I believe this is a fascinating example of the trigemino-cervical complex in action. I've encountered patients whose neck movements triggered vestibular migraine attacks. Could the trigeminocervical complex be the key? It seems plausible

What state are you out of? I would love if I can be treated or looked at as another case study for you. Undiagnosed CD, no history of VM and ENT cardio and Neuro cleared me with imaging and the works. I’ve been dizzy for 4 months now. On and off but every day. Some days few minutes the whole day some days are the whole day. Started when I was at work, sitting looking up at a colleague and head turned 45 degrees, then I will look back down at my paper and back up, did this a few times, last time I did everything went black, never fainted but extremely dizzy, the rocking off feeling and just never went away. Never fainted. Occasional numbness in left arm. Sometimes I’m better when I walk, active, driving but sitting I start to feel off and rocking. Couldn’t work, drive for a month. Better now but all symptoms remain. 33M, no history of migraines. +C5/6 radiculopathy. Active, but do a lot of desk work. I’m desperate to feel normal again to play with my 3 girls and not feel like I’m off unmotivating me to just sit at home.

Hi there! Oh my that is quite the combination. Definitely common to have multi factorial caused of dizziness, but I can see why that’s difficult because you have to do a lot of hand motions for the neuritis, but it will definitely bother your neck so you have to proceed with caution. Just try what you can and set functional goals for yourself each week.

It certainly isn’t interesting phenomenon. Essentially, if you have normal functioning vestibular system you would have symptoms, but if you then have a normal vestibular system, your symptoms should go away. So what would cause such a response? Well the cellular activation can be to causing migraines. So, the assumption is is that the brain is hypersensitive and that is causing a reaction that should not occur. I would say in my experience a majority of patience do fine with the testing with temporary dizziness, then goes away within an hour or so. However, if the test does trigger a migraine or some anxiety response, you have lingering symptoms. So know that ahead of time and maybe having migraine medicine available or talking with a behavioral therapist prior to the test can give you strategies and how to deal with experiencing the dizziness but knowing that overall the test is safe and everything should go back to normal Because it doesn’t cause any long-term damage to the brain or the ear, etc. I think they half the battle is just understanding what the test entails and how the body will respond as half the battle. Now, if you have no dizziness during the test that actually can be more problematic, because it means that you need vestibular therapy and your vestibular system is not working at all! Hopefully that helps keep us posted.

Yes the maneuvers are only as good as the identification of which canal and ear. And to ensure that you have the right diagnosis. Thus resource might be helpful to you stan.store/TreatDizzinessathome/p/get-my-bppv-self-test-and-treatment-guide-

Hopefully, you only have to do this maneuver one to two times and then you should be done. I always wait 15 minutes in between maneuvers. And if it doesn’t work for you in one to two days, try a different maneuver. I have a full playlist on my homepage of different BPPV maneuvers and information, hopefully that helps!
