Why did her new rule have to take all the quirks we don’t know about except for reflect, he literally kept all the important ones at least that what it seems for now
I think you're right as far as why her quirk attacked his that way. It seemed to me that the implication was she took off her first rule on herself to make her as strong as possible and just made her body as durable as possible instead. So she could let her quirk do work.

AFO has back up plans on top of back up plans sure things didn't work out but I'm sure he has quirks stored away he can give back to Shigaraki body
