I believe the only change on UV is the amount of damage received by the player, no additional monsters added, which is unfortunate. However, there are optional parts of campain that will allow you to encounter extremely difficult sets of monsters. Also, some of the finished levels are available to complete any time during campain from the Doom Fortress computer as Master levels, with significantly increased amount and type of monsters in them. At the end of each level, usually before entering the portal, you can teleport to any area of the map via quick transportation, and collect everything you missed without dealing with monsters. Just press Tab and then F to do that. Pretty cool feature of the game if you ask me :)

Yes, you can always replay a mission after you complete it. Inside the Fortress of Doom you have a computer with a Mission Select option.
