It's been pretty disturbing to watch. I studied Trans surgery in college (pictures & all). It's still a pretty barbaric & complicated process. Lot's of risks and complications. The hormones are not a cakewalk at all. Yes, there is a trend towards gender confusion happening in our society, but it's still fairly new. This trend is disturbing & children should not be able to make such a monumental decision. And, yes, there are odd people in power that are presenting themselves a certain way. This doesn't mean that every person in the public eye is inverted. Human beings are not Mr. Potato heads. It's just not that easy (at least at this pt. in time).
True, XY and XX are the same before that, the differentiation only comes after 10 weeks, "normally", and in some cases, it doesn't, and the XY embryo becomes a girl despite being XY (because of Androgen Insensitivity Syndrome, for instance, when the embryo doesn't react to the male hormones that come from the father's Y chromosome).

Estrogen is the key factor (or hormone) in determining whether you are male/female ( if specific slit or tissue of interest closed => male otherwise female). In the past some of these parents made a choice to define gender/raise the child in terms of which was larger and followed the appropriate medical procedures; and many have let it be. We are all androgenous as we have X or Y chromosome to some degree. Your environment, how you are raised by parents/guardians, beliefs, etcs. determines whether you will choose to LIVE your life as a female or male, and that is your choice as gender can not be changed, and DNA test will confirm this (male, female, or hermaphroditic). XXY or XYY are endocrine problems (Turner and Kleinfelder syndromes respectively).

Primarily depends on genetics of parents. Take a look at your parents/relatives, for other body structures which includes are they tall, muscular, short, thin and all and all for other physical relevance after all they "seeded" you and you will see the variation in a family (ie brown, blue, green, or whatever eye colors). At the end of day gender is determined by estrogen before you even reach the level of looking at your physical structures.
