It is due improper arrest of inclusions like sand and slag in gatting system. Use of filter , Proper overlapping or providing chock can help you in arresting such inclusion.

What should be maximum nodule count
There is no such specification but one can aim upto 400-500 max. Above that acheiving nodule count is very tough job
