      @@bigrooster6893HD has zero correlation with weight but I did ask the doc if smoking does aggravate this syndrome. Let’s be kind we don’t know his story

    I have HS and get these on my inner thighs (thankfully not much anymore because of my own awesome derm!) When they get to this point, you can't even think. Your brain is just PAIN PAIN PAIN PAIN. It kinda makes me chuckle that you are asking all of these questions about the light being too bright, the tenderness of it, and I guarantee you the only thing in his brain is RELEASE IT ALREADY! It feels sooooo much better when they finally open up, that nothing before that even matters anymore. I love that you do this for your patients.

      @@skyemccoy5992 well I want to say that I am under the supervision of a dermatologist, so please don't do this without talking to one -- I am on an antibiotic every day, the first half of the month I use Hibiclens and the 2nd half I use a 10% benzoyl peroxide cream. I am also on a med that is made for gout. (the antibiotic is minocycline and the gout med is colchicine). I also wear TomboyX underwear that are like boxers so my legs stay dry and don't rub together. I hope some of this helped! Also, my doc said the next step was to start doing an injectable med, but I haven't needed it yet. I hope yours gets better, I know how much it sucks!!!!

      @@saskiadavies111 yeah, I am very lucky. The other miserable part is that when it's on your thighs, and they are uncontrolled, they bleed all the time. I had to be very careful what surfaces I sat on and always had to check if I bled through. It's no fun.

    The gentleman was so incredibly stoic. Even though he said it didn’t hurt much I’m sure it hurt like heck. He appears to have a high pain tolerance. Such a strong person. You are always so kind and caring to your patients, they are fortunate to have found you💙💛

    My bff would get these in the crese of her skin and she could move and it would send a shooting pain through, you could see her body jerk when it happened, some were so painful her husband would have to bust some of them with a needle.
    I lost her to breast cancer 3 years ago.

      I had the same thing happen when I was 20 right below my abdomen. Couldn't even button my pants. The needle for the numbing was horribly painful and I cried.

    I really like how patients will ask questions and otherwise engage with Greg. That means he’s built a rapport with them or projects a mellow demeanor. The poor guy was a trooper because that looked agonizing! It’s great that he was willing to share a vulnerable time with us so we can better understand HS. 🤩

    Hi Dr. Greg: My daughter has HS and goes to UNC/Chapel Hill, NC for help. There was a doctor there that would remove her skin and let the wound heal from inside out. That worked awesome as the areas never has had any return of HS. She has had several spaces on her body down this way. However, the dr. was a 3rd year resident and moved somewhere to critique his technical skills. You are awesome and I love watching your videos. Beth/NC

    I really feel for this guy as he is dealing with a lot at once. He has clearly had fairly major operation recently and a huge cyst right along his surgical incision, plus other cysts below and I’m sure elsewhere. That’s a lot to deal with at once. And he must have been in a lot of pain regardless of being numbed up. Hopefully this gives him a lot of relief and is the end of all his health issues xx

    This was ANGRY before you opened it up! The waterfall of goo and cyst material that started at 3:45 was amazing! It immediately deflated once you got everything out. He will definitely feel better tonight. Can't wait to see the follow-up ❤

    Ouch! That poor man having to deal with the pain of surgery and abscesses all at the same time. That area looked incredibly painful. He handled that like a trooper. I wish you a full recovery sir. 🤗

    I have HS. I get 4 or 5 outbreaks a year, but none this large. Mine appear in my urinary area that make sitting and cleaning extremely uncomfortable. They are SOOO very painful! I feel for you Sir! 🙏🏾

      I'm so sorry. How horrific especially in the area yours are. Must be a nightmare keeping it all clean and having to be so careful where you sit. Hope you are feeling better. I have several issues unrelated to HS but none the less my pain in the pelvic, lower spine, and urinary area make my life a living h#ll many times. Constantly have to worry about surfaces and hate using public restrooms. Chronic pain & worrying about infection is just the worst. I get it & it sucks. So sorry you deal with this.

      @@debbiefox6846 I'm sorry for you as well! I wish I knew what triggers them to appear. I've read that they occur more often in overweight Black women. That's me! The boils are usually about the size of a grape. Since they're way too painful to squeeze, I've found that using Vicks Vapor Rub helps them to come to a head and naturally rupture. Good luck to you!

      ​@claudiaharrison5765 honey, I has HS as well and have gotten the worst on my inner thighs/pelvic area and they are just horrific!!!! Have you tried a topical called "Boil Ease"? If, not give it a shot! It helps with pain and helps clear em up.

    The release must of been the best feeling ever for this man, know when i release mine the relief is nearly instant (i tend to get a inflamed cyat/abscess about once a month due to HS and a number of other conditions like diabetes). Thanks so much to him for sharing 😊

    You're the best, Doc! I know how painful HS is, & that abscess was so full & so hot. The way he jumped in pain when the 1st Lido shot went in made me so sad for him but you made it go away real fast with your wonderful bedside manner. This was another great video.     oooooh i didn't expect to cry. Jesus i just want to hug him. When you sliced it. I know in MY SOUL that PRESSURE RELEASE felt like everything but hurt all at the same time.My very first episode had to be drained like this under both arms at the same time. I was so scared. As some with HS who diagnosed herself in 2009 with NO HELP FROM ANY OF MY DR.'S. this made me tear up. Your such a good dr. I can tell. I wish I had you!! I can tell because you never once mentioned surgery. this is exactly how you handle something like.

    I had a friend that I would accompany to his HS drainage appts. His normally near lymph nodes in his underarms. Just from observing the immense pain of the procedure, it always made me wonder why there aren't more stages with anesthetic in between the digging and expression of pus.
    I wont even get into the chronic pilonidal cysts he would have to endure and packing removal was always an added treat.

      I had a pilondial cyst 20 years ago and I wouldn’t wish that sort of pain on anyone. I was initially misdiagnosed and then a NP found diagnosed me correctly. The smell alone was brutal.

    I just had to do a similar drainage oit of an axillary abscess for a patient with likely HS. It is so hard to get these patients really numb in the area and you feel bad hitting the worst pressure spots. Needs to be done for many though. Thanks Greg and thanks to your patient.

    You're so gentle with the needle Greg. When something is already do painful, I'm sure that first poke was painful, but then you insert the next poke into the already blanched/frozen skin so he doesn't feel it? What a difference!! You are so kind and only think about your patients comfort.

    My daughter has this and it is horrible. I thank him for sharing to make people aware of how severe this can get. She had it on her tail bone and had to have surgery. Doesn’t this have something to do with the hair follicles?

      Yes your daughters probably did have to do with hair follicles (pilonidle cyst) 😊, hidradinitis is a little different but just as painful 😢

    I'm pretty sure I have HS but my doctors just shrug (and I have some other serious stuff taking precident) so from someone that gets these super bad under my armpits..... That guy was a champ. Good job!

    Your patient is so very tough!! I cringed for him! I have had several abscesses drained in my life since around 4 yrs old! He is not alone and definitely has lots of prayers given up for him!! Hopefully he will not suffer from anymore in his life!

    Poor guy. My sister had one like this in the same spot and as much as I love popping, when she asked me to help drain it she was in so much pain I couldn't keep going. His literally deflated though so I hope he was feeling better after!

    Wow, that looks painful, you were amazing, thanks for sharing, you were smart seeing Greg as he worries about your comfort, not causing any extra pain. I just got diagnosed with diabetes last year and everytime I Greg you mention how much more difficult it is for surgery it scares the heck out of me. After worrying about diabetes in my family since twelve I have watched what I ate and exercised trying to avoid it. Unfortunately due to lack of doctors and waiting 2.5 years for injections for bladder ulcers, the pain was so bad between it and PNE, I was bedridden gaining forty pounds, voila diabetes, other than every six months someone looking at my numbers and giving me medication, I get no direction, help from anyone familiar with it, yes so I am terrified. Wish we had doctors who cared half as much as you do about people/ patients in your position, really admire and respect you. Hope this patient heals quickly

    I just had one of these removed from my back in February. Then last week, they went in and excised it. The worst part was the numbing.

    Humira has COMPLETELY changed my life. My HS is FINALLY under It's also keeping my Psoriasis under control. I feel horrible for anyone who also deals with that nasty disease. I wish I went to a doc who does vids of procedures because I would happily have my HS and the abscesses it's caused over the years to be filmed. I'm just happy people are FINALLY learning what HS is, how excruciating it can be and see the tunnels and damage it causes as it tracts under the skin. So much of my body is covered in scars and I'm embarrassed to show my skin. My armpits, under my breasts, my lower belly (before I lost the "apron" from having my son), my groin and even my rear end are covered in scars. I didn't try the Humira for years because the side effects terrified me. Once my Psoriatic Arthritis and HS got bad enough and I'd been fighting the HS for literally 20 years, I finally gave in. I'm glad I did. 22 years after my HS started when I was 18 and pregnant, I FINALLY have remission. I do want to have surgery to remove the most damaged skin now that I'm not longer flaring 24/7/365. I still have occasional flares of the HS but as quickly as they start to push a nodule up, they clear up.
    Thank you to your patient for sharing! I understand it can be very embarrassing and extremely painful. I'm grateful so many are finally realizing they have this disease and see what happens and the pain it causes. I've known two people that I told them they have HS and to get to a dermatologist IMMEDIATELY. They had the start of it and were able to get immediate treatment at my dermatologist's office. I just hope theirs NEVER gets as bad as mine. I'm a Hurley stage 2 that is borderline 3. I could even be considered a 3 if we go strictly by the guide of stages. I have a massive amount of tunneling and tracts, etc.
    I'm a bit confused by his situation because I've never had any cysts around my HS nodules and abscesses. Is that normal for some or is this the result of cysts vs HS? How do cysts work with HS?

      Lucky! It made both my dermatitis and has worse! My body fought the drug like a disease.
