    Awesome explanations of disc herniations. I've been a medical massage therapist for 30+ years and often have patients who are being treated by physicians and Physical Therapists. Recently, a young man came in with an injury he obtained at work (he's into heavy construction). A surgery was performed on his lower back herniations. A year later, he's still in pain, so came to me. I have worked on him restoring muscle balance, etc. to several muscles that are tight/loose. I also have done some scar tissue work. When I wasn't seeing the results I would like to see, he informed me that he had another MRI which said that the surgery didn't "see" the loosed disc materials and they were now "floating" outside the disc. This is what is causing his pain, according to doctors. I am heartened by your info on disc sequestration. I will talk to him about the positives related to this and to release himself from fear. Any suggestions for me as his therapist? Just continue to keep his muscles relaxed and toned? Let nature do its thing? Thanks so much! Have a beautiful and blessed year!

    An addition to my thoughts below: I just now watched your Joey video. That answered my question on what to do. Also, my young patient is pretty in tune with his body and has showed me all the positions, stretches, movements, etc. that make him feel worse or better. Physical Therapy was actually making him feel worse because they were giving him CORE exercises that were too much for him and he came out of each session in more pain. Now I have some additional suggestions for his healing without constantly focusing on his L-4, L5, S1 herniations. Thanks again!

    Thanks so much for putting this together in one concise video! This is so helpful and encouraging for those of us who have been told there's no hope for real healing or that we need drastic invasive measures to obtain it. You definitely bring hope to those that are desperately seeking it.

    Great video Charlie. It's a good way to reframe thinking about this.
    After an sudden onset of acute lumbar and sciatica pain plus lumbar muscle spasm. The scan showed L3/4 bulge and at L5l/S 1 a focal right paracentral disc extrusion. The moderate size herniated disc causes moderate right lateral recess stenosis, with impingement of the traversing right S1 nerve root. I was unable to leave the house for 3 months due to weakness and acute pain. The pain was all the way down to my foot and I had numbness / tingling. I opted for exercises and pain management - a CBT type programme. I found the Robin McKenzie book and did what exercises I could in the initial 3 months and still those that I'm able to.
    Pain is still a massive issue but it did retreated to my mid-back to right buttock where it's plateaud for the last 2 years but it's too uncomfortable to sit for longer than 5-10 mins leaning to one side. Standing / walking is less painful but very tiring after a while. I have been referred back to neurosurgery for a nerve block guided by ultrasound. I've used mindfullness and distraction techniques but the pain is pretty all encompassing and limiting. I also have numb soles and sometimes progresses to the top of my feet which is disconcerting. This started in my right foot but now has transveressed to my left foot. I have avoided Optiates and SSRI meds for pain relief due to side effects and just adds another layer of complexity and I have a dizziness / balance condition where the neurologist told me he didnt think that it would work on the pain as I've become deconditioned and nerves compressed.
    What's your feeling about nerve blocks and if they work, how long they work for? Also, have you found cases where the numbness and tingling don't improve?

    As a long time hands on hospice nurse, I started sciatica pain 10 years ago. It didnt really bother me until 3 months ago after a fractured hip surgery. A hip surgery that I felt railroaded into. Sciatica was still not bad after that till I went to a chiropractor who took x-rays and told me I had herniated disc condition and piraformis syndrome. He uses that instrument that jabs along my spine, then tells me to lay on bed with rolling balls. He does number 4 on each leg and sciatica is now much worse. I have pain on lower legs, ankles and toes.

    I had small disc herniations in my neck starting in my 20's seen on an mri after a car accident. I was told surgery was rare and you'll feel better, they are only bulges. I would go in and out of pain cycles with my neck. One day my left arm was numb and weak and wouldn't stop twitching and tingling. I had an mri again about 15 years later and had 3 disc herniations pushing on the spinal cord causing bruising on the cord. Saw 6 surgeons and all recommended I not wait on a 3 - level ACDF from C4-C7. I wasn't hyper focused that I would end up like this and it happened anyway. I was shocked!
    Now I do pay attention to my pain. I know when my adjacent disc disease is causing me pain at my C3-C4 and C7-T1. My C3-C4 is slightly compressing my spine, but the surgeon I consulted said he wouldn't do surgery since I wasn't showing signs of weakness or bruising. The disc herniation became larger after a strong hug around my neck at work. A woman just shockingly had her husband pass away and gave me a hard squeezing hug around my neck. My employer filed worker's comp and I've been denied coverage due to my previous fusion ect. But my C3-C4 was slightly herniated that way for 7 years and suddenly caused horrible pain and got larger after the squeezing hug, but all I have is my word apparently.
    It seems my spine slowly herniates over time. From these studies you mention you're better off with a large herniation.
    What would be my best course of action to protect myself? I go to PT but it doesn't make me super strong. Is it dangerous to get a physical trainer, would they understand how to help someone with a 3-level fusion in the neck and adjacent disc disease?

    Absolutely Right your thoughts and emotions have a lot to do with your health and even pain level. Years ago I was diagnosed with ulcerative colitis and they told me I would have it forever. I told him I didn’t really have it ..I did fill the original prescription that I never refilled again and so I did my own research and never went back to the doctor for it . I had people that I knew that also had it and they would ask how come you have no symptoms and I am a
    convinces its because I did not own that disease and they totally did by talking about it constantly

    Doctor, very thankful for finding your channel. You are absolutely correct. Before I had my MRI done I was dealing with the pain, however had hope I’d get better. I was diagnosed with a L5/S1 disc bulge. I took a bad fall at work and a couple days later both my feet began to burn. I went to a spine surgeon who had an MRI done. It showed the disc bulges. He said he didn’t recommend surgery and prescribed me 6 weeks of physical therapy. I would go in once a week and the therapist would have me do stretches alone and come and check on me every couple of minutes. I completed the physical therapy however I felt the stretches they had me doing made my lower back pain worse. The surgeon cleared me for full duty and said to come back and see him if the pain didn’t improve and that we would do injections. I visited a podiatrist for my foot burning and a neurologist. Both stated it was due to my disc bulge and said they couldn’t do anything and that I would have to live with it.
    I really felt alone in this and the lower back pain was taking me away from my two children and the gym. I entered a very bad phase of depression which gave me some pretty bad thoughts. My children however kept me going and I was able to somewhat overcome those thoughts. But they still come and go depending on my pain. I still can’t lift in the gym like I used to. That is really depressing me as well.
    I became obsessed with my injury and pain and gave up on the doctors I had visited in the past months. I found Robin McKenzie and Dr. Stuart McGill and bought both their books, “Treat Your Own Back” and “Back Mechanic”. I began following their teachings and began to improve. I also purchase the Teeter LX9 Inversion table. I have been practicing McGill’s Big 3 and McKenzie press ups. Not sure if either are helping.
    I do find that for some reason when I go to the gym and do leg lunges, glute bridges, and body squats, that I feel lower back pain relief for about two days. Does this mean my lower back and glute muscles are weak, causing my back pain?
    I’m really hoping to avoid surgery and get better. I practice good spinal hygiene daily like Dr. Stuart McGill’s book outlines.
    I will be watching all your videos and have subscribed to your channel. I really feel you are one of the few that truly understands disc injury and rehabilitation. Thank you.

      Thanks for watching and sharing your story a bit, George. Sorry to hear about what you've been dealing with - no fun for sure. Unfortunately, your story is not unfamiliar to me... I talk with people all day every day about this back, butt, leg pain stuff.
      Glad to hear you're on a bit of a better path - sounds like you found some good resources! I'm familiar with all the folks/methods you mentioned...
      So are you looking to work with someone new to help guide you in solving this? Or are you trying to just tackle it on your own?

      @@smith2186 hey brother. I am doing okay. It’s been a journey dealing with this injury. A lot of ups and downs. Some days better than others. Usually everyday has some degree of lower back pain.
      It’s a lot of trial and error until you find what works best for you. The McKenzie Method and McGill Method didn’t do much for me. I waisted thousands of dollars in seeing each of their specialists.
      What I am finding now after almost two years of dealing with this pain and injury is that my back surgeon was right. The only hope I have of healing is simply two things, time and not re-injurying the disc. That’s all. I still do the McGill Big 3 but they don’t do anything for the pain. I also purchased a Westside Barbell Reverse Hypertension Machine. This has been very helpful in my rehab. But you have to know how to use it or you could make the pain worse.
      Anyways I’m still here. Haven’t had surgery nor does my surgeon recommend it. I’ve learned to live with the injury and pain in hope that one day the pain with slowly begin to fade. I believe this is starting to happen.

    Good morning, I unfortunately was told twice (two different discsectomys). That without surgery I wouldn't be able to walk, and the neurosurgeon actually pushed disability. (I freaked out! This all started in my mid 20s) I was in so much pain (L5-S1area.) This All started from taking a thirty-five foot fall off roof. I completely regret getting the surgeries. Now I work in EMS and am afraid that I've completely messed up my chances of any of this helping. I do light yoga and try using foam rollers which sometimes helps, but lately with the weather lots of rain. I've not been able to walk as much and now hurting some with lots of lost sleep.

    Great information!! I just learned so much. I'm currently lying down because of a disc herniation. So you said there are stages in the healing process like there are of a broken bone. But with the broken bone, it gets casted and it doesn't move for a couple months. So for my disc to heal, what should my movements be? Don't kneel? Don't bend? Lay in bed for a few weeks? How do we recover from this? Thanks!!

      Hey! thanks for watching. If symptoms are SUPER yucky, some relative rest may be needed... but goal is always to remain as active as possible. So, first step is to begin "tweaking" or modifying how you move. If you can do some detective work to identify why you're in pain and what your sensitivities are to begin with- then this makes knowing what to change and how to change things much more clear.     Are there studies that conclude the longer a nerve is under duress from the herniated material the less chance of the nerve fully recovering after the duress is lifted?

    What about thinning discs? I had a terrible football injury and landed on my neck, now I’m told one of the disc is flattened and akin to the disc in someone aged 65 years. Really bummed out, I was told that the results of the impact trauma, using an analogy, is that of a cat that used up 7 of his lives on a disastrous neck injury. What can I do??

    Hi. Please give me some hope. I did surgery for disc herniation 3 months ago in my lower back. It all went well but now i kind of regretting it. 8 weeks ago they found on my mri 2 disc herniation in my neck! I'm freeking out. Can i heal this naturally?? How? Please help me 🙏🙏🙏🙏🙏🙏

      Hey Rosie! Sorry to hear- yes, unfortunately spine surgeries carry the highest risk and lowest reward (40% ish success rate). Chances are you can still get relief naturally if you follow a process vs. trying random things. What are you current doing to solve this?

      Hey! Sorry to hear! So, if your herniation isn’t that bad on image- is it possible that your herniation isn’t the cause of pain? I’m not belittling your pain- very real… but making you think outside the box.
      My guess is your chance of healing naturally is still very high! Hang in there!

      @@drcharliejohnson I've been using that idea. I've been thinking I have perceived danger pain. And I've gotten results but somehow always end up flared up. I'm currently working with a coach right now. I was getting results and then u played basketball and did some back extensions. And its been hell for a week nos

    The bigger the better... except when not. I had a big herniation that went worse over months, with conservative treatment, and I ended up in emergency with paralysis in legs, bladder gone... Got permanent damage from that. I got shitty discs. Not everybody is the same. Pay attention to red flags!

      @@drcharliejohnson was bit unfortunate, somehow I felt something isnt right with the pain just getting worse over time, cramps coming in, strange sensations just more and more. I did talk about that, but the physios I went to and even the specialist for muscle trigger points somehow didnt react to it. And I had no clue. I remember one of them asked if there was an MRI done and when I said no, he just said "that's good"... well, if someone did they would have seen the mass prolapse and maybe the cauda equina could have been prevented
