I am a pediatric resident and this is the most helpful and well explained video i have seen about PPHN; especially the Pre and post-ductal saturation. Thank you so much Dr Tala, Greatings from Egypt❤️
Hi Ahmad (or rather Dr. Mamdouh). We're so glad it was informative: it is not an easy concept to grasp, so I'm impressed you're committing to sucking it all in during residency. Please let us know if there's anything else you'd like us to film? Shukran!
Dr. Tala, you are truly wonderful. Please do not stop this teaching, it is wonderful! You have helped me immensely with these videos! Greetings from Romania! P.S: In a week, I will have my specialty exam and what I have watched from you will certainly be very helpful to me! Thank you from the bottom of my heart! 🙏🏻 You are amazing ! I am convinced that you are an extraordinary doctor! I send you heartfelt hugs! 🤗
Thank you! What lovely compliments! So happy you are watching from Romania- we wish you luck and strength for your upcoming exam. (I think that the mere fact that you're looking to reinforce your education in other ways is super impressive, and you'll be great!)
Im a NICU nurse and I have never seen anyone deacribe this so well! I have been using Dr. Tala's videos to study for the neonatal CCRN. She does a great job of breaking down complex concepts so you can understand them.
Oh Nicole- needed your comment today!! Thank you so much for taking the time to write! We were tired over here so it’s so lovely to get such great positive reinforcement! Really thank you!!!!
This is the first time I have seen a demonstration of a heart using a square illustration. It is actually a lot easier to understand. The way she describes and explains everything clearly and simple is just remarkable.
Nice discussion. As an icu paeds nurse i really appreciate your efforts making it simple and fun to learn. All the curiousity has been answered in just within 30 minutes. Greets from National Heart Institute of Malaysia!
HELLO! Wow- thanks so much for watching and for writing to us and for subscribing! We are so happy this was helpful! And from Malaysia- I need a map to plot where all our viewers are from!! Thank you!
Hi, Dr Tala. I am a neonatal nurse from Hong Kong whom is now working for an NGO in Bangladesh for both hosts and refugees. I have been watching your videos every day since a few weeks ago since they are informative, easy to understand and very practical, it's incredible how you made some difficult medical concepts/ knowledge so easy and fun to understand. I was watching some other youtube channels about paediatrics before but your channel is my absolute favourite😊 love the way you teach and how cheerful you look all the time, also you are replying every comment patiently. You are definitely a great teacher and I cannot thank you and your team enough for making such amazing free videos for everyone to learn at home🙏 I will continue to follow and support your channel, as well as recommending your videos to nurses and doctors here in Bangladesh (I am a staff trainer here). I am still catching up on your old videos but if there are some new topics I would like to suggest I will let you know☺️ I am looking forward to your new videos. Thank you again and hope you are having a good day. PS I am commenting on this video because I never fully understood PPHN until I watched this.
This was one of the nicest notes we’ve ever received! Thank you so much! For all of it! Watching the videos and taking the time to write such lovely comments! We’re so touched!!!! Everything you touched upon is what we’ve been trying so hard to achieve- so it all means even more! Thank you for your support- and impressed with your efforts to further your education!!! And please let us know what videos you’d like next!!! Always open to suggestions!!! Xx
Ha! That's a massive compliment- although I think that may be secondary to the interesting (and clinically applicable) subject matter rather than than the lecture itself- but thank you so much for watching and commenting!
Dr. Tala you have a gift for teaching! This was so helpful! I had a patient this past weekend (I'm a student nurse in my last semester, precepting in the NICU) with PPHN, and so many words were thrown around that I didn't know the implications of. I feel like I have a solid grasp on her case now. It's so amazing to me to see how PPHN affects the entire body & to now understand the neonatologist's general thought process when deciding treatment. Thank you!!!
Hi Trisha- thanks so much for your lovely comment and your compliments. SO HAPPY that you feel like this filled the gaps- thank you so much for taking the time to write and for subscribing :)
You are such a great educator! On my last semester of nursing school and you delivered a better lecture than all of the professors I had throughout the program.
Excellent video! Thank you so much Tala, Arianna, and Justin. I am a NICU nurse and also in school to become a nurse educator. All of your videos and lecture content are so well done and inspire me in my future role as a nurse educator in the NICU. Keep up the excellent work - you are helping so many! Thanks again.
Thank you so much Caroline- that means so much coming from a NICU nurse and soon to be educator! Also thank you for acknowledging Arianna and Justin in this process- as you well know from your time in the NICU, every tiny thing we do is a massive team effort! Different aspects fall more heavily on different members of the team each moment! Let us know if you have any suggestions or advice for us! Thank you again!
You're the best. You have a gift for teaching. Working in a NICU.... Without an internship has been difficult at times. Your lectures provide the classroom education that I have missed. Thank you so very much
Hello Suzy! I can't imagine what it must be like to get thrown in like that- so REALLY well done you for persevering and trying to make up the eduction anyway possible! This pandemic has had so many effects that we all didn't even anticipate when it started- so it's great to see everyone make the best out of a really sad situation. We hope you love it as much as we do!! And let us know if there's anything in particular that may be helpful to a relatively new NICU nurse. (We're thinking of starting a more symptoms based series- like- your baby has green emesis- what do you do next? type thing)....Anyway- thanks so much for watching and for your lovely words!
Love love love your lectures. I’ve been a NICU nurse for 19 years and I always learn something from you! Wish all neos would share their knowledge with the nurses like this. Would make for stronger/better units.
What a lovely comment Lucy!! 19 years - go you! Also so so happy that you feel like you’re still learning something! Thank you so much for watching and taking the time to write! We’re all so busy in unit so it’s been really nice to have this way of educating on a larger scale. But I agree- even little pearls from different members of the team help xxx
This was such a great explanation. I work as an RN at a level 4 and recently lost a baby to PPHN crisis (GA 25-->37wks). We tried absolutely everything. Baby was 2.1kg and just barely qualified for ecmo. She ended up being too small to cannulate (the surgeon tried everything). So thank you, this really helps me to understand what happened 😥
Oh I’m so sorry- those are really tough situations :( and after baby was 3 months old- makes it even harder. Poor baby and poor family and poor staff who definitely got close to the baby too. I’m glad this lecture helped in some way
probably too late for me to comment on tthis lecture, im glad i encountered this Tala talks, i finally understand 100 % of PPHN, whewwww thank you Doc Tala.
Thank you so much for this video! Really wish you had a podcast! You’re such a great teacher and it would be so helpful to be able to listen to you on the go as a new nicu nurse.

thank you so much dr. Tala, i've been spending hours trying to figure it out and understand better about PPHN, but here you are enlightened me with a video less than 30 mins. Greetings from Indonesia
That makes us SO happy- love hearing about ah-ha moments where it just clicks :) So happy you're watching from Indonesia :) Thanks for taking the time to comment!
thank you for making a great video!! I’m a new nurse in the NICU working in Korea. PPHN was always difficult, but thanks to you, i could easily understand it. thank you😆
Thank you so much, Dr. Tala. Because of your lectures, it’s easier to understand topics like this. And now, it is easier to teach them to my students with your “square heart” lol. Please continue making videos like this.
First- we love that name! That's what we should all be! Diligent students! And thank you- really appreciate that anybody is watching. It has been weird how many people we have worked with in the past who have casually told us they've been watching!!! Thank you so much though- we appreciate you taking the time to comment :)
THANK YOU! Great work! I am on first week of pediatrics rotation in 3rd year clerkships, this has been incredibly helpful to develop my understanding of PPHN. Wow! This was also FUN to watch, I really enjoyed learning from this video and it was so energetic and highly relevant throughout. I was just memorizing before but now I have a much better and more vibrant understanding. Also really enjoyed the Tetralogy lecture. Whew, excited for more!
What a lovely lovely comment- thank you SO much! Literally no better feeling than helping someone have fun while learning -and truly understanding- not just memorizing. (Even if using the word fun loosely! ha!). Another viewer asked us about pediatric youtube channels we'd recommend, and I didn't really know what to say. Would you have any recommendations for them you've used in medical school?
@@TalaTalksNICU Serious fun! And therapeutic bc I had these concepts loose and disorganized floating and fading in my brain, it feels so good to have a better understanding after watching your lectures! It seems like you've really tried experimenting and explaining this in different ways to see what makes the most sense to patients and your students. This is creme de la creme! Makes me want to go into peds! I only wish I could slow down time to have time watch all your videos and be more ready for rounds! This is the best lecture ever! I am a paediatric residency and your lectures are so helpful and making my residency smooth! Can you also make a lecture of anemia of prematurity! Thnks alot Dr Tala.
Thank you so much Dr. Hassan- that is quite a positive statement!!! We really really appreciate it- and we do need to circle back to anemia of prematurity and transfusions (especially that there was a relatively recent study published). For now, maybe check out the anemia lectures?
Thank you again. All the parts in this lecture are praiseworthy. In the diagnosis part i am a bit amazed to knoe about right ventricular hypertrophy, because hypertrophy is usually a slow process, and right ventricular dominance to some extent is expected in neonates. Correct me if i am wrong.
Thank you so much for your lovely comment! You’re right- the heart is right dominant after birth (on an EKG etc) but the RV shouldn’t be larger. It doesn’t take long for that right side to look thicker and become hypertrophied (days- weeks) but you’re right - the other stuff happens first.
One of the best lecture on pphn , very well descripted with easy language so very helpful to understand , trying to learn more from your vedios , thank you so much from India 🇮🇳 Dr sandesh MD ped
Thank you so much! Detailed explanation is great! Helps to understand this difficult process that God created. Thanks a lot Dr Tala, greetings from U.S.
Ha! Glad it saved you time and thank you so much for writing in. I'm sure there's a lot more to learn in the protocol though!! Thanks so much for being here!
Thank you for a great presentation!!! I am an NP in the NICU and it is the best PPHN video and teaching I found! Do you plan on making more videos covering NICU electrolyte management, HIE, BPD ect? this would be highly appreciated by your subscribers!
Thank you so much for your kind words (and for all you do in the NICU!). We are hoping to get HIE and BPD out soon, and then start a TPN/nutrition/electrolytes series. Thanks so much for watching and commenting!
What an excellent presentation with clear concept and straightforward explanation! Thx for all the effort and time. Thx again for all the sharing, Dr Tala!
very informative. thank you so much .... my pediatrician told me that I should check saturation first at right hand but didn't explained me y . got it here ..
I'm pediatric resident and you've really made this difficult topic interesting for me. Will never forget it hopefully. Thanks for this detailed discussion. ❤️
What a great explanation of the physiology and function of the heart and PPHN! I truly and finally understood it. Could you go over the whole of lung physiology, dead space, PACO and PECO, specifically FEV and FVC, also Bohr and Haldane effects?
Thank you Karen for your lovely comment! Can I ask what field you’re in? I’d LOVE to cover those but I’m wondering whether they’re useful for people to understand? Arianna and I have discussed this a couple of times!
Thank you Dr. Tala. This lecture will really help me in my pediatric residency here in the Philippines. FYI, TALA means STAR in Filipino. You are really a star for helping us 🌟
Hi Dr. Charissa! Wow! I did not know that about the name Tala! I've always been told it means a little palm tree- so this is a huge upgrade! ha! Thanks so much for watching and commenting. Good luck in the rest of your residency :)
Thank you so much for all the videos, especially this one, I have watched it many times and it is really helpful and clear. If I may, I am having difficulties in understanding the pathophysiology of TOPS (twin oligoamnios polyhydramnios sequence), especially the neonathology consequences. I don t know if it was already specified by someone else, but I think it will help a lot of people with the approach after birth for this particular case. Thank you very much for all the work that you do, I am a neonates resident from Romania who practices in France and I can tell you that it is a relief to watch you and truly understand the pathophysiology.
Hello Dr. Pasat- so happy that these videos are helping at all! Going over the pathophysiology of TOPS (TTTS) is a great idea. We'll try to get that out sooner rather than later- thank you SO much for the suggestion- and it wasn't on our list! (Also- ps your English is exceptional! Romanian, French and an English understanding of medical terms is truly remarkable!!)
I can see why this is the most watched video in this channel. Brilliant lecture! I never thought I would learn so much from a square heart! 😅 PPHN has been one of the conditions that always gets me confused. Now I can confidently say I get it now and I can explain it to others as well. I have learned SO MUCH from this. Thank you Dr. Tala and your team! 😊
Hello and thank you again so much BabyBecs :) I love those square hearts too! Impressed how you've been plodding through these videos- and so happy you feel you're learning!!
One of the most excellent discussions and teaching I have had related to this topic. Well done....

Hi Dr. Charissa!

That was crystal clear!!!💜😍
Oh so happy you found it so!!! Thanks so much for taking the time to comment!!
