De Quervain's (Stenosing Tenosynovitis) Surgery

  • www.handandwristinstitute.com/... - Repetitive thumb and wrist use are very common nowadays, especially with the advent of smartphones and computer use. De Quervain’s tenosynovitis, also known as blackberry thumb or mommy thumb, is a natural development from this trend. The abductor tendons of the thumb can become inflamed as they pass through their tunnel while moving the thumb, which in turn can cause pain and discomfort, hallmarks of this condition.
    Bracing is the most common primary treatment of this condition, as well as the prescription of anti-inflammatories to counteract the symptoms. In about 50% of cases, an injection of cortisone may improve patient symptoms, but should this prove ineffective, then a slightly more invasive procedure may be required. In a short procedure of about ten minutes, the patient is sedated and a single small incision is made along the inside of the wrist, where the thumb is, through which the tendon’s sheath is sliced open to relieve pressure and increase mobility and reduce pain. It is occasionally possible that the affected tendons pass through one or more sheathes and so when releasing them one must make sure to get all of them, or the surgery will be ineffective. Following the release, sutures are used to bring the skin back together.
    After surgery, a splint is used to immobilize the affected area, and in three days, this splint is removed. Therapy will be initiated for four to six weeks, after which time full use should return to the extremity.

    Thank you Dr Knight. I am pretty sure I am going to have to have this surgery. I have already been diagnosed with this and have tried the nonsurgical treatment. My wrist is not getting any better, so the next step is surgery. You helped a lot with your explanation.

    Operation is the best choice. It's just a small wound around 1 cm. After operation, no pain again. That's great

    Anyone struggling and stressed should go ahead and get this surgery. I’m 33 and have been active my whole life. Slowly over time, i started losing the functionality of my left wrist due to wear and tear. It was physically and mentally debilitating. For 3-4 years i stayed away from the activities i loved in hopes that it would heal on its own. Everyone is different, but for me, It slowly got worse.
    I finally bit the bullet and got the surgery and within a week, my wrist felt like new. The only pain i felt from the surgery was from the stitches but that went away around day 4 or 5 post op and it was very little pain at that . But anyway, take it from me and get your life back. It’s a small surgery. Good luck!

      So it’s been exactly 15 days post op at this point and i have no pain. There’s still some slight bruising/stiffness in the incision area, but that should go away within the month.

    I just had this surgery done, back on the 9th of this month.
    But they have me in a plaster like partial cast and ace bandage.
    And I don't take it off and have the stitches removed until the 20th of this month.
    This is the surgery they did, but I also had some other soft tissue damage as well, including 2 partially torn tendons and 2 partially torn ligaments, and the beginning of psuedo-gout all in the thumb and wrist.
    This stemmed from a biking accident, and it never fully healed right.
    Tried bracing, and the injections, and pt all prior to the surgery.
    But was still struggling.

    I had a baby and about day three I started developing pain in both wrists. Got diagnosed and had cortisone shots. It helped but pain came back. (I am a grill cook where I use my wrists soooo much all day) My right wrist is the worst and sometimes becomes stuck in place and it's so painful to manually move it back. (I have to move it back because it's just stuck and I can't move it) anyway, I've got thickened inside my wrists and now cysts as well. Getting my surgery in a couple weeks! I'm so excited!

    Would having PsA or Ankylosing Spondylitis make having this more likely? I've had 3 injections that usually last 12-14 months. This time it's coming back after 10 months. I had 3 unexpected surgeries this summer AND got 2 new grands via adoption that are 1 and 2. The baby has her arms up for Mimi to "hold me", anytime I see her. I can't resist, so I've been picking her up a ton. I suspect between that & the AS and PsA flares caused by 3 major surgeries in 8 weeks, that's why it's comeback quickly. However, it's not bad yet. There's no "marble" on the side of my wrist yet. I usually have a big knot of inflammation on the side of my wrist. I'd like to try to be proactive since I'm feeling that telltale pain when I do certain things, but don't know if that's putting off the inevitable?

    DONT GET THE SURGERY DONE JUST YET!!! here's why.. I had the same problem on my right wrist, then my left wrist. The right wrist got more painful as time went on. I am an HVAC Technician so I do a lot of repetitive hand movements. Anyway, I went and got the surgery done on my right hand about a year ago now. The recovery pain was not that bad but a uncomfortable slight pain did stick around for several months.
    Anyway, I had set up an appointment for the left had to get done.... in the mean time while I was working around heavy machinery I decided to lean up against a pipe and other times a wheel shut off valve. So I would stand up under it and press up against the pipe or Valve and massage my back and trapezius muscles. It would hurt but feel good all at the same time.. I did this about 2 times a week. Within one month the pain on my left wrist had gone away.. so you have to do a deep tissue massage to get the knots out. You must try it before going through surgery.. go see a massage therapist if you have nothing sturdy to press against.. Good Luck

    What risk does cutting the sheath have? I’m assuming it gave some protection to the tendons? The injection helped me temporarily so surgery would be my next step. Thank you

    Let me appreciate your educative short clip to point, i want to know the side effects of this surgery, since i have the syndrome which started on my 8th month of pregnancy and thought it would subside and its not...your answer will guide my decision to do or not do procedure. thank you in advance

      Been having the same issue and our youngest is 2 now it has gotten worse and therapy isn't helping. Ive had 2 sets of injections and nothing helps. Have you had the surgery or received treatment that helped.

      @@jillblank2After the second set of injections I saw the department head Dr for Orthopedics with Kaiser and she did a nerve test (feels like acupuncture) on my hands and said that I actually had inflammation in the nerves (something like tendinitis but she said not quit that ) and that I could do surgery or just let it correct itself with rest and ibuprofen. I wore my hand braces daily and iced for 2 weeks with ibuprofen and minimized using them excessively and I haven't had any issues since. I really couldn't tell when the pain stopped since I changed up my activity it went away on its on with minimal use, meds and ice. My husband would also message my hands nightly. I hope this helps. Oh and my hormones was one of the main causes since I have Hashimoto's and hypothyroidism. So definitely get a second opinion and ask for the nerve test to see what they could rule out. It saved me from having surgery.

      @@mrsmincey9731 This is helpful info! Thank you for your feedback! I have been wearing my brace at night but it is finally getting bad enough that the tendon is snagging and causing more pain. I'll try taking ibuprofen, icing, and wearing my wrist brace more often until I hear back from my doctor. Good to know surgery can possibly be avoided!

    I had this surgery on my right wrist in August 2016, I'm happy that the pain is gone it was so painful, the injection didn't help me after 2 weeks, I hope the scar gets lighter in a year I had trigger finger surgery back in June 2015 on both of my middle fingers and you can't see the scars on my hands at all, and I had a lot more stitches about 5 or 6 in each hand and my wrist only 2 stitches

    Have had D in my left hand for 17 years and now in my right hand for about 5. I worked as a receptionist for years and answered the phone 100s of times a day....then typing...computer....then writing...used my hands for fine motor all day long. I find braces to not be practical...I need to remove them so many times a day to wash my hands....go to the restroom....wear gloves in cold weather....etc. I wear them when the pain is unbearable without them....mostly I just grin and bear the pain to do what I have to do. I am glad to see that new types of surgeries are being developed for D. My sister has D too....she worked as a hygenist for 30 years.

      Ithaca Comments I just had surgery on 30 Nov 2017. I was really worried that it would get worse. My wrist dont hurt as much. I can do things normally now.

    I'm more than likely getting this surgery. I have an MRI next week to confirm the problem. I'm terrified but at this point I can't deal with the pain or limitations. Doctor said its only a 5 minute procedure and should be back to normal.

      Maria, did you have the surgery? I have been watching EVERY yt video on how to massage, treat, stabilize. Last night I found one dated 2012. The first one cured, on the other side of the injury. I was giddy. Wanna know what he did? The guy took Glucosamine Chondroitin with MSM, and you know what? He was pain free in 10 days. It may not work for everyone, and everyone's level of inflammation/injury may differ. I keep it in stock for my dogs, so I said, what the hay! I took one dose today worked all day in the yard and the best part WAS I was ABLE TO. I still feel it, 7 hours later, but it is not as severe. It no longer feels like sledgehammer intensity. ONE DOSE. I am the first to encourage anyone with JOINT pain to try the trio. So I find it odd it never occurred to me to take it for this. Soft tissue still needs lube? I realize this is not the best etiquette leaving a non surgery suggestion of a supplement on a surgeons page ... at the risk of offending, I MUST! I am looking forward to sleeping through the night after a long hard labor intense day, and not startled awake when I move the wrist the wrong way.     three years after surgery I'm still feeling pain on my thumb and when I run my finger across my index finger i feel shock while I'm doing that which is on my right hand and the muscles going down my arm is very tired and hurts too plus my left thumb has been painful for the past two years .what's up with that

      Sounds like that rare complication with the nerve that can happen. Requires a follow-up surgery to resituate the nerve

    I developed de quervain's tenosynovitis last June while plying guitar and I haven't got rid of it yet. What should I do?
    Is surgery the only option left and if yes then will I ever be able to play guitar again?

      I got mine playing drums (and using a trackball)! I've received Medrol Packs (twice) and my second shot today. A couple weeks ago it was nearly gone! But the Dr didn't mention I should keep on the brace even when I felt better. So it's been creeping back - even though I haven't played drums, guitar or even piano. I even threw out my mouse and using an ergonomic keyboard and touchpad. Got my 2nd steroid shot today, and going to eat, sleep and live with this brace for the next couple of weeks till I see him again.
      There are a few alternatives. If it's insanely bad, get the shot and do the Medrol packs - and don't move that thumb and wrist!!! Also, soaking in hot water with Epsom salt seemed to help as directed by me PCP and PT Dr friend.
      I'm NO doc - so don't follow my advice LOL. go see a dr, and simply mention what I said here.
      I'm like you - I'm scared crapless of getting surgery. Not for the surgery itself - but I don't trust Dr's/surgeons

      @@monsterbeng1 and Nair: So I did wear the brace religiously - day and night. Only took it off to shower. By the time I saw the Dr, the pain was gone.I had other weird pains in my wrist from being in the brace so long, but that faded in a few weeks.
      The #1 thing I wanted to do was avoid it coming back - and I remembered something when it came back the first time. After a week or so, performing the Finkelstein test was getting worse and worse. My wrist started getting tight, and eventually the pain was back full force.
      So, after the 2nd shot, and after the brace was off, a few days later, I remembered that wrist tension (it was slowly coming back AGAIN) - and was desperately afraid it happening again. I refused to surrender to surgery. I used my left thumb (from my other hand), and softly placed it on my wrist (on the thumb side that obviously has the issue/pain). I formed my hand as if ready to the Finkelstein test again. Now, very, very slowly, I'd start bending my hand and the very moment it hurt, I'd place my left thumb right on the spot where it hurt (not pressing, just placing). Then bring my hand back just enough where it didn't hurt anymore. Then I pressed (as with massage pressure) on the previous painful spot and bent my right hand again. If I pressed hard enough, I could get my hand to bend all the way without pain. Sometimes, I had to do it little by little. Ex: I'd only be able to bend a little before the pain. So, I'd press until the next pain spot. Let my left thumb relax, pull back the bend and start over. Going a little further each time. I was always able to get to the full range of motion - even if it took a bunch of times.
      Then miraculously the pain and tension would go away for about 30 minutes to an hour. Every time the tension/pain returned, I repeated this.The hour of relief turned to two ,then to 5, etc until I'd only have to do it in the mornings. (Took a month or so to get to this point)
      I still do repeat the press/stretch to this day when I feel my right wrist is tensing a little. And especially before playing an instrument. But all in all - I'm happy it's 99% gone! I also took "Osteo Bi-Flex Triple Strength with MSM and 5-loxin" (and still do). But, not really sure if that worked - can't hurt. The press/stretch is immediate relief - so I know that does work - for me at least.
      Good luck to everyone who is going through this hell!!! I sincerely don't miss it at all, and really hope you get rid of it once and for all.

    I got surgery on right hand on 2014...and this year left hand...no need to wait after you get indication de quervain syndrome

    This came on out of nowhere on my right hand (and I am right-handed). OMG...and the thing is, I can still grip tightly and use my dumbbells to lift weights without pain!! What's going on?? Currently in the splint phase for about one month. It only gives me the stinger when I move thumb laterally AWAY from my hand...I ice it for 15 minutes every other day, also using Salon Pas type stick-ons. Scary.

      Iv have had this for about 2 months now. First sign was difficulty writing. At the gym doing barbell curls put slight strain on the affected area but it is bearable. Grip strength hasn't been hampered. Texting or moving the right thumb in a circular motion causes the tendon to snag around the wrist bone. It feels tight and sore. Splint doesnt help, resting it doesnt help either. I think surgery will be the answer. My job requires me to write at times, so I been practicing writing with the left hand. It's getting really annoying.

    Can anyone tell me how they did after the surgery? I'm about to have it and I'm freaking out. I am an artist and I'm right handed which of course is the wrist with DeQuervain. I'm so scared.

      Ryan Thack No, not yet. I'm so scared! Plus, I don't like medications and I don't want to be forced to take anything. So, I'm afraid of the pain, the chance that my sensory nerve could be snipped and risking not having full use of my hand ever again.

      Ryan Thack I know exactly how you feel. I'm at the point where I can't hold my toothbrush or write my name. I have no idea what to do. I got a shot a few months ago which lasted just over three months (I've been begging for another) but since I'm allergic to the numbing agent, my doctor won't do it again. He said it was too barbaric. I'm a nervous wreck but I can't function as it is.

      Ryan Thack Did you get the surgery? If you haven't, make sure you do. It was the best decision I ever made! I had no pain! I was a little achy the night of the surgery but nothing Advil couldn't help! Now I can do push ups again, draw, lift and build!!!!

      @@NilaL I just left another comment to Maria...long story shortened. Look up this video titled: DeQuervain's Tendonitis - CURED. I tried his advice today one dose and I was able to put my wrist through a long day of work. The key to that statement is: I WAS ABLE. Looking forward to repeating the dose tomorrow. : ) Give it a try. It may no work for everyone but worth the effort before the cut.

    Small tiny incision? I watch your definition of a small tiny incision? I just had this surgery 2 and a 1/2 weeks ago and my small tiny incision is 2" long. In my mind a small tiny incision is an inch or less and/or less is really a small tiny incision. Mine is an S running diagonal across my wrist

    Do not get the surgery . One of the worst decisions of my life. It's fine if your elderly or inactive, I lost all strength in my hand years after. Still gives pain.

    Question please. Aside from the pain I feel when I move my thumb or wrist in certain ways, when I stretch my thumb out, I feel something popping near the base. Also when I twist my wrist, I feel something popping on the top side of my wrist. Is this common with this condition? I have had x-rays done and they are negative for any fractures or breaks. Thanks

      Hi, same thing with me. From what I understood from my Ortho, when we extend them and then contract them, they pop bc they are swollen and trying to vote through the sheath and getting caught. My surgery is May 30th, after months of pain. I'm sure this all goes back to an old high school Injury and then life just happened the rest of the years. My xray and MRI were all negative also. I'm REALLY looking forward to no pain and using both hands again.
