Your video came on screen as I lay here having taken mobic for pain in my left buttock radiating directly across to my left front side and down the back left leg and down to the front of the foot. Once again, I'm on pt with deep therapy in my left buttock. I cry from the spot they find and put pressure on. I carefully walk out as I loose feeling in my leg and will fall. I thank you for letting me know what is wrong with me. I've been going to specialists in Texas since I was 40. I am 73 now and retired at 60 from school because of the falling and constant pain. You have been the only physician to give me knowledge of what could be wrong with me. You are certainly a physician specialist who has a huge heart and wants to help the human race. ICU nurse here. Diagnosed with Congenital Spinal stenosis, facet joint arthritis, Degenerative Disc Disease, and herniated L3-L4, L4-L5, L5-S1. I’ve lost 170lbs due to gastric bypass. I’ve tried: PO prednisone, steroid injections, radio frequency ablation (2X), 2800mg Neurontin, Vidicon 5mg (2x daily), PT, tummy tuck that improved my core muscles, laminectomy and discectomy (2020). - had a year of improved life quality. During surgery, the neurosurgeon found pieces of my disc broken off that he had to pick out of my spinal column. He said my back was “decorating”. Since surgery, I’ve felt my back and also heard my back clicking- as if it feels unstable but surgeon was not concerned when I brought this to his attention. My left side was the one that was always effected. Now recently, bad new flare up. This is now on my right side. No relief despite now trying a decadron shot and PO decadron. Also back on Neurontin 2800mg which was previously stopped following surgery. I’m only 33 years old and I can’t even get up to use the bathroom without crying. At first, I had a couple hours in the day that the pain was manageable. Now after 2 weeks on disability, I can’t walk at all. Also currently trying CBD oil because I’m open/ desperate to not be in pain. Have an appointment to see a new Neurosurgeon at the beginning of August but just seems a long wait time with how progressively worse it has gotten. Being stuck in the bathtub for an hour because I can’t get out. Can’t stand long enough to brush my teeth. Very difficult time. I’m so young and I just want to be an ICU nurse to help others. I feel like I can’t even get help myself.
@@neo36 Since this post, I’ve gotten a 3 level fusion and screws in my pelvis. I’m still on Gabapentin and very little pain medication despite what I’ve gone through. The surgeon said I was the worst exaggerated lumbar lordosis case he’s seen and the hardest surgery he’s performed in his career due to the curve of my back. I’m walking now but still have a long way to recovery. My surgeon saved my life and I remind him every time I see him.

I'm a psychotherapist. I can't tell you how many depressed patients often middle-aged men who are blue collar workers come in very depressed and start talking about how bad their back pain is. Many have become so discouraged and depressed because of the chronic pain that they've given up on seeking treatment. Some have develop addiction problems as a result. I'm 56 male have been living with stenosis + herniated disc since feb. I have done PT, Chiro, Accupucture, TENS, oral steroids and epidural shots since then. Recently I was told I would need a laminectomy and possibly discetomy. And as I was being told this my employer cut off my health Ins. Talk about being depressed. I feel like I have been nothing but a burden to my wife and kids. I got back my ins today using COBRA and look forward to my surgery which I hope is soon amd grt back to being a productive person again. Oxycodone or Percocet did not help my pain at all I find that hydrocodone 10mg helps to a degree I take half of the tablet having to take the other half of the tablet later as I take a 10 mg hydrocodone I prefer to take it half first to see if it helps if it does not then I go on to take the second half of the pill
@@mstie3008 when you take pain meds daily, you will find redosing doesn’t always work.. so if the 5mg didn’t work, you will have to wait the half life (about 2-3 hours) before the sconce half will have a full effect.. you’re just getting less effective dose doing that, either the 5mg works or it doesn’t and you’re stuck in pain waiting…

I had an injection about 5 years ago because of pain down both the back and front of my leg. It hasn't bothered me ever again after all this time. I hope your long-term outcome will be positive.
Update/Dec 15, 2022.. I feel fantastic, it took about a month for it to take the full effect, I can even think so clearly now.. I don’t think people realize how you can’t think when your in such pain.. I have such gratitude for the skills of the professional that truly know their Kraft!!.. I just pray it continues..
I've dealt with degenerative disc and sciatica. I was heading into a deep depression while trying to deal with the pain in my left buttock and leg. I have dealt with this for seven years. Severe car crash. I am seeing a dr. like you Dr Su and he was helping me but then Covid and two strong hurricanes hammered us last year. I am about to begin my injections again but was told their rate has slowed because of high Covid numbers so it may be a while. Thank you for what you do and for sharing this information. Thank you Dr Su - I have been dealing with both sciatica and lumbar spinal stenosis for a very long time. My doctor had ordered an MRI over 2 months ago. The insurance company's third party denied me getting one. Stated I needed PT for 6 weeks. Had PT, but the exercises I was instructed to do caused more pain & damage. At times, I would leave PT in tears because I was unable to stand upright. Now I deal with limited amount of sleep because of the constant pain. The only medication I take is 8-hour Tylenol for arthritis.

I finally saw a pain doctor and he burned my offending nerve 2 years ago. It’s grown back now and I’m going to have it done again. In my case, it helped tremendously! For the sacro-iliac joint an injection of anti-inflammatory helped tremendously. I’m so thankful I can get by with these treatments and not take pain medicine or have a more invasive surgery. My doc rocks!
My MRI showed what they told me severe spinal stenosis in L 1, 2,3 & 4. Wait I got 3 months to see the Neurosurgeon to figure out what’s next. Dr. Su, I’m blowing away by your approach to treatment and how you ,progress fairly quickly to surgery. Boy I wish I knew you 20 years ago. I have about 8 herniated disks in my cervical and lumbar spine to varying degrees. I have spinal stenosis and multiple bony structures from osteoporosis. I have tingling and numbness in my hands and in my feet. I have leg pains shooting down from my back some is chronic and never leaves. Some is excruciating if I move a certain way. I have lost muscle tone due to my lack of movement. I stumble and have severe spasms in my legs. I ve had bowel and bladder issues for years. I have been on permanent disability for 5 years now. 3 surgeons of declined to help me. They all said “ I can’t fix wear and tear!”. I was on gabapentin for 6 years. Prednisone for 1 1/2 years, that gave me drug induced diabetes. Plus put on 50 pounds. TSH, morphine, Percocet, suboxone , currently Ketamine. I have been written off by so many people. I feel like I’m just not worth fixing by surgeon’s. I even offered to be a Guinea pig for new procedures at the university. So I’ve had cortisone facet injections. With some relief. Ive recently had ablations in several lumbar facets as well, with limited success’s. I think I’m to expensive for the government to fix

I’ve just been trying to work the situation with intermittent activity so that I can avoid home health care as long as possible. My life has totally changed because I can’t be on my feet very long. I have had physical therapy. Sometime that can be painful. I’m afraid of any surgery because I don’t know what added trouble scoliosis would be. I had spinal stenosis for years. But this last two years were the worse. Meds only took the edge off. I did physical therapy for 6 months for some improvement for a short term. I was beside myself in pain and could not function at home with daily chores. I didn’t want surgery especially a fusion. But the surgery I did have changed my life. I had the Coflex device surgery. Out patient surgery. It’s now been 9 months since the surgery and I am great. I walk about 5 miles a day, have been able to loose weight and can do all my daily chores and much much more. The physical therapy I believe help me recover much faster from the surgery. Currently I am on zero meds and zero pain. I’m 67 years old and now I feel like I’m 40. It was the best decision I made to have the Coflex surgery.
I have stenosis with 3 herniated discs. Severe pain in lower back, sciatic and leg pain. It sometimes takes a few minutes to stand up straight from sitting. I’ve been told I need an operation within the next 3 months. I’ve been prescribed tramadol 100mg 4-6 times a day!
This information would have been useful if I'd of known earlier. So far I've had (2) back surgeries and still have no relief. Alot of pain to endure and it's been since 2016. Shortly after I had a 2nd surgery. To top that I've also had multiple hip surgeries which I have severely dislocated that same hip twice. I'm wearing a hip brace and I'm on antibiotics for life. I refuse to have any more surgeries I can barely walk now and I'm a 69 yr old woman. I'd give anything to be able to walk and get my life back with out pain. I have to wait for months to see a neuro- surgeon, this is very educational in the mean time. I was told that although I have Spinal Stenosis, I would be a bad candidate for surgery due to the fact that I am 82yrs. Old. There is a chance I might not survive the surgery. I consulted both a Neurologist and a Neurosurgeon. I am disabled having to use a walker and a cane. My balance is bad, so I fall every once in a while .That’s my tale of woe .
Do it if you can... If you are strong mentally thats 99 percent of the battle. Surgery is always risky. Look for a a second opinion and check your ears with a good eent MD to help with the balance beforehand. Take your time and pacebyourself, tale of woe 😀😅😁 I will pray 4 you.
I had surgery (10 days ago) to have L4-L5 fused, artificial disk inserted and screws. Symptoms - numbness in one thigh, weird 'snaky' feeling down to the inside of my foot, terrible pain when standing in place or walking for 15 minutes. Pain was not shooting like sciatica. It was always there, but screamed at me when I made it work. I found temporary relief by stretching/touching my toes (opened compressed area). My L4 pushed forward over L5, greatly decreasing nerve space. I had 2 steroid injections which didn't help at all. Doctor said all other areas of my spine appeared normal. Still recovering. Surgeon entered my body on one side (less invasive). Praying for positive outcome. Doctor is pleased with post surgery x-ray results.

Thanks for providing real information that will make a difference in my life. I have been dealing with Spinal Stenosis for 2 years. I am in therapy again for the 3rd time. Taking the same meds and done the 3 injections once. Still having numbness and nerve cramps. If I do hamstring stretches 3 or more times a day, it helps. Major surgery I really don’t want at my age. I will add more walking to try to build the core.

I am relating to all the stories here and appreciate all the input. The lumbar spine issues were also derailed because of my Hip MRI which showed partially torn hamstring tendons. I am 77 and every type of pain that you describe is there with burning pain too. I hope we can all get straightened out soon because we want to our lives back again! It is surprising how many younger people are dealing with this.
I did all that and ended up with 4 level fusion with pedicial and rods. Should have done it sooner but always waiting for a better day. All good now after 9 months

I had my surgery 3 months ago! Multilevel Laminectomy ! I tried everything for years! Exercise and therapy helped a lot! After 3 Epidurals , l had to bite the bullet! Back is better but the numbness in my legs and feet is a little discouraging! I’ve had back problems for 40 years and sometimes you have to do what you have to do. I still have degenerative disc disease but l couldn’t hardly walk and standing still was almost impossible plus painful! Every case is different and there’s NO particular treatment or whatever for everybody! In my lifetime stretching is the best way to go . As we get to getting older, it’s easy to to get out of the habit.

Great video, thanks. Yes, you are right, always try out all the other options first. Avoid the surgery. Wait until you have used up all other treatments before you attempt surgery. I had spinal cord stenosis on my L4-5. The doctor recommends surgery. Had the surgery, and right after the surgery, I lost all ability to urinate. When the urine will leak out my penis, I will drain out the urine with a catheter. Lost sensation to my bowels (cannot feel the urge to poop) Had to put a finger into my rectum and feel for shit). That was 11 years ago. I'm still using a catheter to drain out my urine. Both my left and right legs are numb and painful from the cheeks down to my ankle. I am wearing diapers now. I hope my medical history helps anyone of you postpone surgery for back pain and other ailments. Try other options to the fullest before you do the surgery.

I have this video saved. Wish I'd seen this when I had my debilitating episode. This is gold! My chiropractor did the acupuncture but this info makes much more sense now that I know what's at play.
Chiropractors are snake oil salesmen (look it up…it’s quackery) and there has never been a properly conducted research study showing any success for the magic needles. I’ve paid for and had both just to experience these treatments. Please go straight to a good physiotherapist.
With stenosis, spondylolisthesis, and mild scoliosis, I’m doing everything to avoid surgery but it’ll happen eventually. Core strengthening and TENS does help some. Consistency is key!
@@terriliggett544 scoliosis is well managed thru bracing, (corsets). And corrective exs, manipulations and also strength ex's, though after scoliosis has been corrected.

Im 41 my dad always had horrible back problems… im afraid that this is what im dealing with right now… I can’t stand or walk too long before im in excruciating pain. I feel better when I sit down. However, sitting for long periods of time, does make my butt hurt. So much so, after work I have to lay down for a bit. I wasn’t sure if it was ok to constantly take aleeve or advil. I do take a muscle relaxer at night for bed time (fibro issues) . I hate not being able to just walk to the store or even do my dishes and laundry!! 😞😕

Thank you for your information. I have both cervical stenosis and lumbar stenosis. The pain in the lumbar area has gotten worse as time passes. I cant have surgery. I have osteoporosis and osteopenia. Have had epidurals in both cervical and lumbar area. I have heard of the Superian Vertiflex procedure for lumbar stenosis. I am wanting to check on this due to pain everyday and numbness now in both legs and feet

Very active but getting to a point after intense activity it get pissed off and freezes me for 2 weeks. XRay done yesterday. MRI in 3 weeks. I’ll let you know the results. No leg or butt pain. No tingling.
My family has been plagued with spinal stenosis, both lumbar and cervical. Our mother had it and all six of my older siblings have it and each had at least two surgeries. All started showing symptoms in their 40s and I'm now at that age so I'm absolutely freaked out over it. I do try to live a much healthier lifestyle than they do, keeping a healthy weight range from both eating healthy and exercising regularly. I'm hoping to be the exception and not have to have surgery since I've seen very little help it has done for my family.
I have stenosis (L5/S) and it began with a fuc...pain in my right buttock/leg. Not sensitivity in lateral side of my feet. However I practice during 5 years and every day Qigong Shibashi (18 exercises), and it's GREAT!!. I highly recommend you to try the same. I hope you are able to avoid this "genetic" problem...keep focusing on healthy lifestyle. I have heard a doctor say that genetics loads the gun, but lifestyle pulls the trigger. That makes so much sense to me...lifestyle can prevent, or at least delay the expression of a genetic problem.
I am going through this right now and I am getting the transforaminal steroid injection in a few weeks. Your explanation of lumbar stenosis has been wonderful-easily understood and covering all the options. I am on melaxocom and tramadol with mild relief but at 77 years old this has been the worst thing I have been through. I believe I have had this before but for not as long or severe. I used a lateral elliptical type machine at the gym staying on the right side for 45 minutes about 7 years ago. I had a hip MRI which showed partially torn hamstring tendons so the lumbar stenosis was derailed at first. I still wonder if the hamstring issues are still part of this.
Nothing beats weight loss, exercise(walking 2 miles/day) and gut health. For anyone with spinal stenosis I highly recommend these 3 simple actions. Surgery and NSAIDs / Steroids should be last resort. Plus in my experience you will be seeking surgical intervention the rest of your life.
I visited a neurologist and consulted with him about my spinal stenosis. Walking is out of the question. I am 82 yrs. Old and my age might be against me. I may see him again. I used to be able to walk an hour or more. My wife had spinal stenosis and she had surgery and it helped her a lot. I use a cane around the house and I have to use a walker out doors. Don’t wait, see a good neurologist. I was referred to a neurologist first. She had surgery and it didn’t cure the problem but it REALLY helped her.
Dear Dr Su. It is great of you to post this very informative video. I viewed it several times both Part 1 and Part 2 just to get a very clear understanding of your message, which is so very useful to me. I am diagnosed with Spondylolisthesis after the initial X-rays and am awaiting the MRI scan results. From both Part 1 and 2, I am certain I do have L3, L4 and L5 misalignment but will wait till July when the specialist sees me again. For now, I am religiously spending on average 2 hours to give some TLC to my spondy-ed spine. But I am surprised to hear that the specialist who diagnosed, gave me Daneuron 2 tabs each morning and Lyrica 75m per night.
Shew be careful of lyrica side effects weight gain it affects your brain I.e. brain fog and dangerous thoughts of self harm. Also addictive. Trying to wean of lyrica with help from doctor.

Aloha Dr. Su, Great information! Mahalo nui loa! My backstory: Multiple back episodes the last 30 years. Diagnosed with spinal stenosis 8 years ago. Put the surgery off through a thorough multi therapy holistic approach, until I could not stand upright or walk. Had a series of prolozone injections. Helped delay the surgery-Laminectomy L-3-4-5. Laminotomy 5-S-1.Your thoughts on Prolozone Therapy injections for the lumbar spine. Also, what non surgical methods would resolve Anterolisthesis at L-3-4? Annulus fibers tear L-4-5 and ruptured disc L-5-S-1 within 6 months of each other-2 years prior to my surgery. 1.5 years after surgery herniated L-3-4 while surfing. Athletic Trainer and Sports Performance Coach in excellent shape. Have gone back to the prolozone injections since I don't want another surgery-spinal fusion-which I have been told would be very complicated. Should add I have had 3 hip replacements too. Dr Su, you didn't address numbness and tingling as a reason to consider surgery. I have severe stenosis at L4-5, but not a lot of actual back pain unless I overdo something. But over the last couple years numbness and tingling have become constant and I am worried I should consider surgery even though I wouldn't consider it for pain. I'd love to know your opinion! My surgeon says fusion is the right treatment based on the MRI, but he was surprised that I am as fit and active as I am once he met me. I'm 72. I am now walking much less, however.
Hello Dr. I am having serious problems in my lower lumbers i am doing theraphy but slow relief. I try to avoid the medication but my bladder is a problem i hsve to choose places to go... thanks for clarity on my problems.
Stenosis can certainly cause numbness and tingling. Surgery can help pins and needles and tingling but usually does not help numbness. Numbness is a property of nerves that once it's damaged typically stays damaged. Numbness relief is a 50/50 chance with surgery. Fusion is rarely indicated for straightforward stenosis.
I just had my second Epidural steroid injection in my L-3 area today. The first one helped tremendously, a couple years ago. I have great hopes for this one, as the use of prednisone pack helped the inflammation of the area.
UPDATE...well my second shot has been a horrible experience.. Instead of making the pain better, it has been the reverse. I am in more pain than before..Have pain constantly now..Both of my doctors recommended another shot..Nope. I have an appointment in November with a nurosurgeon... Before the second shot, the pain was only on my left side and happened after long periods of standing. Now the pain is bilateral across my lower back and is constant. I am now having to use a walker to help me...

I do have lumbar stenosis! It‘s at the level of L4/L5 and L5/S1. Currently the symptoms aren‘t that bad, but since i do have the symtoms for a relatively long time already, im not sure if i should do surgery! After what time period would you recommend doing surgery?
You should only do surgery if the pain is affecting your quality of life. Another reason would be having severe weakness. Only a spine surgeon could assess that.

Thank you for this informative video. I have had L5/S1 repaired twice. Second one 5 weeks ago but I am still having small muscle twitching and massive calf and hamstring Charlie horses at night. My surgeon feels it’s due to my spinal stenosis and recommends an injection. Wondering if you think it can be contributing to my symptoms or if it can be something else and if you would recommend an injection or Aleve for 14 days. Thank you!
It sounds like it might be nerve inflammation or a recurrent disc herniation. 6 weeks is quite early after surgery and patients can take up to a year to have their nerve pain improve. Typically patients have more improvement at 3 and 6 months versus 6 weeks. As a disclaimer you should always consult your spine surgeon.

I am on 6 months of spinal stenosis and herniated disk. I totally relate with the emotional along with physical pain. I cannot work. Prednisone only worked for 6 days. Nerve drugs didn't work at all.
How long to heal, to see if it worked. 2 weeks since laminectomy. Still have weakness in leg. No pain in butt. Am I being to anxious. And need more time.
It was a blessing. Your video came on screen as I lay here having taken mobic for pain in my left buttock radiating directly across to my left front side and down the back left leg and down to the front of the foot.
Once again, I'm on pt with deep therapy in my left buttock. I cry from the spot they find and put pressure on. I carefully walk out as I loose feeling in my leg and will fall.

ICU nurse here. Diagnosed with Congenital Spinal stenosis, facet joint arthritis, Degenerative Disc Disease, and herniated L3-L4, L4-L5, L5-S1. I’ve lost 170lbs due to gastric bypass. I’ve tried: PO prednisone, steroid injections, radio frequency ablation (2X), 2800mg Neurontin, Vidicon 5mg (2x daily), PT, tummy tuck that improved my core muscles, laminectomy and discectomy (2020). - had a year of improved life quality. During surgery, the neurosurgeon found pieces of my disc broken off that he had to pick out of my spinal column. He said my back was “decorating”. Since surgery, I’ve felt my back and also heard my back clicking- as if it feels unstable but surgeon was not concerned when I brought this to his attention. My left side was the one that was always effected. Now recently, bad new flare up. This is now on my right side. No relief despite now trying a decadron shot and PO decadron. Also back on Neurontin 2800mg which was previously stopped following surgery. I’m only 33 years old and I can’t even get up to use the bathroom without crying. At first, I had a couple hours in the day that the pain was manageable. Now after 2 weeks on disability, I can’t walk at all. Also currently trying CBD oil because I’m open/ desperate to not be in pain. Have an appointment to see a new Neurosurgeon at the beginning of August but just seems a long wait time with how progressively worse it has gotten. Being stuck in the bathtub for an hour because I can’t get out. Can’t stand long enough to brush my teeth. Very difficult time. I’m so young and I just want to be an ICU nurse to help others. I feel like I can’t even get help myself.

@@neo36 Since this post, I’ve gotten a 3 level fusion and screws in my pelvis. I’m still on Gabapentin and very little pain medication despite what I’ve gone through. The surgeon said I was the worst exaggerated lumbar lordosis case he’s seen and the hardest surgery he’s performed in his career due to the curve of my back. I’m walking now but still have a long way to recovery. My surgeon saved my life and I remind him every time I see him.
Look into prolotherapy. Eat a carnivorous diet, meaning no carbohydrates at all.

I'm a psychotherapist. I can't tell you how many depressed patients often middle-aged men who are blue collar workers come in very depressed and start talking about how bad their back pain is. Many have become so discouraged and depressed because of the chronic pain that they've given up on seeking treatment. Some have develop addiction problems as a result. Thank you for this wonderful resource so I can help point these individuals towards better medical care and help them get some hope and find treatment for one of the factors that are making their depression difficult to treat.
I'm 56 male have been living with stenosis + herniated disc since feb.
I have done PT, Chiro, Accupucture, TENS, oral steroids and epidural shots since then. Recently I was told I would need a laminectomy and possibly discetomy. And as I was being told this my employer cut off my health Ins. Talk about being depressed. I feel like I have been nothing but a burden to my wife and kids.
I got back my ins today using COBRA and look forward to my surgery which I hope is soon amd grt back to being a productive person again.

Oxycodone or Percocet did not help my pain at all I find that hydrocodone 10mg helps to a degree I take half of the tablet having to take the other half of the tablet later as I take a 10 mg hydrocodone I prefer to take it half first to see if it helps if it does not then I go on to take the second half of the pill
@@mstie3008 when you take pain meds daily, you will find redosing doesn’t always work.. so if the 5mg didn’t work, you will have to wait the half life (about 2-3 hours) before the sconce half will have a full effect.. you’re just getting less effective dose doing that, either the 5mg works or it doesn’t and you’re stuck in pain waiting…

I had an injection about 5 years ago because of pain down both the back and front of my leg. It hasn't bothered me ever again after all this time. I hope your long-term outcome will be positive.
I had about 7 over a couple of years. I no longer have any problems. I have not had a problem for several years.
Update/Dec 15, 2022.. I feel fantastic, it took about a month for it to take the full effect, I can even think so clearly now.. I don’t think people realize how you can’t think when your in such pain.. I have such gratitude for the skills of the professional that truly know their Kraft!!.. I just pray it continues..

I've dealt with degenerative disc and sciatica. I was heading into a deep depression while trying to deal with the pain in my left buttock and leg. Thankfully, physical therapy, nsaids and 2 tapers of prednisone have given me relief. Thank you for your videos Dr. Su, they're informative and reassuring.

I have dealt with this for seven years. Severe car crash. I am seeing a dr. like you Dr Su and he was helping me but then Covid and two strong hurricanes hammered us last year. I am about to begin my injections again but was told their rate has slowed because of high Covid numbers so it may be a while. Thank you for what you do and for sharing this information. I need to find video number one. Lumbar 4-5 was affected the worst. Thank God for doctors like you because I really don’t want traditional back surgery because I have had so many surgeries right after my accident.

I had the epidural 10 years ago and haven’t had any leg and buttock pain like that ever again. I’m 75 now and have occasional pain but I see a chiropractor to unlock muscles and relax tension.

Thank you Dr Su - I have been dealing with both sciatica and lumbar spinal stenosis for a very long time. My doctor had ordered an MRI over 2 months ago. The insurance company's third party denied me getting one. Stated I needed PT for 6 weeks. Had PT, but the exercises I was instructed to do caused more pain & damage. At times, I would leave PT in tears because I was unable to stand upright. Now I deal with limited amount of sleep because of the constant pain. The only medication I take is 8-hour Tylenol for arthritis.

I finally saw a pain doctor and he burned my offending nerve 2 years ago. It’s grown back now and I’m going to have it done again. In my case, it helped tremendously! For the sacro-iliac joint an injection of anti-inflammatory helped tremendously. I’m so thankful I can get by with these treatments and not take pain medicine or have a more invasive surgery. My doc rocks!

My MRI showed what they told me severe spinal stenosis in L 1, 2,3 & 4. Wait I got 3 months to see the Neurosurgeon to figure out what’s next. Thank you for this series, it is very helpful for me to know what is happening as I can now only walk with a cane and my feet do not want to raise up off the floor for walking.

Dr. Su, I’m blowing away by your approach to treatment and how you ,progress fairly quickly to surgery. Boy I wish I knew you 20 years ago. I have about 8 herniated disks in my cervical and lumbar spine to varying degrees. I have spinal stenosis and multiple bony structures from osteoporosis. I have tingling and numbness in my hands and in my feet. I have leg pains shooting down from my back some is chronic and never leaves. Some is excruciating if I move a certain way. I have lost muscle tone due to my lack of movement. I stumble and have severe spasms in my legs. I ve had bowel and bladder issues for years. I have been on permanent disability for 5 years now. 3 surgeons of declined to help me. They all said “ I can’t fix wear and tear!”. I was on gabapentin for 6 years. Prednisone for 1 1/2 years, that gave me drug induced diabetes. Plus put on 50 pounds. TSH, morphine, Percocet, suboxone , currently Ketamine. I have been written off by so many people. I feel like I’m just not worth fixing by surgeon’s. I even offered to be a Guinea pig for new procedures at the university. So I’ve had cortisone facet injections. With some relief. Ive recently had ablations in several lumbar facets as well, with limited success’s. I think I’m to expensive for the government to fix

Thank you for this video. Very helpful. I have the degenerative spinal stenosis with lumbar scoliosis. I’m 64 now. It can get nasty painful. I’ve just been trying to work the situation with intermittent activity so that I can avoid home health care as long as possible. My life has totally changed because I can’t be on my feet very long. I have had physical therapy. Sometime that can be painful. I’m afraid of any surgery because I don’t know what added trouble scoliosis would be. Years ago I had X-rays done and the doctor told me I’d be in for some serious pain later in my life. He was right. Thank you for doing these videos.

I had spinal stenosis for years. But this last two years were the worse. Meds only took the edge off. I did physical therapy for 6 months for some improvement for a short term. I was beside myself in pain and could not function at home with daily chores. I didn’t want surgery especially a fusion. But the surgery I did have changed my life. I had the Coflex device surgery. Out patient surgery. It’s now been 9 months since the surgery and I am great. I walk about 5 miles a day, have been able to loose weight and can do all my daily chores and much much more. The physical therapy I believe help me recover much faster from the surgery. Currently I am on zero meds and zero pain. I’m 67 years old and now I feel like I’m 40. It was the best decision I made to have the Coflex surgery.

I have stenosis with 3 herniated discs. Severe pain in lower back, sciatic and leg pain. It sometimes takes a few minutes to stand up straight from sitting. I’ve been told I need an operation within the next 3 months. I’ve been prescribed tramadol 100mg 4-6 times a day!
This information would have been useful if I'd of known earlier. So far I've had (2) back surgeries and still have no relief. Alot of pain to endure and it's been since 2016. Shortly after I had a 2nd surgery. To top that I've also had multiple hip surgeries which I have severely dislocated that same hip twice. I'm wearing a hip brace and I'm on antibiotics for life. I refuse to have any more surgeries I can barely walk now and I'm a 69 yr old woman. I'd give anything to be able to walk and get my life back with out pain. I have to wait for months to see a neuro- surgeon, this is very educational in the mean time. The more I know the less fearful of surgery I become ( lots of horror stories out there, including my own father ). Thank God for the internet and thanks for your presentation. Age 71, still working, severe and moderate to severe in several lumbar locations.
I was told that although I have Spinal Stenosis, I would be a bad candidate for surgery due to the fact that I am 82yrs. Old. There is a chance I might not survive the surgery. I consulted both a Neurologist and a Neurosurgeon. I am disabled having to use a walker and a cane. My balance is bad, so I fall every once in a while .That’s my tale of woe .
Do it if you can... If you are strong mentally thats 99 percent of the battle. Surgery is always risky. Look for a a second opinion and check your ears with a good eent MD to help with the balance beforehand. Take your time and pacebyourself, tale of woe 😀😅😁 I will pray 4 you.

I had surgery (10 days ago) to have L4-L5 fused, artificial disk inserted and screws. Symptoms - numbness in one thigh, weird 'snaky' feeling down to the inside of my foot, terrible pain when standing in place or walking for 15 minutes. Pain was not shooting like sciatica. It was always there, but screamed at me when I made it work. I found temporary relief by stretching/touching my toes (opened compressed area). My L4 pushed forward over L5, greatly decreasing nerve space. I had 2 steroid injections which didn't help at all. Doctor said all other areas of my spine appeared normal. Still recovering. Surgeon entered my body on one side (less invasive). Praying for positive outcome. Doctor is pleased with post surgery x-ray results.

Thanks for providing real information that will make a difference in my life. I have been dealing with Spinal Stenosis for 2 years. I am in therapy again for the 3rd time. Taking the same meds and done the 3 injections once. Still having numbness and nerve cramps. If I do hamstring stretches 3 or more times a day, it helps. Major surgery I really don’t want at my age. I will add more walking to try to build the core.

I wasn’t sure if the hip pain (I need both hips replaced, total hip replacements) that was causing the butt and leg pain as well as the cramping in my legs. I am relating to all the stories here and appreciate all the input. The lumbar spine issues were also derailed because of my Hip MRI which showed partially torn hamstring tendons. I am 77 and every type of pain that you describe is there with burning pain too. I hope we can all get straightened out soon because we want to our lives back again! It is surprising how many younger people are dealing with this.

I did all that and ended up with 4 level fusion with pedicial and rods. Should have done it sooner but always waiting for a better day. All good now after 9 months

I had my surgery 3 months ago! Multilevel Laminectomy ! I tried everything for years! Exercise and therapy helped a lot! After 3 Epidurals , l had to bite the bullet! Back is better but the numbness in my legs and feet is a little discouraging! I’ve had back problems for 40 years and sometimes you have to do what you have to do. I still have degenerative disc disease but l couldn’t hardly walk and standing still was almost impossible plus painful! Every case is different and there’s NO particular treatment or whatever for everybody! In my lifetime stretching is the best way to go . As we get to getting older, it’s easy to to get out of the habit.

Great video, thanks. Yes, you are right, always try out all the other options first. Avoid the surgery. Wait until you have used up all other treatments before you attempt surgery. I had spinal cord stenosis on my L4-5. The doctor recommends surgery. Had the surgery, and right after the surgery, I lost all ability to urinate. When the urine will leak out my penis, I will drain out the urine with a catheter. Lost sensation to my bowels (cannot feel the urge to poop) Had to put a finger into my rectum and feel for shit). That was 11 years ago. I'm still using a catheter to drain out my urine. Both my left and right legs are numb and painful from the cheeks down to my ankle. I am wearing diapers now.
I hope my medical history helps anyone of you postpone surgery for back pain and other ailments. Try other options to the fullest before you do the surgery.

I have this video saved. Wish I'd seen this when I had my debilitating episode. This is gold! My chiropractor did the acupuncture but this info makes much more sense now that I know what's at play.
Chiropractors are snake oil salesmen (look it up…it’s quackery) and there has never been a properly conducted research study showing any success for the magic needles. I’ve paid for and had both just to experience these treatments. Please go straight to a good physiotherapist.
With stenosis, spondylolisthesis, and mild scoliosis, I’m doing everything to avoid surgery but it’ll happen eventually. Core strengthening and TENS does help some. Consistency is key!
Consider performing straight leg raises too. At least 10reps and 5 sets.

Great video. I am going through Nerve pinch since 2013 and Physiotherapy and nerve medication has been helping me so far.
Im 41 my dad always had horrible back problems… im afraid that this is what im dealing with right now… I can’t stand or walk too long before im in excruciating pain. I feel better when I sit down. However, sitting for long periods of time, does make my butt hurt. So much so, after work I have to lay down for a bit. I wasn’t sure if it was ok to constantly take aleeve or advil. I do take a muscle relaxer at night for bed time (fibro issues) . I hate not being able to just walk to the store or even do my dishes and laundry!! 😞😕

Thank you for your information. I have both cervical stenosis and lumbar stenosis. The pain in the lumbar area has gotten worse as time passes. I cant have surgery. I have osteoporosis and osteopenia. Have had epidurals in both cervical and lumbar area. I have heard of the Superian Vertiflex procedure for lumbar stenosis. I am wanting to check on this due to pain everyday and numbness now in both legs and feet

Very active but getting to a point after intense activity it get pissed off and freezes me for 2 weeks. XRay done yesterday. MRI in 3 weeks. I’ll let you know the results. No leg or butt pain. My family has been plagued with spinal stenosis, both lumbar and cervical. Our mother had it and all six of my older siblings have it and each had at least two surgeries. All started showing symptoms in their 40s and I'm now at that age so I'm absolutely freaked out over it. I do try to live a much healthier lifestyle than they do, keeping a healthy weight range from both eating healthy and exercising regularly. I'm hoping to be the exception and not have to have surgery since I've seen very little help it has done for my family.
I have stenosis (L5/S) and it began with a fuc...pain in my right buttock/leg. Not sensitivity in lateral side of my feet. However I practice during 5 years and every day Qigong Shibashi (18 exercises), and it's GREAT!!. I highly recommend you to try the same. I hope you are able to avoid this "genetic" problem...keep focusing on healthy lifestyle. I have heard a doctor say that genetics loads the gun, but lifestyle pulls the trigger. That makes so much sense to me...lifestyle can prevent, or at least delay the expression of a genetic problem.

I am going through this right now and I am getting the transforaminal steroid injection in a few weeks. Your explanation of lumbar stenosis has been wonderful-easily understood and covering all the options. I am on melaxocom and tramadol with mild relief but at 77 years old this has been the worst thing I have been through. I believe I have had this before but for not as long or severe. I used a lateral elliptical type machine at the gym staying on the right side for 45 minutes about 7 years ago. I had a hip MRI which showed partially torn hamstring tendons so the lumbar stenosis was derailed at first. I still wonder if the hamstring issues are still part of this.
If you get relief from the injection in the spine then you know it's not the hamstring

Nothing beats weight loss, exercise(walking 2 miles/day) and gut health. For anyone with spinal stenosis I highly recommend these 3 simple actions. Surgery and NSAIDs / Steroids should be last resort. Plus in my experience you will be seeking surgical intervention the rest of your life.
I visited a neurologist and consulted with him about my spinal stenosis. Walking is out of the question. I am 82 yrs. Old and my age might be against me. I may see him again. I used to be able to walk an hour or more. My wife had spinal stenosis and she had surgery and it helped her a lot. I use a cane around the house and I have to use a walker out doors. Don’t wait, see a good neurologist. I was referred to a neurologist first. She had surgery and it didn’t cure the problem but it REALLY helped her.
Dear Dr Su. It is great of you to post this very informative video. I viewed it several times both Part 1 and Part 2 just to get a very clear understanding of your message, which is so very useful to me. I am diagnosed with Spondylolisthesis after the initial X-rays and am awaiting the MRI scan results. From both Part 1 and 2, I am certain I do have L3, L4 and L5 misalignment but will wait till July when the specialist sees me again. For now, I am religiously spending on average 2 hours to give some TLC to my spondy-ed spine. But I am surprised to hear that the specialist who diagnosed, gave me Daneuron 2 tabs each morning and Lyrica 75m per night.
Shew be careful of lyrica side effects weight gain it affects your brain I.e. brain fog and dangerous thoughts of self harm. Also addictive. Trying to wean of lyrica with help from doctor.

My backstory:
Multiple back episodes the last 30 years. Diagnosed with spinal stenosis 8 years ago. Put the surgery off through a thorough multi therapy holistic approach, until I could not stand upright or walk. Had a series of prolozone injections. Helped delay the surgery-Laminectomy L-3-4-5. Laminotomy 5-S-1.Your thoughts on Prolozone Therapy injections for the lumbar spine. Also, what non surgical methods would resolve Anterolisthesis at L-3-4?
Annulus fibers tear L-4-5 and ruptured disc L-5-S-1 within 6 months of each other-2 years prior to my surgery. 1.5 years after surgery herniated L-3-4 while surfing.
Athletic Trainer and Sports Performance Coach in excellent shape. Have gone back to the prolozone injections since I don't want another surgery-spinal fusion-which I have been told would be very complicated. Should add I have had 3 hip replacements too.

Dr Su, you didn't address numbness and tingling as a reason to consider surgery. I have severe stenosis at L4-5, but not a lot of actual back pain unless I overdo something. But over the last couple years numbness and tingling have become constant and I am worried I should consider surgery even though I wouldn't consider it for pain. I'd love to know your opinion! My surgeon says fusion is the right treatment based on the MRI, but he was surprised that I am as fit and active as I am once he met me. I'm 72. I am now walking much less, however.

Stenosis can certainly cause numbness and tingling. Surgery can help pins and needles and tingling but usually does not help numbness. Numbness is a property of nerves that once it's damaged typically stays damaged. Numbness relief is a 50/50 chance with surgery. Fusion is rarely indicated for straightforward stenosis.

I just had my second Epidural steroid injection in my L-3 area today. The first one helped tremendously, a couple years ago. I have great hopes for this one, as the use of prednisone pack helped the inflammation of the area.
Is prednisone safe to take?

UPDATE...well my second shot has been a horrible experience.. Instead of making the pain better, it has been the reverse. I am in more pain than before..Have pain constantly now..Both of my doctors recommended another shot..Nope. I have an appointment in November with a nurosurgeon... Before the second shot, the pain was only on my left side and happened after long periods of standing. Now the pain is bilateral across my lower back and is constant. I am now having to use a walker to help me...

I do have lumbar stenosis! It‘s at the level of L4/L5 and L5/S1. Currently the symptoms aren‘t that bad, but since i do have the symtoms for a relatively long time already, im not sure if i should do surgery! After what time period would you recommend doing surgery?

I have the same thing. Do not sit for a long time. Always sit straight up, not hunch. Do a lot of core exercise and lose belly fat.
You should only do surgery if the pain is affecting your quality of life. Another reason would be having severe weakness. Only a spine surgeon could assess that.

Thank you for this informative video. I have had L5/S1 repaired twice. Second one 5 weeks ago but I am still having small muscle twitching and massive calf and hamstring Charlie horses at night. My surgeon feels it’s due to my spinal stenosis and recommends an injection. Wondering if you think it can be contributing to my symptoms or if it can be something else and if you would recommend an injection or Aleve for 14 days. Thank you!
It sounds like it might be nerve inflammation or a recurrent disc herniation. 6 weeks is quite early after surgery and patients can take up to a year to have their nerve pain improve. Typically patients have more improvement at 3 and 6 months versus 6 weeks. As a disclaimer you should always consult your spine surgeon.

I am on 6 months of spinal stenosis and herniated disk. I totally relate with the emotional along with physical pain. I cannot work. Prednisone only worked for 6 days. Nerve drugs didn't work at all.
How long to heal, to see if it worked. 2 weeks since laminectomy. Still have weakness in leg. No pain in butt.
Am I being to anxious. And need more time.
