For those damaging moments, Wanda was aware and she used her powers well to defend against the attacks, however in this fight hela used her giant spear to stab her, she did not use defense magic and was directly hit, in terms of normal humans there is no time for regenerations for that kind of attack because that would insta kill. As we can see her bones and organs fell out.

With the captin marvel thing, she was using her magic the whole time which we can easily predict that she used her magic for defence right before the explosion. And plus like I said wanda did not use her magic to defend, her bones and organs were out and spear that sized will insta kill her before she even regenerates.
