. Essentially, if you have normal functioning vestibular system you would have symptoms, but if you then have a normal vestibular system, your symptoms should go away. So what would cause such a response? Well the cellular activation can be to causing migraines. So, the assumption is is that the brain is hypersensitive and that is causing a reaction that should not occur. I would say in my experience a majority of patience do fine with the testing with temporary dizziness, then goes away within an hour or so. However, if the test does trigger a migraine or some anxiety response, you have lingering symptoms. So know that ahead of time and maybe having migraine medicine available or talking with a behavioral therapist prior to the test can give you strategies and how to deal with experiencing the dizziness but knowing that overall the test is safe and everything should go back to normal Because it doesn’t cause any long-term damage to the brain or the ear, etc. I think they half the battle is just understanding what the test entails and how the body will respond as half the battle. Now, if you have no dizziness during the test that actually can be more problematic, because it means that you need vestibular therapy and your vestibular system is not working at all! Hopefully that helps keep us posted.
