    why use the pillows for flexion when the patients chin is a mile in the air. So their neck is in severe extension.

      It looks like a lot, but it was a very subtle application of the technique at the end. This guy does not appear to be running around causing strokes with his hvla technique.

      The subject has a lot of range of motion so you'll have to go further into side bending and rotation to actually lock the joint.
