    Excellent video...and a really great NPA that is pre-lubed.
    I think the lube might be the reason why it is able to last longer in storage.

  • @PalmettoNDN

    Pre-lubed is fine, but it's what you already have that matter until you run out of it or replace it. I use simple tape to hold my lube tube to the NPA. That way it isn't floating around my IFAK and I have both needed items in my hand when they're needed.

    I had a post nasal bleed that was clogging my airway, which I had to clear to breath, thus preventing clotting. I wish I'd had one of these.
    Horrible vacuum clearing failed in the ER. I had to use the vacuum to catch the clot and remove it myself through the sinus. I'd lost enough blood to get a plasma infusion, and I'd removed the clot in early shock. Two inflated balloons and emergency surgery for cauterization later, and all I could think of was how I could have breathed and let the vessel clot with a the hemostat and nasal pharangeal tube.

    One option for lubrication of an NPA in an active killer event is to use the patient's own blood as a lubrication for insertion of the NPA. KY jelly is also good to keep in your kit.

    I see a couple or three comments about breathing first, bleeding cessation second. While this doesn't seem to be something too often in writing, my take is if you do CPR while massive bleeding is occurring, what will the heart pump when one had bled out??? My training has been stop the bleeding, start the breathing.

    How big of a deal is it if you use expired stuff? What about stuff with water damage such as guaze? If you used sealed guaze but that had water on it that soaked through but kept dirt particles out could normal antibiotics kill any germs if medical staff were alerted when they arrived? For instance my pack isn't waterproof and stored in my vehicles truck. Well if it rains a lot it gets wet. I didn't realize this till I opened it and saw wet bandaids and some rust on my shears. I threw away the bandaids and aired out my kit but curious how bad it would be (as long as things only looked wet NOT moldy or stained.....yes I know looking clean isn't always clean)

    • @magnetar2524
      @magnetar2524 4 года назад +1

      TJ Jones If regular gauze (not treated with any other substance) is expired but the packaging is still intact then you should be ok to still keep it on hand and use it if need be. But if the gauze contains a hemostatic agent or is treated with anything else then I would replace it according to the expiry date and use the expired gauze for training/familiarity purposes. If you can see that moisture breached the protective packaging then I would toss it just based on the fact that bacteria loves moisture, or use it for training as well.

    What are your thoughts about someone only first aid certified (did not cover npa) carrying one of these and using in an appropriate scenario? Seems like pretty simple to apply if needed. More along the lines of being covered under good Samaritan laws where things can get grey if doing things above your certified level of care.

      Better to have and not need then need and not have. While you may not remember or have skills to utilize, someone else on scene may have the proper training to use it.

    People complain about expiration dates and it is stupid sometimes. But if I spend thousands for a hospital visit, I want a new gauze or NPA. So there has to be a time to restock. As a civilian it's dumb to follow expiration dates on something like an Israeli bandage or NPA.

    Great product!! So you put an npa before checking breathing (during the A of the March/xabcde) and then if the patient is breathing you put him in the recovery position and if is not you start cpr or you can put it in a second moment?

      Before checking anything in an unconscious person, a pulse should be checked and if none is present, then CPR, it was decided it was kinda silly to check an airway and breathing if once you find they’re in cardiac arrest you’re going right to compressions first anyways. Once you know they have a pulse, then massive bleeding, airway, breathing, circulation, etc.

    • @bobbya8241
      @bobbya8241 4 года назад +1

      The airway and breathing interventions kinda waste time for which doing compressions instead may be indicated, hence, for an unconscious, pulse check, then the rest

    • @alexmariani2807
      @alexmariani2807 4 года назад

      @@bobbya8241 yes, I guess you are talking about the cab protocol for assessment but that's not a lot for the average lay responder is more for health care providers and you are correct when you talk about the priority of the chest compressions for ohca Infact they enfatize a lot the hands only cpr and for recognizing just un responsiveness and ineffective breathing, but I also think that it depends by the level of train that someone has because the more you can do and you are equipped for better is, I think that if someone can use an npa is absolutely a great think chest compressions is the priority for that few minutes but airway are important too so I don't think that it is a waste of time if you are trained to do it

      If the airway is swollen shut for the nasal, check to see if an OPA will work if the throat isnt swollen shut. Other than that you are gonna need to go a much different route for making an airway.

    Question: What agencies provide training to civilians for using a NPA? I have had multiple CPR and First Aid trainings throughout my life, but this has never been part of any of them. Would like to add this to my arsenal by legitimate training protocol and certification.

    • @bobbya8241
      @bobbya8241 11 месяцев назад

      No class really offers certification. However, if you take the health care provider level BLS/CPR these should be covered

    • @blacksheepbasic

      I'm like you. I love training and like having the knowledge and skills. Have you thought about becoming NREMT certified? Becoming an EMT-Basic will give you a wealth of knowledge beyond that of what is taught in a typical civilian class.Of course, you have to find the time, but the training is worth it. I also suggest volunteering a local volunteer fire department to serve the community and get hands on real world experience.

    The contraindication of a NPA in head injured patients is old dogma based on no real evidence. There has never been a reported case of a NPA entering the cranium.
    Otherwise, the idea of a prelubricated NPA is smart. They should also sell pre lubricated ET tubes, Foley catheters, etc
