The sound is alternate wail. This sound is used on epws 612, 1212, etc. The siren itself: no idea. Just search "alternate wail siren" and chanses are you will find something thats sounds exactly the same.

2004 we're visiting downtown Chicago from California. A wild thunderstorm (even by Chicago standards) blows through. All of the sudden one of these is going off down the street. Totally freaked out my wife.

I heard somewhere that this specific wail is used in places such as Chicago because the different tones (at least one of the frequencies) travels further so there doesn't need to be as many sirens, which seems reasonable to me but I'd like to hear what others have to say on it.

Sounds like the T-Bolt in downtown Cincinnati is still active as well. It's interesting that they're using Alternate Wail for testing now. Last time I was down there for a test a few years ago they were doing Westminster Chime for tests.
