@mimi19891987 the directors wouldn't have had Jack and Lily in an intense physical struggle, since the actress was quite pregnant at the time. I think Kelly Rutherford (Lily) is a great actress, and I think Lily didn't scream at first because she was caught off guard and trying to deal with the situation on her own by reasoning with him. She only screamed when she saw that wasn't working. Also, sheer terror can sometimes steal/soften your voice :(
