      If they are di-di the risks are pretty similar if fraternal or identical! Congratulations! Try to enjoy your pregnancy!!!

    Dr. Tala - another awesome video and great bottom line explanation. Quick 2 questions that come to mind - i have always had trouble distinguishing btwn why LMP is used as a way for us to estimate gestational age. Can you provide an explanation that is simple about why LMP is a good surrogate for eGA? In addition, for TTTS (Twin-twin transfusion), is that really only an issue if pts are mono-mono? What other potential complications aside from TTTS can arise for pts mono-mono?

      Hello JH- thanks for your lovely comment :) your questions1) : in a cycle a woman ovulates around 2 weeks after her period- so when a woman misses her first period she is usually ~ 4 weeks pregnant. This is pretty accurate as compared to later ultrasounds. 2) With mono mono patients were most concerned about them getting their cords all tangled up and affecting blood flow to the babies. Does this all make sense?

    Thank you fo this video. I had bloodwork done and was told my twin boys are monozygotic. But I was told due to them having there own placenta and sacks they are not identical. Would you say identical if thats what the nipt test showed? Thank you.

      Hello! Congratulations on your twins! How exciting! If they are monozygotic according to the NIPT then they are identical (removing the tiny chance that the NIPT is wrong). Many identical twins have separate sacs and placentas (it just means the fertilized egg separated sooner). Hope this helps!

    I have mono mono twins. All boys. They differ in their sizes. And not so identical. Is it possible?

      Genetically mono-mono twins are identical. But there could be external forces in utero (e.g. less placental blood flow) or after they're born (e.g. a different haircut!) which could make them look different. Congratulations!

      I think that so often with identical twins- esp if they’re different sizes when they’re young. Funny how they grow similar though!!

      Congratulations on your pregnancy! Your OB will be following you closely- and many mono-mono pregnancies have smooth pregnancies, even if they end a little early. Good luck!

    i actually had a rare awesome experience to have taken care of twins, one white (Caucasian) and the other african american.
    is that a case of 2 eggs fertilized by 2 sperm from 2 different dads (a white one and a black one)?

      Hello Rukir! It's funny, we feel like we're in these situations quite often- where one twin looks so vastly different from the other. Did you have genetic testing on the twins proving different fathers? The case you described is possible, but also think about how different siblings can appear. This can be even more pronounced if one twin is a vastly different size, or has some other issue (e.g. albinism or Prader-Willi - where the skin is much lighter). For the purposes of the NICU or newborn stay, it doesn't really matter who the biological father is, unless the mother brings it up. Our job is to make them as healthy as possible!
      A colleague of mine once described twins he took care of, where one ballarded 4 weeks younger than the other- and so he assumed the mother must have still ovulated 4 weeks later and that egg was fertilized too.
      Lots of amazing stories with twins!!!

      Yes! least risk of twins. They have their own placentas and their own amniotic sacs. Congratulations!

      No- if they share a placenta- they MUST be identical.

      No! They share same genetic information- so unless something very very very unusual happened - they are either not mono- di or they are same sex
