This is semi-realistic. Things get heavy, you run out of breath when sprinting(even if it's WAY too fast), and it hurts your feet to walk on hard stuff. Although you wouldn't lose hearts from any of this. You would just get slowness II or hunger II/I

Yes, feet get hurt. 5 hearts. Acid rain. Monsters are charged. Ender dragon is 90% smaller and go to the ground. No enchanting. Diamond tools only. (Seems easy, but you will have to find tools in generated chests) Swords, axes, shovels etc. have 0 attack damage and have 10 attack speed. Hoes deal damage, 1 damage point and 17 attack speed. You cannot walk more than 10 blocks straight. That will kill you instantly. Breaking a block damages half a heart. EVERY MOB CAN KILL YOU. Creepers spawn behind you every minute. Every mob that has the get angry mode will be angry and will be angry with you. Regeneration reduced by 70%. You can’t eat golden apples or enchanted ones. Walking on the desert biome for more than 5 minutes will set you on fire. You breathe. You will die if you stay in water for 10 seconds. You can always give up, but remember that every time you die, 5 TNTs will be activated at your current spawn point with the place and YOU. That’s it yet. I’ll try to add more things.
