  • After 8 years of school and 1 year of residency, I finally accomplished my goal of becoming a clinical inpatient pharmacist! I'll be recording my pharmacy journey - navigating life as a first time pharmacist fresh out of my pharmacy residency.
    Today I will be talking about how to verify IV orders and how to check/verify different IV medications (IV mini bags, IV duplex bags, IV premade bags, and IV compounded bags).
    00:54 How to verify an IV antibiotic order
    03:59 IV minibags
    06:30 IV duplex bags
    08:00 IV premade bags

    Hi Miss Sarah new subs here. I was so impressed on your video relating about the days as an IV tech at the facility I had work before . So much fun everything you’d said related on my job practice. Being an IV certified tech it was fulfilling looking back the days how busy but at the end of the shift you have help someone and keep their wellbeing again. Thanks for our job. GodBless

    Love this video and how your thought process is for a medication order entered, allergy checks, retiming meds, renal adjustments, etc. This is the first video I came across regarding a real-life clinical pharmacist with a lot of helpful information! I feel a little more confident since watching your videos and have a good idea of what to expect, especially your nightshift videos. Thank you!

    You can make a video of what the colors mean to divide the medicines that will be dispensed.

    👋🏼🙋🏼‍♂️👋🏼 future pharmacist here. Quick question, you mentioned gen Vimpat. Isn’t the dosage on that according to the pts BMI and the level of the seizure? Whether it be a grandma seizure, epileptic seizure? Like stuff like that?

    • @SarahPharmD
      @SarahPharmD  Год назад +3

      Yes, the dosing is lacosamide is complex and can vary depending on the type of seizure, renal function, provider preference in terms of titration, weight, etc! When verifying orders, I make sure the dose is appropriate and ensure that the technician compounds the IV lacosamide correctly (math calculated correctly, correct amount injected into bag, solution is compatible, BUD is correct, etc)

    Does acyclovir dosing require Adj body weight or IBW? I think it was IBW ...

      @SarahPharmD  2 месяца назад

      Dosing weights for acyclovir are controversial! The original package insert recommends dosing on ideal body weight, but now subsequent studies suggest adjusted body should be used to reduce risk of underdosing. Each hospital should have a protocol in place so I would follow what the hospital protocol is!

      The cost of living is high where she lives. May be better to get paid low but everything around you is affordable such as housing and food. A $20,000/year in an affordable area may be an equivalent to $100,000 in California.
