US health worker shortage: Filipino nurses say they are being exploited

  • The US has been battling a shortage of healthcare worker for years.
    To fill the void, it's recruited a record number of nurses from the Philippines.
    But labour and migrant rights activists say many are being exploited.
    Al Jazeera’s Barnaby Lo reports from Manila, the Philippines.
    Eleanor Nolasco is the president of Filipino Nurses United - an advocacy group supporting the rights and welfare of nurses.
    She explains what the government needs to do to keep nurses from seeking jobs abroad and that poor working conditions and low wages in the Philippines are pushing nurses to move abroad.

      Exactly truth employers gave $10 gran bonuses then they go back there and recruit then charge again these Nurses with another $10 gran.

      In America if you sign a contract you are on a leash. If you don't like don't sign the paper. All you really need to do is finish the job and you will find a much better job after the contract ends.

      @@pongmolina2157true. Here in Sacramento Valley CAlifornia, nurses are paid high with benefits. Most of the nurses are in the upper middle income class especially if both husband and wife are nurses. It actually depends on where one lives and what hospital.

    I was a nurse in the UAE for 7 years and now a US citizen. The agency that got me here is one of the worst but I can say the workload and being mistreated is nowhere near in comparison in the US than what I’ve experienced in the middle east. Normal work hours in the UAE is 48hrs a week with no overtime pay, here you get overtime pay in excess of 40. Yes, the workload is a lot but a lot of support systems are in place and your voice here is being heard compared to when I was working in an Indian-Arab owned hospital in the UAE where working condition complaints will be held against you. I don’t know where Al Jazeera is getting all this but I belong to a community of filipino nurses who previously worked in the middle east and I’m 100% sure they’ll back my statement on this.

      True I found that strange. My friends said they landed Saudi Arabia the employers hold their Passport right away. Then they ask them if they go back home.

    I work in the Eldercare sector, with Alzhiemers residents. Many of my coworkers have been Filipino, the most hard-working and compassionate people I've had the privlege of working beside. I've heard stories firsthand and in the media how they are exploited and subjected to wage theft and other labor law violations. The U.S. healthcare system is a nightmare, and these luxury Assisted Living facilities charge thousands a month, while the people providing the care services are often paid little more than fast food workers- sometimes less. It's even worse for professionals who come from overseas. I hope someday soon there will be major reforms to this system so corrupted by the profit motive. Both the care workers and the vulnerable people who rely on those services are at risk every day due to chronic understaffing, low pay & burnout from heavy workloads.

    This happened before in the late 1990’s when the US needs nurses, these despicable Filipino recruiters take advantage of these hopeful nurses. The Phil gov’t should put a stop to these recruiters. There is no regulation at all. The US should also investigate the contracts of these recruiters or staffing agencies. I know for a fact that they pay these nurses half the running rate in the state they are hired in. This is unconscionable. 😢😩

      The Philippine government don't really care about the nurses. They do not even help improve the working conditions of their nurses in their own country. The filipino nurses are highly educated, under paid and abused by their employers. The recruiters are probably paying them off too .

    Well it sounds like the problem is the nurses aren’t checking these hospitals/nursing homes/etc out online before they accept a job offer. If a company is making you sign a contract with a big fee for quitting before your contract is up then it’s probably not a great place to work anyway. They’ve clearly been having high turnover rates (because they are a bad employer) so are trying to lock staff into contracts. And foreigners may not know this isn’t normal for hiring in the US so they just sign it & get stuck.

    Nurses here in Philippines paid $200 a month and some considered as volunteers and they don’t receive any money from hospitals at all but all they need is experience so they can apply to work abroad

    This is sad, For my 20 years experience in America - i have not heard of this. Though in Nursing Homes you get more patients, its a shock to all of us Filipinos to get 30 patients. Alamin po ninyo if its a hospital setting or Nursing Home. It's crucial to be informed and discerning about the agencies you work with, especially in such critical fields like Nursing.
    Please don't allow yourself to be in this situation, be bold and reach out to Filipino Community in your State. While it may be true that some reports highlight issues of exploitation among Filipino nurses and other healthcare workers in the US, it's not a universal experience. Different agencies operate differently, and some prioritize the well-being and fair treatment of their staff.
    It's best to thoroughly research and vet the agencies you're considering working with. Look into their reputation, reviews from current or former employees, and any available data on their practices. Hope Ariel is in better situation now.

      In between nursing jobs.. I worked agency. Nurses from agency most times may be given heavier work load as regular nurses know we get paid more.
      Once during a job in ICU, I was given 3 very acutely I'll post cardiac patients.. I felt my unsafe as my license was on the line. After speaking with charge nurse with no resolution.. I walked out of that hospital and reported to my agency the following day.
      Please do not condone unsafe nursing to patient ratio.
      As nurses whether immigrant or working visa... safe guard your nursing license. We worked hard to obtain THIS

    Sorry she didn't answered the last question quite well,it's not really dependent on the applicant ,there should be proper law or contract that is specifically designed that tells what is the limitations that they can do or cannot do

    This is so true. I work at a big hospital in Pennsylvania. When I was at a different unit… a heavier load unit, majority of the nurses I met there were contracted nurses from the Philippines.

    One of the main reason from way back then is that a flood of nurses graduates every year enters the workforce market. Some nurses that have passed the board licensure exam and with ample work experience go abroad to get a much better salary and the new ones can work locally to get experience and these has been the trend eversince.

    lies lies and Lies, Even the worst hospital I have work here is still way better than working conditions in the Philippines, I can say that because I have been a traveling nurse here for years before setting down in a good hospital. Filipino nurses are known to be the best here in US because we know how to work hard. in fairness I've seen few that didn't make it, had to cut their contracts. These few didn't really have a real nursing experience in the Philippines and think that having an US RN license is enough. No it's not, only a real nurse knows how hard it is to Finish a shift with a joy in your heart knowing you have been a help or touch someone's heart. Shame on you complaining about your working conditions, we all know nurses have right here, You probably did something at work and start to point fingers at management's.

    I’m not sure this is completely true. I’m a Filipina American, my sister is an American born Filipina nurse practicing in California.
    I was actually horrified at the poor quality of care my family member received from the predominantly Filipino nursing staff.

    This is why you have to work direct hire---MAGNET status healthcare facilities. There are in-house talent recruiters and they are legitimate.

    The problem goes much deeper than just Nursing in the Philippines. Your gracious interviewee failed to mention that this exploitation starts in the Philippines. Usually native Filipino's who already made the successful immigration act as "job finders" and recruiters. Those contracts aren't drawn up by greedy American hospitals but by the companies owned by their fellow Filipino countrymen. This happens with maids, merchant marines, construction, entertainment, and healthcare workers. Sadly, the country is dependent on overseas remittance. Money earned in another country sent back home. Very little incentive for the government to change the status quo by upsetting the outgoing flow of professionals.

    This is the reason why you should carefully research agencies and facilities before accepting any job offer. Although we are more exploited in Ph, the consequences in making a mistake in US compared in Ph has much more weight.

    I'm from California and this is not entirely accurate. I am not a legal expert but If this really happened, the patient ratio is fair depending which unit you were assigned. If Nurses assigned out of ratio, employee can anonymously report to the State Department of Health. Employment contract or no contract, the immigration status will determine how long they can work for the employer which usually take 5 years (sponsored work permit). During this time, the employer is 100% responsible for the employee (abuse or neglect of well-being). While employed, may apply for permanent residency (green card) which typically last for 4-5 years process. If you prefer not to involved your current employer in your permanent residency, you can asked other person or entity to become your legal support while in the US until employee becomes a US Citizen.

    Well oversupply of nurses is the main culprit why there is substandard wages and benefits offered against their profession. Those who plan to be future nurses or those still studying must have to think a couple of times whether the current and near-future conditions surrounding their career is still worth-considering.

    First and foremost before you sign-up the contract I think you really understood what’s written there regarding breaching your contract. All agency they have their agreement if you wanted to move to another hospital if you’re not happy anymore nor can’t bear the workload. Secondly why you will pass the ball to your agency regarding your workload? Why you didn’t escalate it to your department head then to your hrd? Proper channelling first right???? Working here in a government hospital in the us but if not happy nor can’t bear the work load I immediately inform my superiors since I don’t want to jeopardize my license nor the welfare of my patients. Lastly to the reporter did you ask other Filipino Nurses working in that hospital if they’re experiencing the same scenario as ariel experienced? Thank you. Just my two cents. 😊😊😊

    Administration disregards workplace violence as well. Patients always think that they can just abuse nurses. And nurses think especially those who were not born and educated in the US they should just keep quiet even if doctors and patients and their families verbally abuse and sometimes even threat then that they will get fired. They made the nurses feel and think that customer is always right.

    Im sorry for her but i did nod experience those, i came here as an immigrant with my husband, 2 year contract, got what the US nurses benefits have, no placement, flown free on Northwest Airlines business class.

    This is not a fair assessment . It might be from the agency that brought them to this country that's abusing them. I came 4 decades ago. Yes, it is hard work and adjustment but i am still thankful that i left the Philippines to come to the US to work. We have many opportunities here in the US. We just need to work hard and further our education to be able to compete. There will always be discrimination but filipino nurses are resilient ,patient , excellent nurses and our patients like us. We persevere. There is no other country in the world that could compare to the US. Speaking of abuses, The Philippines is the worst. I cannot possibly survive that kind of treatment. The only good thing is our education. We have good education and training but never will i want to go back to practice IN THE pHILIPPINES OR ANY OTHER COUNTRY IN THE WORLD. I am eternally grateful for the chance and opportunity to work in the US.Thank you America!

      @@Cnichal Because we do. IT is common sense USA corporations import nurses from phillipines because they are cheap labor. This maximizes profits for the hospitals that are run like factories. That is why hospitals do not hire enough nurses on purpose to overwork the nurses while at the same time maximizing profits. Hospitals are not in this to help people they are in it to make money. That is why healthcare in USA sucks.

    They are not treating you like a slave. It is more like you are allowing them to treat you like one. Anyone here, even fellow Filipinos, will give you more patient loads when you are quite new. Or either cant nor want to express their issue. I make sure I always check the number of patients assigned to us. Otherwise, if I feel overwhelmed or I feel the assignment is too much, I talk to my supervisor. Problem solved.

    UK LIKES Filipino nurses. They should move there instead. There are videos here on RUclips about Filipino nirses working in UK. One clip I remember is a high ranking British nurse in a hospital was saying that she really like how hard working Filipino nurses are AND she is even trying to convince the Pinoy nurses to stay for good in UK. ❤

    1. His recruiter gave him a bad contract with a strong negative covenant. He was foolish to sign that contract with that recruiter. That isn’t the hospital’s fault.
    2. In the US, nurses can be in unions. Join the union if your facility has one.
    3. You get treated far better in the US than you will in the Middle East.
    4. The PH government should be regulating these recruiters and/or providing legal guidance to nurses. Why isnt the PH government and OFW agencies doing more?

    @00:25 "They are treating us like slaves...like we are cheap labor..." Welcome to America! Also, I think Asian ppl, especially SEA, forget that they are not Yt. When you work as a brown or black person in the US, you need to go above and beyond to prove yourself and you can't expect recognition for those extra hours, etc. If not, they just replace you with another person who looks like you who will just be happy to have the job or a Yt person who they will not expect as much from but give them the same wages and benefits that you got doing more work.

    I dont think this is the majority of the Filipino nurses.. i know coz i was once a US RN since 2003 and became a practicing doctor since 2009.. as a nurse in US i did not experience the alleged exploitation and abuse/s those Filipino nurses should file grievances to appropriate authorities in the hospital and appropriate US government departments.. these work abuses and exploitations are real and hope no one will undergo those bad experiences especially Filipino workers!!

    Super short staff dito sa US, some filipinos who came in here are in contract, 3 yrs max... exploited kame kasi walang gusto mgnurse na local, pero once u are out of contract, dun lng babawi, healthcare in general all over the world is trash. Nagtitiis lng kame, dhl sa opportunity.

    US, I think is the best place for Filipino nurses. My wife and I are nurses and practicing since 1995 in the US. My advice to Filipino nurses who are planning to go to US is read your contract and understand it carefully. Get referrals and google search the agency. Second is to have at least 3 recruiting agencies to compare before signing. US has a strict Federal, State and local labor laws that hospitals and recruiters have to follow so if you feel abuse or being harassed in your work place speak to your charge nurse, director then HR. You can also file a complaint with local department of labor or get a legal representation. It’s okay to speak up or voice your opinion in a respectful manner. It’s against the law to retaliate against you. You have rights as a permanent resident.

    hmmm sorry, that's reality everywhere here. Even the Teachers who were hired here. They regret coming here because they were put in the remote areas with low salary just higher a little compared to Phil. Teacher Salary. Their consolation is they just here for 3 years and they are going home.

    That 's true I've been working in the US for 32 years. Work is so much but benefits sucks! One of the company that doesn't treat their nurses well in Kindred Healthcare. This company is a long term acute care facility. They treat their nurses and other workers like slaves.

    Even if the government will pay them 60k - 70k pesos, they will still get out of the country and go to the US because they will get 250k in the US, they will be able to have a good life … get a big house, a luxury car and luxury bags. The Philippines will never be able to compete with that.

    ? How is it slave work If you’re getting paid? It is normal if a corporation to pay for your schooling you must enter in a contract to stay working for them for X amount of years or you will have a penalty. This is no different. I hope he gets to go back home once his contract is up so the mean United States won’t take advantage of him by allowing him to make sooo much money here

    Hmmm… If I may say… he has the right to speak up. Whether you come from the Philippines or anywhere else ,your voice needs to be heard. There is what you call racial discrimination and if one feels that he or she is being treated unfairly , he can report to higher authority .and they listen . Now a days , nurses here in general who works in nursing home and hospital can have an overload. They may have 3-5 patients in a hospital setting shift but they do everything when they are understaffed. Meaning Nurse / CNA / everything..the pay is good but you have to be strong specially mentally. Filipino nurses are highly praised because of their work ethics - hardworking and patient. on the other hand there is also a negative side . Other co workers who’s not Filipino think that because you are not complaining you’re good and you can take care of the job. What I like about the other race is that they are straightforward . It’s a yes or no answer- take for example - you’ll be ask “ can you do an overtime today? “ to them straight away will say” No I can’t today but for Pinoys - it’s going around the bush. You end up working overtime because of an added pay or most times it’s our nature to please .

    Middle Eastern hospitals and medical care facilities also underpay nurses and over load their schedules. The only good thing is that they provide housing for them and airfare once they are hired.

We are born to take care of our seniors. We never send them to other peoples homes or businesses to be taken care of.     The USA is not the life that the "American dream" is portrayed to those abroad, lower paid workers are all expoited, service workers having to rely on tips to live rather than proper pay and the lack of regulatory holiday time off paid.
    My advice to any Filipina nurse is forget the USA and forget using a Philippines agency but to contact agencies in European counties or the UK or direct to the Health Services of those counties.
    You will be better treated, better working conditions and much safer counties to live in.

    Filipino nurses seek employment abroad due to the salary and benefits which the Philippine government could not do so. Nurses in the US esp Filipinos should be more assertive and speak up for themselves here in the US. I learned that.

    Hahaha I work here and I don't think this is accurate. If anything, I think every place has some lazy people but based on my experience, I have never been abused or treated like a slave. Also, we have more rights here than in the philippines. 5 patients to 1 nurse ratio is nothing compared to the 20 patient per nurse in the philippines and getting paid like a highschool graduate.

    Unfortunately they cannot just walk across the border like all the illegals and virtually impossible for Filipinos to get a visa to the US so many resort to getting ‘jobs’ in the US through recruiting companies which are able to ‘buy’ the work visas to bring them to the US under contracts which pay the nurses (and others) low wages while the recruiting company makes big profits. National Park system continues to give contracts to these recruiting companies so it is really the US government which is the problem.
    Filipinos want to come to the US to work, not live off the system like the illegals and so many of the US citizens. Open immigration to Filipino nurses, problem solved. And yes they dont want to stay in the Philippines where the minimum wage is $10 per DAY
