If we are writing to a doctor , why we should ask the doctor to wean prednisolone? bec he will examine the pt n decide further care wright?is it necessary to mention smoking cessation to a doctor?
In this letter pt came with cyanosed and spo2 was 84 to 85% . During hospitalizations more than 5 medications and in discharge plan also 5 to 6 medications. Pt was on 02 therapy. This also relevent right? Case note was simple but too much relevant data was there. Very difficult to concise upto 200 words
