The biggest fail you could have in minecraft: you are so prepared for the ender dragon and you're about to open the portal, but just before you place down the last eye, a skeleton comes and shoot you directly in the lava.
Or when you are on pe and u turn around to put your final eye just to realize your ender eye activated cuz u hold your screen for too long
