Doxycycline for Pneumonia w/ Dr. Brad Spellberg (E13)

  • Doxycycline is an old tetracycline-like antibiotic with a comparatively good side-effect profile, but it doesn’t get much love in the guidelines for community-acquired pneumonia as the evidence level was deemed to be low. A recent issue of Clinical Infectious Diseases contained a systematic review with a meta-analysis.
    Dr. Brad Spellberg is the chief medical office at the Los Angeles County-University of Southern California Medical Center. Dr. Spellberg is also a professor of clinical medicine and the associate dean for clinical affairs at the USC’s Keck School of Medicine.
    I talk with him about why doxy might be a good CAP drug (and when it is not a good choice), pneumonia guidelines and the process of creating guidelines in general, and resistance patterns, minimum inhibitory concentration, and clinical significance.

    Just Diagnosed for the first time in my life with Pneumonia. Was prescribed doxy 100mg/2x a day for 14 days. I don't think I will be able to do 14 days as my vision is blurry and head pressure is intense after only day 2. My wife gave this to me, she was prescribed methyiprednisone 4mg(z-pak) and also azithromycin 250mg. She literally just finished her dose yesterday, her symptoms aren't getting any better. I told the ER doc, and showed what she was prescribed. after an x-ray of my chest, her meds, that I informed aren't helping her,was prescribed this, after day two, it's clearing my chest. I hope 7 days will do the trick.

    As someone with lung cancer, I get mild pneumonia all the time. Doxy is great for clearing it out when needed.

      glad this is heps you. I currently have pneumonia. I noticed blurring n head pressure while taking this 100mg 2x a day. on day 2

    My dad has pneumonia currently. He didn’t require oxygen and doesn’t have a cough or congestion but had mental confusion and difficulty completing sentences. He was diagnosed by X-ray and elevated blood cell counts. He’s been given 5 days of amoxicillin and azithromycin. After 6 days he still gets occasional fever of 100.6. Is that normal? Can we expect it to get better over the next 3-4 days now that his pills have ended. Does he need another round of pills?

      Unfortunately, we cannot give advise on individual cases. If your dad does not improve, he or you should get in touch with his treating physician or provider. If he has a medical emergency, you should call the emergency number (911 in the U.S.)

      @@IMJournalClub thank you and I realized after posting the comment that I was asking question which wasn’t my intention. I merely was just recording my experience but wanted to notify the greater audience of the symptom of cognitive confusion which seems to be widely missed or not familiarly associated with pneumonia. Thank you

      Thank you for mentioning this. Confusion can indeed be a symptom of severe infections, for example in sepsis.

      So after he finished his 5 day amoxicillin and azitromycin his symptoms got worse over the next 3 days without any meds and his temperature still spiked once again which required Tylenol. So now he is on a 10 regimen of Levaquin. I’m praying this works and stops his pneumonia
