Last time I checked, gas lines and false walls (and many other customizations) specifically designed to torture and murder guests constitute a confirmed "house of horrors."

What actually happens is that a suspect under interrogation is placed inside the Iron Maiden,
Whilst the subject is pondering what happens next, several kittens are placed inside the Iron Maiden with him and the door is closed. In fact, so many kittens go in for company that all the client can do is sit or squat. A large bowl of milk is placed at his feet through a hatch in the modified torture device, a man who is kind to and even loves cats, then monitors for signs of feline distress. If the unmistakeable sound of a kitten in distress is heard, he will duly open up and then deliver a roundhouse clout to the client as a sign he is to look after the cats better.

I did actually already know the general truth of this fact due to researching a videogame that I am a huge fan of. Vagrant Story is a JRPG on the PS1 set in an abandoned/ruined city, the bonus dungeon area is called the Iron Maiden. Unusually for videogames every single room or passage has a name on the map, including this dungeon area. One of these was an particularly odd sounding word 'Squassation' and I looked it up on a whim. Let's just say it is unpleasant. Looking up some of the other room names that I hadn't given much thought to it was clear pretty quickly that _every_ one of the 40 to 50 rooms was named after some form of historical torture. I discovered that while the Iron Maiden that gave the dungeon its name was mostly a modern day fiction I also learned a whole lot about the gruesome devices and deeds of the past that were not.
Some time after this I started reading a book that began with a 'witch trial' using methods that I now knew the names of (Strappado and its partner Squassation) and after feeling ill for a while I declined to read any further.
Many of the Iron Maiden room names are self explanatory - Impalement, Burning, Chair of Spikes, etc but there are many that you would not immediately know the nature of until you realise the overall theme (such as the one that prompted me to look them up in the first place, Squassation). I'll not describe them here but I'll list a few names (you probably won't want to look them up if you don't have a strong stomach however) - Bootikens, the Spider, the Pear, cleansing the soul (interestingly this one lead to the modern parental threat of 'washing your mouth out with soap'), Heretic's fork, knotting, the oven at Niesse, Pressing, Spanish Tickler, the Wheel.
One particularly gruesome thing I realised was just how many were meant specifically for use on women.
Something else I found interesting in a less hideous way was recognising a device in a Bond movie from the description I'd read. In The World Is Not Enough Bond is strapped into a device by Electra in a sitting position and as she turns a wheel a brass pole is slowly extended into the back of his neck designed to first choke him against the straps holding his body in place before ultimately snapping his spine (luckily he is saved by Hagrid's final act). This device is called the Garrotte.
Another thing I recognised in two separate movies was a method of escaping from a prison cell, you bend or break the bars of a cell by wrapping a large piece of fabric or robe around two bars and insert a stick of some kind through the fabric and keep rotating it so it tightens and slowly bends the bars together. This is a variation of Knotting and while not used on people in the movie examples it was one of those tortures mainly aimed at women and is pure nightmare fuel when you know how it is applied.
8:08 While our gracious host is talking of the coat of shame here you can also see on the middle figure something called the Scold's Bridle or Branks. Another of the devices designed solely for use on women, this apparently originated in Scotland as a form of punishment for outspoken women (scolds). A cage for the head with an extension for holding down the tongue (use of an actual spike for this part is optional). obligatory QI clip - ruclips.net/video/wYpwbPmUa2E/видео.html

As such the Swiss cannot be blamed for it and would not have to supply any kind of reason as they had simply no say in who is assigned. When he did misbehave on Swizz soil, they expelled him.

Making any kind of metal contraption would be a very expensive endeavor. Even a shaped wooden contraption. Torture was a much more ad-hoc kind of affair, using things that were to hand: tables, ropes, hammers, tongs, fire, sticks, animals - or various combinations thereof. Along the way, torturers realized that most people would talk if you just showed them the torture implements, which led to there being more of a display aspect to it. But if people wouldn’t talk the solution was usually very straightforward: crushing fingers, pulling teeth, hot tongs. Those methods work and are super cheap.
Another reason the Iron Maiden is not real is medieval authorities were not in favor of blood, for religious and possibly hygiene reasons. Sure, beheading someone was a thing but that was a quick affair that could be cleaned up and it was nobility that were often executed this way, and they are generally less likely to induce fear of disease. The Iron Maiden would cause prolific bleeding and would be damn hard to clean, making it both expensive and impractical.

It would surprise me if it wasn't used at least once during ancient or medieval times. It's almost like rule 34. However, custom fitted spikes would be less likely.
BTW: we Austrians had a lazy variant: hang someone without hoisting him up. At some point the legs get weka and he suffocate himself. Cheap, fast and easy to implement and yet slow and cruel. It just lacks some flair.

Iron Maidens are both fake and real. The casing is real, but the spikes are not, here is how the story went:
Two archeologist find a Iron Maiden, and one of them basically put spikes on it to make it look like it was a torture device.
That's the entire story. you can research it too, this is just a short nutshell summary of it.

If it was from medieval are, it would be a rarity beyond compare considering what it’s made of. Several hundred kg. of iron, probably heavier than a horse, would be VERY expensive. Don’t even mention the cost of forging the thing, would have to be done by the smith of a castle or the like. Lots of cheaper and more fun torture tools were available.

I had a mentally challenged cat that didn't like walking on our waterbed. She would walk up the rails to the mirrored head board. She couldn't figure out how to get down so she jumped down onto me and dug in for the ultimate long jump. Everytime the fridge cycled or the cuckoo clock sounded also sent her skittering from the room (hopefully she wasn't sleeping in your lap when those happened!) It got ugly. She was the best cat you could ask for till she was in fear of her life from everyday sounds. I can't tell you how MAD I got got when she dug in, me screaming "damn cat" just to hear hubby laughing. She was the kitten of a barn cat born during February. That free kitten cost us $500 in vet bills the first month between the respiratory infections, pneumonia, fleas, worms, etc. I still miss her, still smile thinking about how if you spooked her while trying to catch her she would run to the bedroom walk in closet and lay down flat in the middle with nothing around her thinking she was hiding. She didn't go to the sides where boxes were, just layed in the middle of it in the walking area. RIP Brie, you lived a very loved long life.

I'm sure the tomb like body casket that the person was locked inside of and the screws pierced was made wide and tall to fit to the size of most. Also, there could have been multiples made at different sizes.
