Different causes of tubal blockage and its implication
•Fallopian tubes are very important structures of female reproductive system
•It sucks the ovum in after ovulation and thus helps the fertilisation process which occurs inside one of the tubes
•In case of complete blockage of both tubes, pregnancy fails to occur
•Partial blockage of tube can lead to ectopic pregnancy
•Ectopic pregnancy can be very dangerous if not detected early
•Normal conception is possible after treatment of most cases of proximal blockage
•In Vitro Fertilisation (IVF) is the best possible treatment for distal blockage
•Recanalization of tubes are possible but success rate depends on many factors
Fallopian tubes are very important part of female reproductive system. It pulls the ovum inside by suction, after ovum comes out of the ovary at the time of ovulation. Thus, it helps the fertilisation process to be done inside its lumen. If both the tubes are blocked, fertilization cannot occur, as ovum and sperm cannot come close to each other.
The part of tube closure to the uterus is called proximal and distant part is called distal part. Blockage in proximal part may be due to mucous plug. Sometimes, tubes close their opening by spasm which falsely detected as block while doing hysterosalpingography, where a radio-opaque dye is pushed through tubes to document its patency. It may be in response to pain. Laparoscopy is more definitive process to detect tubal blockage as it is done under anaesthesia and pain stimulus can be avoided all-together. Mild endometriosis and fibrosis are also common factors for proximal tubal blockage. On the other hand, salpingitis isthmic nodosa, pelvic inflammatory disease and tuberculosis are the common causal factors for distal tubal block. Unfortunately, the reasons for distal tubal blockage are not very easy to cure.
Proximal tubal blockage due to mucous or spasm can be easily detected and treated by hysteroscopy. If the reason is mild endometriosis or fibrosis then also the treatment can be done by hysteroscopic dilatation by passing guide-wire. But in case of distal blockage the prognosis is quite bad. Laparoscopic tubal surgery also has a limited role and success. Sometimes, there may be partial blockage of tubes. In these cases, chances of tubal ectopic pregnancy are more. Inside the tube, zygote is developed by union of ovum and sperm and normally zygote move towards the uterine cavity after fertilisation and this movement is aided by tubal peristaltic movement. If there is partial block of tube or tubal motility is less due to damaged tube, Zygote may not be able to reach the uterine cavity and may start to grow inside the tube. Uterus is the organ which has enormous ability to expand and grow along with the development of the growing foetus. But tubes do not have that capacity. So, if zygote fails to reach the uterus and starts growing inside tube, it eventually disrupts tube and cause internal bleeding. This is known as ruptured tubal ectopic pregnancy, and it is a life-threatening situation. Heavy internal bleeding, severe pain abdomen, low pressure, fainting are some of the common symptoms of ectopic / tubal rupture of pregnancy.
In Vitro Fertilisation (IVF) treatment is the best option for distal tubal block as the prognosis of laparoscopic surgery is not very good. In IVF the formation of zygote takes place outside the body and cultured for 5 days, then transferred to the uterus for further development, thus tube can be bypassed.
Recanalization of tubes may be possible if sterilisation / ligation process done in a particular way, where there was no thermal damage or where tubal loss is less than 30mm. Otherwise success rate is not very promising in those cases IVF is the best possible treatment.
In summary, proximal blockage can be treated with hysteroscopic procedure but distal blockage may be difficult to resolve. Recanalization is not possible in all cases of tubal ligations and IVF may be a good option for many.

আমার স্ত্রীর 23/02/23 এবং 26/02/23 দুইবার hsg করার চেষ্টা করলেন ডাক্তার বাবুরা মিলে, কিন্তু সম্পন্ন হলো না। কারণ, প্রথম মুখটা খোলা, দ্বিতীয় মুখটা বন্ধ। তার উপর দ্বিতীয় মুখটা নিচের দিকে অথবা উপরের দিকে। প্রায় 14 মাস বাচ্চা নিতে চাচ্ছি, বাচ্চা আসছে না। 1 বছর ৮ মাস আগে একটি বাচ্চা নষ্ট করে ফেলেছিলাম, সেটাই ছিল প্রথম। আমাদের বিয়ে হয়েছে 23 মাস। রিপ্লাইয়ের আশায় থাকলাম।
