I think what happened is you gave him the wrong amulet since they were all ever similar in description and if I remember correctly in a past episode it said what it looked like. Got to love that attention to detail. So far this mod is amazing .
You gave him the wrong necklase thats why he died. When he introduced himself to you at the start of the quest he siad his necklace had waterbreathing. Tldr in the water section you picked up alot of look a like necklaces and you gave him the wrong one so yeah you indirecty drowned him

You should drop your armour and stuff somewhere near by before getting in the water so you can swim faster.
