In many cases planet rules two houses except for sun and moon. So in case of parivartana yoga which houses should we consider. Houses where planets are? Or moolatrikona....?

Will Mars in Scorpio and Ketu in Aries count as a parivartana yoga?
No! As Ketu does not have true lordship of Scorpio. Its a co-ruler. Besides its a shadow planet hence Yogas are not formed by shadow planets - only true planets.

I've noticed great effects of it but I've also noticed almost fatal effects of Saturn too (Scorpio asc). It's getting better with age though.

What about if 2nd Lord is placed in Kendra from & in Trikona from its house, e.g. for Virgo lagna Venus is the lord of 2nd house & is placed in 10th house and from Tula i.e. in Trikona and in 2nd house from Vrish sign but combust.. so how would you see it ?
