- In this episode the surgeons discuss the popliteal cyst, aka Baker cyst, aka the bump behind your knee. They discuss the cause of Baker cysts and how to treat them. Mainstay of treatment is to treat the underlying cause. If excised or drained they usually come back unless you treat the cause of the cyst. They are benign and can often be ignored.

I went to the doctor today for my left knee. She said it was Baker's knee. First time hearing of it. I'm currently in pt for my right knee. I hurt my foot back in April. I'm a runner and I haven't been able to run since April and now a new injury. Another setback!! Praying I'll be ready to run the Army ten miler in October. She put me in a brace and instructed me to take Ibuprofen 800 and ice it front and back. I'm kind of venting. 🙏🤞

I went to an orthopedic doctor & he gave me a shot of cortisone in my knee. Symptoms were pain swelling mri, said I had 2 tares in my meniscus &a leaking baker's cyst. He then said I will have to have cortisone shots every 6 mos. for 2 yrs.! Then have knee replacement! I'm 66, I wanted to go back to school & become a phlebotomist that's out. Even holding a part time job in a doctor's office. We had slymby but with my SS check of $317/ mo. took away all of our coverage & assistance which both of us worked all our lives. My husband had hip perthes as a child & spent 3 years with his foot lifted up his back. They told him he'd have chronic arthritis at 40 or so & it would only get worse, it was at that time I stopped working because he had only to sit in his truck as the foreman but I, in retail worked harder for half the amount. I worked too hard. I had women come over to me & say I was making them look bad. I was born at a time when women were fighting for equal rights and I was all for it. But I thought of physical too! So I ended up as 22, a cart collector & bagger. I pushed as many carts as I possibly could & banged them with my hips! After a few months of this I awoke with a bed filled with blood! I also awoke nights screaming in agony in my shoulders that was more painful than I could ever be specific but I hadn't any health insurance I just had to stay home & take it. I eventually found out I damaged my uterus & would have to lay in bed in order to keep from losing my baby. Oh I would get pregnant but I'd lose it & we couldn't afford for me to have just 1 child. My husband had outdoor work and was laid off every year around Thanksgiving/Christmas so we would never have even 1 child. My husband refuses to believe any of this for reasons I won't go into. But my point is that here I am again laid up with a double torn meniscus & no doctor to treat me. They used to repair the meniscus, scrape off the arthritis/ remove the baker's cyst but now I am bed ridden, gaining weight which will lead to diabetes no doubt, no exercise. I have gained 34lbs. even though I do 35-50 sit-ups every other day but what else can I do? If there's anyone out there who can help please get in touch.
I had ACA, then it went to medicare/Medicaid but as soon as I turned 66 they cut it all off! Thank you Kevin McCarthy! @@mikethebike2456 I still have the cyst but it is a little smaller. I had a cortisone injection in my knee in July 2021. It helped the joint feel much better. But I do still have the cyst because I have arthritis in the joint.

I've learned more from you both than from the doctors I've gone to for my knee problem. Mostly dismissed by them. Thank you for emphasising it's a symptom of a deeper problem. Neither of my doctors seem to care when I asked for help for "my poor knee" as I called it. Life isn't easy. I guess I'll be looking for another doctor when we move next year.

I had a Baker’s cyst and it burned and was very painful. I had a right total knee replacement and I hope it is gone !

@J ber Great that you're recovering well. This Friday will be 8 weeks for me (2 months) since my surgery. I'm coming along, need to be dedicated to doing the exercises and it would help if I lost of few pounds :) The stationary bike seems to work the best for me. Good luck with your recovery.
I was just diagnosed with a large bakers cyst in the back of my knee after 2 years of on and off pain. My calf is larger than the other and the veins in the calf have pushed to the surface, i was very concerned! After my doctor told me the result i felt a lot of relief but immediately she asked me if i wanted to go ahead and get it removed. I asked her if it was truly necessary and we booked a follow up appointment to discuss it further. Im glad i came across this video before my next appointment. I would be happy to live with it and manage the flaring pain if i can!
I am having a right knee replacement in a couple of weeks. I have a bakers cyst on the back of my knee. The Dr. said it may deflate. So, that is where I am at. The video is very informative.

Such good information. I had an MRI first of June left knee arthroscopic surgery the middle of June the end of June postop appointment I have severe pain left calf with swelling going all the way down to my ankle. Had an ultrasound done to rule out DVT and lo and behold it showed a ruptured Baker’s cyst which I dealt with until I had total knee replacement August 5 and the swelling and the Baker cyst completely have disappeared. Love you guys your information is so hilarious and extremely educational

I guess when they put the bar in my knee at this time that they removed a bakers cyst.

It definitely is a thing! My lime size Baker’s cyst has been present for 7 years and is quite painful . Yes, I’ve got arthritis, totally messed up meniscus’s (is that a word) but I’m not bone on bone so not ready for a TKR. Tell me about it after one of you docs have one!
Thank you this video has put me at ease. I saw my doctor today because I have some painful burning to the right of my right knee. After examining the knee she asked me to stand and she pointed out the little bulge on the back of my knee and told me it was a bakers cyst.
She has referred me to get an x-ray. Thank you both for explaining this so very well.

Thanks to you guys I asked all of these questions (draining, injection, etc) but ultimately had a "semimembranosus ganglion cyst" surgically removed last week. Along with a scope to check out an acl tear. I have full trust and respect for the surgeon (finally) but don't these things come back if the source, so to speak, isn't addressed? The recovery isn't so sweet and to go through it over and over seems daunting!

Yes Dana sounds like it might be. Fluid is pushed from inside the joint to the back and it can get trapped there and feel tight and sore. Best of luck. Thanks for watching and subscribing

I developed a baker cyst due to arthritis and took some strong pain medications that have lots of side effects. The doctors did not want to empty it saying it wouldn’t help and could come back. After I insisted finally it took 3 s to do so and it changed my life. I still have the arthritis pain when moving but no more night pains and still pains. It never came back. On the other leg it comes back regularly but I always have it emptied. So much better than medications
Thank you for your information..I got a DVT in my left leg ..which also led to finding out I have blood disorder called factor 5...it’s been 15 years..glad I’m alive it went into my lungs ..very scary. I also have a baker’s cyst in that knee..I’m on blood thinners for the rest of my life...and it’s hard to take an anti inflammatory. I wear a compression sock everyday because of the swelling..sometimes there’s a little pain especially when I excise hard.

In 2008 I had a big huge cyst size of a baseball. I had a lot of numerous aspiration then wrapped,rested kept getting more fluid inside again then surgery but it's a movable cyst that does hurt not much know like it did before. Then numerous ultrasound aspirations on it. Now I only have about 30cc drained out a every year know. I feel happy about it. I know when it gets hard back there that it's time to get it drained out by it hurting me. The way it's hard, stiffness for the knee and have a hard time to been my knee when it gets like that. I had 2 acl repair done have to wait 10 year's for a total knee replacement since I'm 43 year's old. I have oa in my knee's

I think my doctor should review your video..you both are concise and make it is so easy to understand. So happy I didnt get cyst aspirated. Its actually healing.
Yeah, I'm uninsured so videos like this really help. I know I have one hip higher than the other and the domino effect from that and just being out of alignment can/will cause issues like this. Trouble bending knee back, inflammation on outside of knee and behind knee. I stand for hours at work so that doesn't help. Will try ice ( 20 minutes) and then moist heat in front and then in back of knee. Will also restart my exercise/stretching. Also taking gelatin ( 2 tsp. unflavored gelatin + one quarter cup cold water) every morning. Will do this for 30 days and see if I get results. We uninsured folks have be our own doctors. Great info doctors. I've just noticed my inside right calf (gastrocnemius) has grown at least an inch. Over the past 6 months i've been going on walks for maybe 20 extra miles per week, and I've been doing the stair masters at the gym. I've also done one legged calf raises while doing the stair master. My left leg has not seen any hypertrophy or muscle growth. Also, my left leg has also been through achilles tendon surgery maybe 8 years ago so I'm not sure it would be affect the the left legs strength speed unlike the right leg.
I've also noticed that during the body weight calf raises, my left does not sustain the flex as well as the right leg which is bigger. I did not expect such increase in muscle gain from doing any body weight exercise.
Do you think my calf muscle increase in the gastrocnemius is from exercises? It feels firm and looks normal, but I just don't like the bulky look of it and was not expecting the increase of muscle if so.

Okay great video I have previously ruptured my own bakers cyst in error and now about 3 years later I have another. I am a runner and don’t really want to stop running 🏃♀️. I will talk to my doc about it because at 57 I obviously have arthritis or a tear. Thanks for making this video!

Good information, guys! In the instances you perform surgery for other underlying knee pathology and the person also has a baker's cyst, do you do anything to address the cyst or just let it be? Does having one present effect surgery, such as knee replacement, in any way?
Hi Knee Replacement Therapists. Thanks for the kind words. So the cyst is more a result of the problem rather than the cause of the problem. Thank you for the candid video. I found out several months ago that I have a 5 x 2.2 x 3.8 Baker cyst behind my left knee. I waited about 2 weeks before going to my family doctor (D.O) who ordered an X-ray and MRI which clearly shows the cyst. I've been wearing a compression knee sleeve and taking Alieve every 12 hrs for the past 2 weeks. He said that if it doesn't get any better in a month that he would fix me up with an Orthopedic Sports Medicine guy.

@@mikethebike2456 Yes it did, but it takes time, took it months to do so. It's been 1 year now it walking fine, but it's about 95% back to normal.

@@mikethebike2456 My doctor said just to take some NSAID's and rest the knee if you can. I also went on Amazon and bought some of those knee compression supports (~ $15) to wear for a while which seemed to really help.

I had a baker’s cyst on the top of wrist. Doctor said he would drain it, to just look away. All of a sudden I was in immense pain. He had taken a large book and slammed it on the cyst. It was gone, but I thought he had broken my wrist! New doctor now!

Mine ruptured yesterday and I was at work. Actually worked through it my last couple hours and after work drive strait to the hospital in tears thinking it was a blood clot. I was historical by the time I got to the ER!!!! They did blood clot blood panel and after a few hours did ultrasound of my leg and veins. I was so hysterical that a paramedic walked by and ended up staying at the hospital with me. Turns out to be a bakers cyst that ruptured. I have never seen my leg in such a shape. My leg actually bruised from the swelling. For past 2 days I've been elevating my leg and I bought some support stockings and a knee brace. Work tomorrow and I'm not sure if I can do it. My job requires me to walk for 12 hours

Thank you for this explanation re Baker's Cyst. I had a TKR recently and within a month had pain in back of leg. Ultrasound ruled out DVT but did show Bakers cyst. I suspect I may have had it pre TKR as it doesn't sound likely or possible following TKR? If it is a one way valve, does the synovial fluid just reabsorb into the lympathic system or is it there forever? I hope not. Thank you,

Great video! I Tell my patients it’s basically an extra pocket in the knee, and the only reason it shows its because there is sn underlying condition. Can you guys believe there are still some orthopedic surgeons here in spain that operate this?! Maybe Dr Billy Baker is possessing some surgeons.....

Great video. I have been battling a bakers cyst off and on for about 1.5 years. As a cyclist there are times of the year that I am riding more, and of course that is when it bothers me more. Right now my calf muscle has been sore for the past week, and I think it is because I am walking different because of the swelling in the back of my knee. What kind of Dr. should I see to figure out what is going on in my knee to cause the cyst. My hope is that there is some sort of rehab that can be done to stop this cyst... Thank you.

I have a small bakers cyst caused by a TTT/TTO and MPFL surgery I had in August 2019. Even after physiotherapy, I still can’t bend my knee beyond 100deg range of motion. In March this year (2020), I had my tibial screws removed. That helped the nerve pain I was having. But still haven’t worked out the underlying cause of why it won’t bend and why I’m still having tendinitis and bursitis.
Hi Amy. It is hard to say the exact cause of your lack of flexion but it may relate to how much flexion you had before surgery. Once your knee gets stiff it can be very hard to undo that. It would be uncommon for a Bakers cyst to prevent flexion that much. Usually physio and sometimes static bracing can help. Good luck for you. Thanks for watching and subscribing if you did.

Just had a MRI today. I was diagnosed w/a meniscus tear & a Baker's Cyst. I injured my knee 5 wks. ago. No arthritis in the knee. Very painful. I want it gone, already!

Strange that you said you would NOT inject cortisone directly into the cyst, but that is exactly what worked for me after my first doctor agreed with you. 1st doc just injected the knee and the pain would come back, 2nd doc said lets try injecting the cyst directly and thankfully that worked and has not come back in 4 years.
Thanks guys!. i know you're not a fan of aspirating a Baker's cyst but, during full knee replacement surgery and to achieve the best possible outcome for the patient, wouldn't it be prudent just to drain the thing? For some the pain from the BC can be more excruciating at times than the pain from the failure of the knee joint.
Hi Steven. Often during knee replacement there will be a gush of fluid from the back of the knee as the cyst decompresses so it often is taken care of as part of the procedure whether intended or not. Hope that clarifies. Thanks for watching, subscribing and sharing with someone if you did.
Mine occurred after my recent TKR and it’s driving me crazy. My recovery has been going so great, but now that this has popped up, it makes pushing myself to get those last few degrees of bend so painful. It also totally grosses me out. I wish aspiration was an option!

I’ve had my Bakers Cyst for years.Drs don’t want to aspirate as they can come back anyway as I have O A. I do think it contributes to my walking badly though and struggling to go up and down stairs😢😢
I have mentioned this before on one of the other videos and thank you for answering the question I asked. I don't have a Baker cyst but I do have a ganglion cyst on the outside of my LTKR. I noticed it about three months PO. What would cause it? I told you that my surgeon decided to leave it alone and you responded that you agreed but what would cause it? I'm now a year PO and it's still there. Don't think it's growing now but it has grown since the first time I noticed it. Can't keep from worrying about it a little.

Thanks for confirming what my doctor said I had because of Arthritis. When it broke I screamed then called him he said start moving don’t need to see him, it will get better it did but sure hurt.
Now I put my arthritis cream all around knee and massage when it shows up.

I had the cysts in the back of both knees aspirated. My doc advised that the procedure should not be done without ultrasound tech imaging the joint during the procedure. As mentioned in the video there are significant structures that could be damaged going in with an needle in the back of the knee. I have stage 2 OA, looking at PRP injections. I had the cyst rupture in one leg, and yeah, the MedExpress docs told me to go directly to the emergency room, DVT was indicated.

My left knee has been checked for bakers cyst but is not there. That’s great! The front of the knee swells up, and if I’ve been walking/standing for a few hours, I end up in a lot of pain. The swelling will increase a lot and my leg from the top of the knee right down into my foot will be swollen. I’ve had about 5 or 6 falls onto that knee on concrete. Have had physic a number of times and will get the feeling of pressure in the back of the knee. I don’t know if that’s all part of the inflammation or not. I’m 68 and arthritis wasn’t showing a few years ago when I last had xrays of that knee. Guess it’s all part of aging that my knee still bothers me after 10 yrs. thanks for your videos. @@TalkingWithDocs I ended up in ER (as instructed by a Dr through nurses hotline) a couple of weekends ago as I had severe pain in the full length of the back of my left leg. When I described the pain as going straight up the center of the leg without redness anywhere, the Dr thought there might be a blood clot in a vein. Said to get to ER right away. I went to the hospital and spent bout 6 hours there. They couldn’t do an ultrasound as they don’t do ultrasounds t night. So the next morning I was sent to an ultrasound clinic. Thrilled to say, no clot! We have no idea what the pain is and sometimes it travels to the top of my foot just below the toes. I do have Vascular disease in the right leg but not sure about the left leg. Also type 2 diabetes, so could be neuropathy? (I’m falling apart! 🤣😉)
Dont ignore it. They are an indicator of other problems. I didnt ignore mine. I simply didnt even know I had one until it ruptured 10 days ago and now I'm stuffed. I can't walk far at all and pain and swelling of the calf is severe. Subsequent x-ray of my knee shows its ready for TKR. I had no idea it was that bad. Since the rupture, I now have a dry joint which is now also painful. My cyst was measured by US at 165mm X 69mm X 18mm and I was unaware.
You guys are good! Very thorough info but why is ultrasound useless? What about MRI? I have a bakers cyst but docs and physios diagnosing meniscal damage and ligament (after jarring my knee twisting movement). Terrified of the cyst rupturing. Didn’t realise it was common nor ‘a thing’!! Been referred for MRI. Love you guys!
Hi Moira! Thanks so much. So ultrasound is good for identifying blood clots typically. The cyst can often be felt with an examination. If there is concern about a torn meniscus or ligament damage that requires an MRI not an ultrasound. They do rupture sometimes and it will hurt but is typically not dangerous. I had a Baker's cyst several years ago, but I didn't know what it was until Orthopedist Dx'd me. It was extremely painful. He aspirated it, and my extra pain was immediately relieved. I think we removed at least 3 full syringes of fluid. I also have arthritis, and I had an extreme tear of the lateral meniscus which required surgery. Now I've had BOTH knees replaced! I won the genetic lottery, lol.

I also have Lupus, Fibromyalgia, Ankolosing spondyloarthritis (Cimzia not working well - need SI joint injections), Reactive Arthritis (HLA-B27 +), CRPS, Type 2 Diabetes, Reynaud's, Pulmonary Fibrosis from PE in 1994, Factor V Leiden mutation (IVC filtering place), Asthma, PCOS, Spinal stenosis, arthritis, degenerative disc disease, PAD/PVD, hyperparathyroidism & diastolic heart failure. I'm very grateful for every day I wake up and breathe! The challenges are great - it's complicated to explain everything to new medical providers. I've been sick all my life. I had a Baker's Cyst behind my right knee that ruptured. One of the worst pain I have experienced. Suffered for months with it. I was an ESA in a hospital and never missed a day of work. It was over 3 years before both knees were replaced.

I got a baker's cyst about a week ago, left knee, also operated 30 years ago, dislocated kneecap. I cured myself with stretching, massage with herbs and lastly took a foot bath with cold and hot water in tubs, in other words changing the foot from cold to hot and vice versa, so the body got a shock and regains better blood circulation, venous oxygenation. this method usually helps with gout
not to forget the venous valves in the leg that are supposed to work

I have an arthritic bone on bone problem and am scheduled for a TKR in September. What should the surgeon do to eliminate the Baker cyst during surgery? Hearing you talk about a rupture and potential blood clot is scary!

I was diagnosed with a bakers cyst, but I move my leg wrong and it will feel like something pops out of place, causing sharp pain up my whole leg, then I push on it and it pops back, causing pain and swollen knee for about a week. Is this a side effect of the bakers?;

As a teenager, my son suffered with this often, especially when playing basketball. He would end up in emergency, it would be wrapped with an elastic bandage and he would be sent home. They did not do ultrasounds at that time. He is now 51 and has not had this happen since he was a teenager.

I was treated for a Baker's cyst just before my hyaluronic acid gel injections. I so appreciated the doctor aspirating this Baker's cyst (even though over time it may return) via ultrasound guidance, a palliative not a cure. My suspicion is the orthopedic surgeon community would rather opt/promote the total knee replacements over any patient wishing to defer this major surgery for a more elegant/better treatment option that targets only the defective hyaline/articular cartilage defects. Sawing the long bones, sacrificing quadricep tendons, ACL to accommodate medieval titanium implants should be a last resort. The fact that FDA is stalling approval for most health insurance companies approving coverage for PRP or PRF combined with the hyaluronic acid gel injections is telling... The baby boomers are a cash cow for TKRs so "studies" continue to apply pressure to the FDA dismissing the efficacy of PRP/PRF - why? - TKRs are more lucrative for the orthopedic surgeon community. Now don't miss out cashing in on the the baby boomers... Implants are flying off the shelf like hotcakes! An implant is an implant is an implant.😏

what is in the back of your knee?? I'm and avid squash player and continued to play, saw a specialist and followed his instruction to ignore it. one year later, cyst was gone and has never returned!

Great video, I have had bakers cyst for 3 years and I was a exercise freak , but cannot do it anymore. To much pain. I have had 2 open heart surgeries valve replacement ,take Coumadin. The pain is so intense in my leg and not working out I have been do depressed and have high level of anxiety. Please tell me what to do. Thank you.

My chiropractor diagnosed my Bakers cyst....so i googled it and found out that there is not much to do about it. I ignored it best I could until one day, months later it ruptured. Since i had read what to expect i wasn't concerned but, oh my it hurt like hell. Also i could feel the fluid running down the inside of my leg under the skin. A very odd feeling...thought i peed myself at first. The cyst has not returned. Apparently i was one of the blessed ones and not have to live with that terrible pain for years.

I've had a baker's cyst for 24 yrs believe it or not. The first time I had my knee brained was in 1998. A couple of yrs later I was diagnosed with osteoarthritis. The baker cyst would come and go but in 2020 it got really worse. I've known for 18 years that I was going to need a knee replacement, well in 2024 I had my TKR. And the baker cyst is the worst it has ever been. It's hard for me to exercise with the new replacement. I can't walk and I can't sit !!!! What do you do in this situation??? If I don't exercise I won't get my range emotion back. I have tried exercising and stretching, ice and rubbing the back of my knee. This is much worse than it was before I had the surgery. They won't do anything for 6 months!! How do you get your range emotion when your in so much pain and NO i will not take pain medication. I was a drug addict before for almost 20 yrs and I refuse to take it. Ibprofen can damage your stomach. I'm a medical advocate and I know the research and studies on the body and medication. We need natural remedies for once!!! What would you do????
