Edit: Okay, I figured it out, he used his reverse Wheel of Fortune? (which does a random dice room effect) and I guess the dice room was One, which rerolls all your currently held items, and I guess one of his items became Missing No which eventually rerolled one of his items into TMTrainer and after it rerolled again, since TMTrainer cannot be rerolled, all of his item became "corrupted" which somehow killed him. What a sequence

D4 effects in repentance after unlocking TMTRAINER are like a bell curve where the value of the effect goes up when you get more items, but then starts going down when you get too many cause of the chance to get TMTRAINER lol

Always dodge parasite on breaking runs, that's the one that crashes the most in my experience
Parasite, Haemolacria, Curse of the Tower with too many bomb effects. All as good as a crash really. Then you get the other end like Mom's Knife and Ludovico Technique which kill synergies instead.
It's good to know albino's luck ALWAYS chooses the least probable path for him, good or not.

So, at first when he used reversed wheel of fortune - that's at 12:00, then again he used joker, which gave him missing no. which gave him TMTRAINER at start of descension, which rerolled every other item into corrupted version. You can see Missing no. in his death recap
