#37 | Root Canal Treatment for Dentist from the U.S. with Microscope (1/2)

  • I had known this dentist through Facebook for a while. He graduated from a dental school in Korea and is currently running his own practice in the USA. One day, he messaged me saying, "I will be visiting Korea and would like to receive endodontic treatment for my lower left second molar from Dr. Yoo." Although I felt a bit pressured, I told him to come to the clinic. He arrived in Korea at 5 PM on July 2nd and came to our clinic at 8 PM. Additionally, the treatment had to be completed by July 5th.
    During the history taking, he mentioned that he had received deep margin elevation and crown treatment on the distal area from an associate doctor, but he experienced sensitivity to cold. The X-ray revealed a C-shaped canal. The mesial margin was opened, and the distal margin appeared to be in composite. I decided to proceed with the endodontic treatment, anticipating imminent pulpitis if left untreated.
    First, I removed the zirconia crown and accessed the canals. I found the ML canal to be independent, while the MB-D canals were connected. The pulp was trapped under the cavity wall, and removing it enlarged the access cavity slightly. Initially, I was able to achieve patency in all canals with a 15H-file and smoothly performed initial shaping with PTG S2. I shaped the ML canal with PTG F2 without issues, but the MB-D canal was problematic. I felt resistance and PTG F2 did not reach the working length. Even with PTG F1, the situation was the same. After re-establishing patency with a 15H-file, PTG F1 finally reached the working length.
    However, PTG F2 still could not reach the working length, and continuous attempts seemed impractical. Therefore, I decided to use AJP to establish the master apical file of the ML canal at size 25 and soaked it with 6% NaOCl for 10 minutes. During the temporary sealing process,I confirmed that the ML and MB-D canals were connected.

    저도 해외에서 원장님의 엔도영상들 보면서 늘 많이 배우고 감탄합니다! 저는 그냥 보철만 열심히 해야겠다는 마음이 생깁니다. 정말 대단하십니다 :) 그럴 일이 없길 바라지만 혹시 제가 또는 제 가족이 신경치료 받을 일이 있다면 원장님께 받고 싶습니다. 늘 건강하시고 화이팅하세요 !

    소킹하실때 니들을 근관내에 넣어서 나오클을 근관내에 소킹하는게 아니라 와동에만 뿌려주시는건가요?

      Soaking 과정에서 제가 원하는 것은 NaOCl의 chemical effect이기 때문에 굳이 needle을 깊이 넣을 필요가 없다고 생각합니다. NaOCl을 drop by drop으로 사용하시는 선생님도 계십니다. (링크 참고)
