No dude, he got relieved because they were doing an UNREP (Underway Replenishment) and they lost steering control in the middle of it for 10 minutes. The captain attempted to complete the UNREP despite not having full control from the pilothouse. The ships eventually got so close that they had to do an emergency breakaway which broke off the arresting gear and the nozzle heads from the replenishment ship. The after action investigation found most personnel weren't properly trained and "let's see what this does" was the basis for any knowledge. Multiple senior enlisted (the Chiefs) had little to no experience with gear they were in charge of overseeing and CASREP (Casualty Report) feedbacks were not followed and instead a new CASREP would be generated. It was systematic failure of leadership in the command, so he had to go. The McCain is better off with him gone.

I have repaired diesel electric locomotives. Those things are beasts! Extremely heavy, and once they get moving they do not stop in a hurry. And that's just the locomotive. Put a bunch of railcars behind it, and you aren't stopping that thing very quickly at all. Physics!
Diesel electric locomotives mass between 100 and 200 tons. Your 40-60 ton armored vehicle doesn't stand a chance, if that loco is moving at speed. This is why you don't cross in front of a moving train. It doesn't care that you are there, cannot stop, and will absolutely destroy whatever you put in front of it.

As for terrorists, you're right, combatant protections only extend to lawful combatants. Many countries (including the US) have their own rules for conduct which apply to their servicemembers no matter the target.
