Best 16 Bit Castlevania IMO I like it better than Rondo Of Blood and Castlevania IV because of its Arcade feel and good soundtrack. Its obviously more expensive these days too when compared to Rondo and Castlevania IV.
The Japanese version is yes, the US version though sells for less than Rondo. The cardboard box variant (Majesco variant) also sells for less than the original clamshell box version. CV 4 used to sell for much cheaper, hell I got it at a pawn shop years ago for under $5, and I bought Bloodlines in 98 (along with Contra Hard Corp, both cardboard boxed Majesco variants) for $9.99, Contra was the same price, brand new from Toys R Us. They were clearing out the last copies of these, there were still plenty left. The only way to tell a MNajesco variant from the original Genesis release, no Konami logo on the back of the cart, and the Majesco carts were assembled in Mexico. Bloodlines is great, but is not without it's problems, like walking through stairs, instead of on them (their feet are actually inside the stairs as they climb them), that and the whole connection Konami made to Bram Stoker's Dracula novel, which wasn't necessary to Castlevania lore. As for Rondo, it is the Holy Grail of Castlevania for a reason, for years it wasn't available outside of Japan, and copies always ran in the triple digits (I ran a burned copy in Magic Engine for years before getting it for the Wii VC, and on the PSP with SotN, and the remake). Couple that with the anime feel, and then exoticness of not being able to get access to legal copies cheaply for years, and you have yourself a hotly wanted item. Another game in the series that has gone up considserably, and quickly, the PS1 CV Chronicles, another one relegated to an obscure to westerners Japanese PC, and one even I didn't know about till it was released that it ever existed on something else prior (thank you EGM for mentioning it back in the day).

Bloodlines, the soundtrack and just the gameplay and using the official 6 button controller which is my favorite controller over the SNES and PC Engine controllers it just edges Rondo IMO slightly. I love all 3 as Bloodlines is one my favorite games of all time on the Mega Drive as Rondo for the PC Engine and Castlevania IV for the SNES. I have been playing a lot of SNES lately and been having a lot of fun playing its best games. The Mega Drive is my favorite system of all time though. PC Engine second and the TGCD playing it at a friends house back in the day was my first Wow next generation gaming when seeing Fighting Street running on it and the amazing soundtrack.
There are lots of small differences. The best one imho is the fountain in the palace in which the water becomes blood (around 29:40) At least my bloodlines version (i have both cartridges) is very tame in comparison. I do not have the new generation which is the least bloody.

Such an amazing Castlevania game. It has superb action and fun gameplay. Feels more like an arcade title rather than a megadrive one. And that's good.

Yes, the Japanese Version of Castlevania: Bloodlines (known as Vampire Killer in Japan) is fully in English.

I have yet to test it, but I think both games work with small changes in american or japanese consoles.
