      dB IL can't be converted to dB SPL as they both are different entities. Intensity is the energy required to produce the sound or per unit area carried by the wave. dB IL is the intensity of the sound produced with respect to a reference (10^-12 W/m^2). However, SPL is the amount of air pressure changes that takes place while producing sound. Once sound is generated by a specific and transmitted in air (compression and rarefaction) as air pressure variations or the pressure amplitude, it is measured as sound pressure level. dB SPL is the measured pressure variation of a sound with respect to a reference sound (2*10^-5 Pa or 2 microPascal) Power is intensity; pressure change is SPL. The relation between Pressure and Intensity: Intensity is proportional to square of the pressure; eg if pressure is increased 10 times, then the intensity will be increased 100 times (10^2=100). In dB doubling of pressure will lead to 6 dB increase and doubling of power leads to 3 dB increase.

    Does this means (For the Acoustic Masking procedure) that we will Mask with the Mmid forumula instead of the traditional approach of using Mmin? In the books Ive read so far they recomend Mmin = PL - IA + Largest ABGnte as the formula to find the masking

      Yes we do have Mmin and Mmax at this point of time. We also know that effective masking is some where between min and max masking. Based on this we are pretty much sure that certainly we are providing adequate amount of noise for masking. When we present at Mmin there are chances that it may be under masking and the same holds good for Mmax where it could be over masking. However between these two values it is proper masking.

      Effective masking (EM) level in simpler terms: Assume the hearing threshold is 15 dBHL for a pure tone, now the minimum amount of noise required to just mask this puretone will be 0dBEM; if the amount of noise shifts this threshold to 30 dBHL then that noise level will be 15 dBEM; if threshold is shifted to 20dBHL due to noise presentation, then that noise is 5 dBEM. Hope this helps

    Are any of these implants other than baha and cochlear implant useful when the stapes is fixed as in otosclerosis?

      Surgical corrections to improve the mobility of stapes (Stapedectomy/stapedotomy) can be performed, if this doesn't improve the hearing part, vibrant sound bridge may be an option. Still people do get benefit from conventional hearing aids. The management options need to be evaluated based on existing conditions, the client's preference and expected outcomes.

      The outcomes of middle ear implants are highly variable depending on the physiological alterations after implantation and due to the pathology (cause) by itself. I am aware of a client who got a middle ear implant (not Carina) and had no improvement at all.

      If dendrites are there, very good. Even if there is loss of dendrites, but spared spiral ganglion cells and intact neural pathway till cortex, yes cochlear implants will be helpful.

    Sir i have one doubt, Why air is needed for vibration of ossicle??? Suppose if there is no air in middle ear , will the ossicle vibrate normally???

      The air pressure in middle ear and ear canal has to be equal for optimal vibration of tympanic membrane. If there is negative pressure in middle ear (Compared to atmospheric pressure/pressure in ear canal) TM will be pulled towards ME restricting its movement, if there is positive pressure in middle ear, TM will be bulged towards (mostly fluid) ear canal - again altering the optimal position of TM thereby reducing the vibration of TM. When TM vibration is restricted, sound conduction is reduced. Ossicles vibrate only to the extent the TM vibrates. Hence air (pressure) in middle ear is important for optimal mechanical vibrations.

      The conductive loss is simulated by placing a piece of cotton rolled into the ear canal. It can be removed after performing the procedure. Caution to be taken while doing so.
