I just wrapped up a campaign about this time last year with Atropus as the BBEG. I didn't use him as well as I would've liked, but still, the image of them fighting on a ruined moonlet full of undead, as they attempt to bring down Atropus' aspect, with the planet below them will forever be one of my favorite images from our games. Also, happy birthday.

The fact that these things apparently show up right before Great rituals are cast makes me wonder what the world look like right before a dude cast Karsis Avatar. Looked like a hellraiser movie I bet. Could also be that those anti-magic worm things they were fighting may have been star spawn of a type, in a weird self-filling prophecy type of situation. THAT would be cool narratively.
Great episode as always guys! Somebody else mentioned this but the 5e star spawn are really like the foulspawn from 4e, so it is an odd design choice to change their name like this. You also mentioned the fact that the larva mage turn into an insect swarm and that it is a shame that it probably can't escape like that. But what if it turned into a swarm of worms instead (basically an insect swarm but with a burrowing speed)? It would be more in line with the design of the monster and make it easier for it to escape. And, I don't know, a big mass of squirming worms would be really gross xD
Dender lives in Kelemvor’s realm and has devoured a sun before. She was a character in the Avatar series. There are also a lot of sentient stars that count as elder evils even tho they are mostly neutral and one is even good aligned
Ao didn't not create all the gods. Even if we ignore pantheons that are present in other DnD settings like the Dwarven and Elven ones that are present in Greyspace, the Mulhorandi pantheon came from another world to free their people who were kidnapped and enslaved by the Imaskari. The Celestial Empire/Bureaucracy of Kara-Tur is also seperate from all of this, if I remember right. Ao is just the overgod of Realmspace, he's basically just there to manage divine things.
The 5e version of these annoys me because these are not the 4e starspawn (found in MM2 on page 195 and in MM3 on page 184) - these are the 4e foulspawn (found in MM on page 112). The 4e starspawn are so much more interesting in my opinion.
About to run a 1-17 campaign featuring star spawn. Starts after a falling star lands near Waterdeep. Really looking forward to it. Topping it off with the Emissary.
It's sad how the 3.X's Alienist PrC is always so long forgotten. Skip the whole worshipping and sacrificing parts of the deal and get right in to the summoning of Lovecraftian horrors. Hell, even have your familiar be transformed in to an adorable little Shub goatbaby monstrosity. The Star Spawn feel like someone wanted to add Lovecraft monsters to D&D without knowing anything about the history of Aberrations. It's some of the most on the nose, uninspired, stuff to come out of D&D and that's taking it as an aside from 4th as a whole.
Bane’s position might be interesting depending on certain player choices in BG3. The dead three stand to gain or loose everything based on what I can put together
An Atropal, and Atropus, The World Born Dead, are two different things. The first is a monster with stats that you can fight and kill, is most commonly a stillborn baby God, and is definitely NOT an Elder Evil in its own right. Atropus, the actual Elder Evil, is as you said an undead planet, that may or may not be the severed head of the Prime Mover, the missing head (Haha) of the human Pantheon, creator of the human race and one of Ao's lieutenants. Both by virtue of its easily god level power and ridiculous size, Atropal doesnt have stats and can't be defeated in the manner of a regular monster. Similar names, but two very different beings, which you kinda conflated into the same thing. Also you should check out the Star Spawn Emissaries from Van Ritchen's guide to Ravenloft. Adds 2 additional, much higher leveled types of Star Spawn

the novels covering Fourth edition, the spellplague, were great x.x Each change made to forgotten realms wasn’t the retcon its being made out to be. many of them were stories six books long. It sucks that this narrative exists, many of them are on audible, give them a read
The real truth abouth starspawn isnt that they were foulspawn in 4e, is that theye were an even more obscure thing called an Ushemoi in 3.5 but never mentioned again. I swear if you look them up! they are exacly like the seer, grue, and Hulks
For attacking a creature that's in the same space as an ally, couldn't you just treat it the same as if your ally was grappling it? Maybe I'm not up on the rules but it seems adding a chance of hitting the ally would do the trick.

There was a time when new Planescape content would have been super exciting but after the way WoTC butchered Ravenloft and Spelljammer i kind of want them to just leave the classic settings alone. The Ravenloft reboot took a deep detailed gothic setting and turned it into a series of shallow scooby doo episodes. Spelljammer wasnt AS bad but had very little of the charm that it originally did..
I always saw them as more bubblegum pop meets emo with other true emo bands like The Used, MCR, etc standing a bit apart from them. But Emo isn't my genre so my knowledge is limited.

Yeah it is an odd design choice. And I agree on the swarm of Insects being a bit disappointing. The worms sounds like a good homebrew option.

The star spawn emissary in Van Richeten's guide feels closer to 5e Starspawn - slightly reminiscent of the Emissary to Caiphon
