And some skin actually puts your team at disadvantages. I've fallen for my teammate's skill that's colored bright red multiple times. I actually hate that skin, like it can really throw you off on a team fight

Think of it as if you were ordering something, if the item is faulty you can request a refund however, if the item was damaged because of the customer's carelessness the item cannot be refunded (warranty can only be granted mostly if the item is faulty). In this case the skin appearance in game was bugged so it's a technical error from the game however, the summoner spell selection was completely human error from the player hence why they didn't give him a chance to remake that game for that reason.

Crazy picked one skin and he spawned in game with the other (also some shen skins are pay to lose because for example his sword is more visible so it's easier to track it's position) the other guy just picked ignite instead of smite so it was his fault.

Okay the ruling depends on if it was a client bug or not. Forgetting to take smite is not a bug. The wrong skin appearing in game is a client bug, there have been many situations where playere forget runes and where runes bug out. It varies depending on if the player fucked up or client did. They have video evidence of these things as well.
Select the wrong skin and forget to take smite are totally on different levels of issue. The other team would not mind if you want to change skin, but forgetting to take smite basically guarantee you always lose when it comes to taking dragons or baron.
While I fully understand your point, as a counterpoint the mistake was so punishing to the team that they may as well have called the game there as a loss. Even in low elo that mistake makes the game nearly unplayable and on the pro scene where the enemy knows how to abuse that to the fullest there wasn’t even a chance.
