Psilocybin and Antidepressants: What Patients Need to Know

  • What does psilocybin-assisted therapy look like when clients enter the clinic on antidepressant medication? Common practice is to tell patients on SSRIs and other prescription drugs that they must stop taking their psychiatric medications before they can safely take psilocybin, but neither anecdotal evidence nor the scientific literature support this. Researchers and practitioners alike are eager to find effective evidence-based treatment to assist people with anxiety, depression, and other disorders that require pharmaceutical intervention.
    Psychedelic Support hosted this conversation on May 13th, 2022 with integrative mental health professional and psychedelic facilitator, Dr. Erica Zelfand. We explored the the interplay between antidepressant medications and psilocybin. We reviewed the pharmacology of these substances and consider new research on how clinicians can understand and utilize modern psychedelic research to assist clients.

    I have been depressed for a long time, but after taking shrooms few months ago, l feel much happier and highly motivated and my ADHD gone , lost a ton of anxiousness and had a few epiphanies about how I should live my life. I decided to buy an ounce for backup, but haven’t yet felt the need to take any more since then.

    I swear : Sertraline changed my life in ways I hadn't imagined possible. At 46 y-o, I finally became my true self, without the constant and paralyzing anxiety triggers. But I feel like there'a still a part of me that hasn't fully blossomed, so I decided to try psilocybin microdosing, to hopefully improve my focus, memory function, creativity and to be less disorganized.     Psilocybin saved me from years of uncontrollable depression, anxiety and illicit pills addiction. Imagine carrying heavy chains for over a decade and then all of a sudden that burden is gone. Believe it or not in a couple of years they'll all over for treatment of mental health issues.

    This is a great interview - very educational. I say microdosing is the way to go... I moved through the effects of meningitis with micro-dosing psilocybin. I am 69 now and super healthy with no medications at all. We grow our own now and microdose for evolutionary purposes. It's life's gift for us to get in balance and learn to 'know thy self'.

    I’ve been on an MAOI (phenelzine) for 20 years and it has worked miraculously for my depression, even after 9 types of antidepressants plus 18 ECT treatments failed. Even though I’m not currently depressed, I’d really like to have a psilocybin experience for spiritual reasons. Has anyone here done psilocybin who’s chronically on an MAOI? If so, what dose of mushrooms did you take? I’m concerned about the blunting effects.
    Also, although this presentation says that there’s a risk of serotonin syndrome with MAOIs, I’ve read elsewhere that the risk is extremely low because psylocibin doesn’t increase serotonin in the synaptic cleft. Anyone have info on this?

I was questioning myself about use heroic dose too have a deep trip and work on it sober or microdose ... im now on isrn and modafinil and i smoke tons of weed ... i stop all then after a week I will try 3g to see how it goes ... Ive used before but Im very afraid this triggers schizofrenia .....

      One of the uses of psychedelics is administering psilocybin to cancer patients, it allowed them to confron their mortality, remove fear and anxiety and to die with peace and equanimity. What a gift this is.

    This lecture mentioned that a sudden cessation of an SSRI is "cruel". I tapered off Zoloft last year and it WAS very bad or painful. Migraines, severe flu-like symptoms, dangerous depression & anxiety. Had to begin taking the Zoloft again, and all that quickly stopped. She gives great advice here, and i think She's trustworthy.

      I took 75 mg. Zoloft for ~5 years and ended up weening myself by halfing all 30 pills for next 2 months. Then I started thriving, divorced my mental verbal abusive husband of 17 years and I'm happy & healthy now for the last 19 years 👏😁🙌🏼

    Amazing Doctor.
    Not many know about 10% a month reduction of AD’s.
    Especially the SERT occupancy, the lower you get ex. 5mg Escitalopram to 0mg is NO, NO!
    10% a month from 5mg to 0.02mg is the way to go.
    I know it will take years to come off this but is a safe way to avoid withdrawals.
    You can try every 3 weeks and do twice, if no withdrawals...try every 2 weeks and do it twice. If no withdrawals or minimal, you can do 10% a week reduction and you will be out of the meds faster.
    Good luck!

      Thank you for this, I started by going down to 1/2 an ad but awful withdrawal. It’s hard to tell if the psilocybin is working while trying to get off venlafacine 75mgs :(

    Thanks so much. This has cleared up so much misinformation about how tryptamines function versus Antidepressants. The understanding of how psilocybin functions at the synaptic cleft is a crucial understanding. I was one of those who, four years ago, got off my 3 meds, and went to Costa Rica to do Ayahuasca. Then we were up sold to come back 3 months later, this was Rythmia , and then the crisis came while I was there. Psilocybin is a different animal than Aya. Thank you for the knowledge.❤️

    Thank you, this was very informative. I'm a long-time user of escitalopram and I'm looking into the possibility of tapering the dose while I am micro-dosing with psilocybin.
    I have tried "cold turkey" and thankfully I had no real negative side-effect besides vertigo during the day. I am back on half the dose I used to be, and I function quite well, similarly to the original dose.
    You have given me great insight into how an SSRI works in the brain and what the long-term effect is. Also, I have learnt what the negative effect may be to quitting cold turkey, so also a big thank you for that.

      @@rickyjenkins5501 just been notified that lexapro blocks most of the effects of microdosing. But can be helpful when tapering off, and could have a positive exp some time after getting off the lexapro.

      @@danselzer I tried 3.5g while on Lexapro and mostly experienced a little bit of weird mental discomfort. I tried it again last year 2 months after going cold turkey off Lexapro, did 4g and it WORKED lol. It would have been a very positive experience if I hadn’t done it by myself with no prior experience.

    This was a very informative and easy to understand presentation, thank you for that.
    I would find it interessting to know the risks and benefits of combining classic tryptamines (LSD, DMT, Psilocybin) with mood stabilizers. I'm taking valproate due to BPD.
    There are many controversial meanings when having bipolar and taking psychedelics.

    This was very informative. I take medication for bipolar mood disorder. They include Quetiapine (Dopaquel 300mg at night), Escetalopram (Lexamil 10mg), Lamotragine (Epitec 100mg twice daily). I started taking a new psilocybin capsule yesterday and felt nauseas, had severe headache, temperature rise and faster heartbeat. I find the SSRI alone does not relieve my depression and that's why I want to supplement with mushrooms. But it is not making me feel good. Maybe my brain still needs to get use to it or I should lower the dosage. Anyway, thanks for making this video. I also need Dormonoct (Benzodiazepine) to sleep at night. My dreams are more vivid and I often wake up from them so I have disturbed sleep. I will keep experimenting with all the tools I have. Thanks again.

      @@Camilashein-xh4wwI was on 200mg Zoloft and started doing this. Eventually I got hooked on the combo because, for some reason, ssri’s and shrooms made me feel amazing. I felt as if all my problems went away, and after battling severe depression for the last 5 years due to my dads death, I felt as if I found the cure. I felt as if I could be truly happy. they drastically helped my life in the beginning. Long story short I started abusing them, was taking 2-5 grams of shrooms daily for the past year and a half. I started back doing music, became delusional as if I was going to make it and started to go crazy. I ended up getting sent to a psychiatric ward, PLEASE if your son has underlying bi polar do not have him mix the two. In my experience, the good and happy feeling I got was actually a manic episode. Today is 2 weeks that I’ve been off them and honestly, it hasn’t even been that bad. No withdrawals, maybe the occasional sadness but that’s it. I still want to do them, but it scattered my priorities and I learned a valuable lesson. In life you cannot cure depression. It’s apart of us as human beings. It’s best to use in moderation and not be an idiot like me. However I’ve always suffered from addiction. Not even hardcore drugs, just weed and bouts of Xanax here and there. Don’t do drugs kids

    This is such good content. Thank you. I have a question for you. A friend of mine (lol) started taking psilocybe mushrooms for the first time, including a few macrodoses and regular microdoses while on an SNRI. My friend started having vivid dreams every night which has never been the norm. Even after discontinuing the mushrooms for a week, the dreams persist. It's not a negative thing, but very interesting. In relation to your comment about remembering dreams being a sign that you can taper off SSRIs more quickly, what could be a cause of the change in dreams? Thank you again, this info is indispensable!

    Effexor f’ed me up not only with the side effects while taking it (numb emotions, night sweats, decreased focus , and others), but also with major withdrawal issues. She makes it sound like ssri’s are harmless.

    I can't find a source anywhere for any good psychedelics in my area, I suffer some pretty bad depression and i got a chance to try K and man it was a miracle substance, I felt free,the only high or euphoria was from the relief of my vices being released, that's exactly what it did

    I was hit by a drunk driver back in 1984. I was abused by alcoholic family. I have been traumatized when my father beat my mother in front of me when I was 2. The drunk driving accident caused nerve damage left side, bones through my skin, severe head injury, along with all the physical and emotional abuse Ive had over 40 years.. I started micro dosing several days ago.. .15 g. I feel different.. No negative thoughts, depression, anxiety. I need to do this consistently.. Somebody PLEASE help me feel normal.. PLEASE.

    So question re gut health and mind. I had ulcerative colitis and no longer have my colon. How does this affect gut health and depression? As well as I cant eat insoluble fiber so nutrition is a concern. Are there any books that anyone recommend address these things? Im just off cymbalta and now am ramping up on Prozac and want to try mushrooms. I don't want to go off Prozac. Been on antidepressant for over 10 years. My bf took 3 grams and it was very intense. Is it better to try 3 or will it do nothing?

    Appreciate this. Been very curious/worried about microdosing while taking SSRIs. I've been nervous about SSRIs in general and have been sitting on a Lexipro prescription for a week. But this makes me feel far more comfortable and even hopeful that the two different methods may be supportive of each other in some ways. Though it does seem microdosing may not be effective [.15mg] when combined with the lexipro [5mg] so there may be some adjustments to be had there. I'm frankly not sure, beyond the first day, how much the microdosing has helped, though I've been very cautious with dosage and schedule so that may be part of it too. Regardless this is fascinating and hopeful! Thank you for your study and for sharing!

      So how has your expreience been? I am on 10mg lexapro and I want to start microdosing. I'm in the early part of the video so I'm not sure what she says about SSRI's x microdosing. However, I gather from your post that you've starting dosing while on Laxpro. What have you experienced?

      @@EdwardBose So I was microdosing before lexipro. I was afraid to start the medication so I wanted to give it a shot first. I'm on 5mg of lexipro. I've only been for a week and a half so far. I decided to wait until I adjusted before attempting to combine the 2. Sorry I don't have a practical response, but I do intend to give it a shot in a couple more weeks.

      From my own personal experience with over 10 years of experience with trying to get off of various doses of celexa.
      I had the brain zaps, the nausea, the dizziness, everything. It was absolutely horrible and debilitating a lot of the time. I finally found a doctor, who was willing to help me taper down very very slowly, and I started taking the supplement Sam E while I continue to gradually taper. I had to taper down over the course of a year in order for me to finally be able to do it with very minimal side effects. That was about 10 years ago and microdose thing is much more studied now, and much more of a known thing. I had had experiences with psychedelics, but never in terms of using them as an antidepressant. Although in retrospect, they definitely did help me get through some major issues in my life.
      My main point here though, is that, if you have not yet started your prescription, I would wait. I would try microdose thing first and then if you’re still thinking, you really need the antidepressants then give them a try. I’m not a doctor, so I’m not going to say that this is medical advice or anything it’s just from my own personal experience I would strongly discourage anybody from ever starting to take anti-depressants of any kind ever unless all other options have been exhausted. They are a nasty medication. That very rarely actually helps people. In fact, the placebo groups in the studies they’ve done I’ve done just as well, if not better than the actual group taking the real medication

    One time when still in my teens I was able to use psilocybin. I am saddened that I never had an opportunity to use it again, nor did I have a prescribed opportunity. Where I live in North Carolina there is little hope that the doctors treating me will prescribe psilocybin for me. I have incredible difficulty stopping the use of cigarettes.

    This video contradicts what i know about tripping under SSRIs . Everybody says that they cannot feel anything at all if taking SSRIs and psilocybin. Even if thay ingest 40++ grams.
    So many studies confirm what I just wrote.
    So where is the truth? I don’t understand. I listen carefully to the dr in the video and she seems so convincing and convinced.
    Big big question mark ⁉️

    Thanks SO much! This video was VERY informative and helpful. I've been prescribed a variety of antidepressants over the past few years, primarily because I am very sensitive to the side effect of 'nausea'. I just recently had a scope (upper and lower) which revealed I have chronic gastritis-undefined. Is this a result of antidepressant use? I'm currently taking 10 mg Trintellix. Will micro-dosing psilocybin help or harm? What dose of psilocybin would be a good starting place?
    Also, you mentioned a 'Jamaican' trip. Where can I get information regarding that experience? I go to Jamaica frequently and would be interested in learning more!

    Thank you for putting together such an informative presentation! I've struggled with depression for decades and was prescribed fluoxetine, which helped with anxiety, but did NOTHING for the depression. I tried psilocybin mushrooms with positive results. I'm interested in how muscimol works differently to achieve similar results, as I understand that it works with GABA, instead of 5-HT2, receptors, and is commercially available in most states.

      @@olivia-rm7dk The muscimol is much easier to access, and can travel through the mail. It works similarly psilocybin and can move through the mail. It works well for me. My question was almost an aspirin or tylenol one, as both much mushrooms seem to achieve similar results, but why is one illegal and the other not?

    I was really hoping combining psilocybin and Trazodone (SARI) would’ve been discussed. It’s not an MAOI, but also not technically an SSRI. Can I take psilocybin if I stay on my current dose of 50mg of Trazodone per night (for insomnia)?

    I’m interested to know what the conflicts or benefits would be taking psilocybin while on PDE4 inhibition medication. As I understand it PDE4 inhibition medication is mainly an anti-inflammatory mode of action drug.

    Thanks for answering so many questions! I’m on fluoxetine 20 mg and was afraid to try psilocybin because of serotonin syndrome. I’ve been up to 40mg on the fluoxetine, but began to feel too numb. Def want to try the shrooms ❤

      Get off the Prozac. Decreases your lifespan by 10 years due to cardiogenic effects. You're worth more than an early death caused by big pharma's greed. Definitely try different approaches. SSRIs will give you rebound depression. Really bad. Hope you make it.

    Im currently weaning off 20mg of lexapro and microdosing psilocybin at the same time. Im taking 25 mg of psilocybin 3 times a week. Is it safe to take 50mg of psilocybin or even higher? Or do i run the risk of serotonin syndrome?

      Hi Diana, i am on SNRI (Efexor/Venlafaxine) since 9 years and i am wonder if i can use mushrooms as i am suffering again from a severe depressive episode. I tried so many AD the last 20 years but nothing really helped. can you share your experience with using mushrooms whole on SNRI? For a pity i can taper them as they cause bad withdrawal, I hope to improve under mushrooms and to be able to get rid of this poison one day. thanks alot, Hasna

    I just started taking Lexipro for the first time in my life. No side effects really. No major change in feelings other than real life changes making me feel better. I don't think it's pushing the needle as much as I thought it could. I tried to eat shrooms and do LSD and i couldn't get high. Is it the medication stopping it?

    How long is chronic SSRI use? I had Zoloft 75 mg for 5 years. I weened myself for 60 days. I've been fine for 25 years since.
    However, looking to get 🎇 fireworks.
    I'm going to try 3 grams as soon as they're harvested.

    Time: 56:00 slide about root cause: you say that need to address the root cause, and if it's chemical, or nutritional then psychedelics wont work, BUT!
    our conscientiousness/ mind belief and perception controls our chemistry,
    and the level in which we are connected to our body makes us aware of what we need and dont need to eat, which will solve nutritional in-balance.
    your approach is pragmatic for a good reason, but still quite materialistic.
