There are two strains to the virus:
The Flower & The Cannibal Strain.
The king, Lord Ahn, and the people at the village that were unleashed upon the Japanese army, they were all specifically made into the undead with the resurrection flower one by one. Since the original Flower Strain isn't infectious, anyone that were bitten were never turned into zombies themselves. That's why the physician's assistant, Cho Hakju, and the Japanese army were never turned into zombies.
But it was when the physician's assistant was eaten by the people at the clinic, the virus mutated into the infectious Cannibal Strain, and this strain was still being contained within the southern provinces.
These two different strains co-exist on the show, and it's a fascinating concept.
But it does get confusing as they only explain this concept in little tidbits over the course of several episodes, and you could totally miss it, and wonder why Cho Hakju conveniently got the luck of the draw and never got turned into a zombie.
However, if you follow the two strain logic, Cho Hakju was never in danger of getting turned as he was bitten by Lord Ahn who had the original Flower Strain of the virus.
The reason why the court ladies are so devoted to her is because they are either the ones that cared for her since her birth as nannies or wet nurses or even personal servants. This case is very rare, but not uncommon. Since girls those days had to go through hell since their childhood to become a Palace Court lady, it is rare that a court lady would be brought from outside. But given how powerful the Haewon Cho clan is they might have sneaked in the queen's personnel with her. OR on the contrary they might be of Cho clan. Some clans send their daughters to work at the Palace since it is a great privilege those days and queen's maids might have been Cho clan girls that grew up working at the Palace but utterly loyal to their own clan. Hence the loyalty to the queen.

Yes usually the Queens maids/court ladies are supposed to be from their clan because that would assure loyalty to the nobles in the clan. This was across most ancient cultures including India where Queens from distant lands would come with their own servants, maids and ladies in waiting because they would know her and her tendencies the best. You couldn’t trust some other clans women to care for her 100%
I am so glad that you guys are enjoying Kingdom. This is my favorite out of all the zombie series, and I have watched many zombie stuff. This is the best one out of all for sure.

In answer to Hunter’s question at 19:22, the women and children were killed to get rid of evidence of the crimes of the queen.
If they were kept alive wouldn’t it just look like a normal stay for pregnant woman? Then there wouldn’t even be a ‘crime’

Young-Shin, the tiger hunter, is from Sumang village where Hasen disease patients lived together.
They had bandages on their faces and that implies they are Hansen disease patients.
Cho Hak-Joo decided to sacrifice them to fight against the Japanese.Deok-Sung who killed himself to block the zombies at the watergate said sorry to the tiger hunter because he killed people in Sumang village and he knew the tiger hunter was from that village.18:42 It is natural for Hunter to have difficulties recognizing Korean women.
